    • The incidence of social isolation in Chinese elderly population: a meta-analysis

      ObjectiveTo systematically review the incidence of social isolation in Chinese elderly population. MethodsCNKI, WanFang Data, VIP, CBM, PubMed, EMbase, Web of Science and CENTRAL databases were electronically searched to collect studies on the incidence of social isolation in China from inception to May 2022. Two reviewers independently screened literature, extracted data and assessed the risk of bias of the included studies, then, meta-analysis was performed by using Stata 15.1 software. ResultsA total of 20 studies involving 86 111 subjects were included. Meta-analysis results showed that the incidence of social isolation among the elderly in China was 27.54% (95%CI 22.15% to 57.74%). The results of subgroup analysis showed that gender, age, educational level, marital status, self-assessment of health, living style, year of publication, and region surveyed were all influential factors of the incidence of social isolation among the elderly in China. ConclusionCurrent evidence shows that the incidence of social isolation among the elderly in China is relatively high. Due to the limited quality and quantity of the included studies, more high quality studies are needed to verify the above conclusion.

    • The Treatment of Colorectal Carcinoma With Acute Colonic Obstruction in the Elderly: A Report of 78 Cases

      目的:探討老年結直腸癌并急性腸梗阻的處理原則和方法。方法:回顧性分析10年間收治的78例(70~80歲)結直腸癌并急性腸梗阻的臨床資料。78例中Dukes B期20例,C期18例,D期40例。全部患者經手術治療,包括急診手術46例,擇期性手術32例。采用右半結腸一期切除腸吻合治療30例,左半結腸一期切除吻合40例。Hartmann 5例, Di×on手術2例,腫瘤近端腸管造瘺1例。結果:術后出現并發癥16例(20.5%)21例次,包括切口感染14例次,腹腔感染5例次和吻合口瘺2例次,死亡3例(3.85%),75例(96.2%)痊愈出院。結論:對于急性結直腸癌性梗阻除非有急診手術指征,應首先采用非手術治1~2 d,盡可能轉為擇期手術;一期切除吻合治療結直腸癌并發急性梗阻,是方便可行而安全有效的方法,加強圍手術期處理是手術成功的關鍵。

    • Association of Arterial Stiffness with Cardiovascular Risk Factors: the Northern Shanghai Study

      ObjectiveTo investigate the association of arterial stiffness with cardiovascular risk factors. MethodsFrom July 2014 to August 2015, 1 599 participants aged over 65 years old were recruited from 10 communities located in the northern Shanghai. Carotid-femoral pulse wave velocity (cf-PWV) was measured by SphygmoCor device. Cardiovascular risk factors were assessed by questionnaire, anthropometry or conventional biochemical measurements. SPSS 20.0 was applied for data management and statistical analysis. ResultsCf-PWV was significantly associated with cardiovascular risk factors (P<0.001). Moreover, with the accumulating cardiovascular risk factors, cf-PWV increased gradually and significantly (P<0.001). The increasing trend remained significant in all participants, in men and in women, respectively. ConclusionArterial stiffness is significantly associated with cardiovascular risk factors and their accumulation.

    • Association of cognitive frailty with mortality and hospitalization in older adults: a meta-analysis

      ObjectiveTo systematically review the associations of cognitive frailty with mortality and hospitalization in the elderly. MethodsThe VIP, PubMed, CNKI, WanFang Data, CBM, Embase, Cochrane Library and Web of Science databases were electronically searched to collect cohort studies on the association of cognitive frailty with mortality or hospitalization in the elderly from inception to May, 2023. Two reviewers independently screened the literature, extracted data and assessed risk of bias of the included studies. Meta-analysis was performed by R 4.2.2 software. ResultsA total of 19 cohort studies involving 63 624 elderly were included. The results of meta-analysis showed that compared with healthy elder, the elder with cognitive frailty had a higher mortality (OR=2.75, 95%CI 2.10 to 3.59, P<0.01) and hospitalization (OR=1.67, 95%CI 1.40 to 2.00, P<0.01). Subgroup analysis showed that cognitive frailty was related to the risk of death in different status of frailty and cognitive function, different assessment tools, different countries of development, different follow-up time and research sites. At the same time, different status of frailty and cognitive function and different levels of development of countries were related to the risk of hospitalization. ConclusionCurrent evidence shows that cognitive frailty can increase the risk of hospitalization and mortality in the elderly. It is suggested that early screening and intervention of cognitive frailty should be carried out to effectively reduce the risk of adverse consequences, so as to achieve healthy aging.

    • AN INVESTIGATION OF AGE-RELATED MACULAR DEGENERATION BY USING DENSE MATRIX MAPPING IN MACULA

      PURPOSE:To approach the changes of visual function by using the dense matrix mapping in macula(DMMM)in patients with age-related macular degenerationl(AMD). METHODS:The DMMM was designed using Humphrey-640,thee study included 51 patients(71 eyes)in dry type,AMD,23 patients(27 eyes)in wet type and 16 normal elderly(21 eyes). RESULTS:In 2.5 degree of macula:the mean light sensitivity(MLS)was 35.61plusmn;1.37 in normal elderly.33.30plusmn;2. 49 in dry type group and 20.67plusmn;8.30 in wet type group,and differences were highly significant statistically among them. In S degree the MLS is 34.93plusmn;1.46 in normal elderly.32.95plusmn;1.82 in dry type group, 22.11plusmn;7.74 in wet type group ,and the differences were highly significant statistically among them. CONCLUSIONS:The MLS of DMMM declined at early stage of dry type AMD and there was relatively high sensitivity of MLS of DMMM in evaluating the visual function in early AMD.     • Frailty status and its influencing factors in elderly Parkinson’s disease patients

      Objective To understand the frailty status and main influencing factors of elderly Parkinson’s disease (PD) patients. Methods The elderly PD patients who attended the Department of Neurology of Changshu Hospital of Traditional Chinese Medicine between November 2023 and March 2024 were selected. The patients’ frailty conditions were investigated using general information questionnaire, Chinese version of Tilburg Frailty Indicator, Hoehn-Yahr Rating Scale, Mini-Nutritional Assessment Short Form, Movement Disorder Society-Unified PD Rating Scale Part Ⅲ, PD Sleep Scale-2, and Mini-Mental State Examination. Multiple linear regression analysis was used to further determine the influencing factors of the frailty status in elderly PD patients. Results A total of 170 PD patients were included. Among them, 117 cases (68.82%) had frailty, while 53 cases (31.18%) had not frailty. The average score for frailty was (6.48±3.34) points, the average score for nutritional status was (11.89±1.65) points, the average score for motor function was (27.40±13.73) points, the average score for sleep quality was (16.05±7.76) points, and the average score for cognitive status is (26.25±4.51) points. The Pearson correlation analysis results showed that PD patient frailty was positively correlated with motor function and sleep quality (P<0.01), and negatively correlated with nutritional status and cognitive status (P<0.01). The results of multiple linear regression analysis showed that age, education, place of residence, course of disease, Hoehn-Yahr Rating, nutritional status, motor function, cognitive status and sleep quality were the influencing factors of frailty in PD patients (P<0.05). Conclusions Elderly PD patients are prone to frailty. Healthcare professionals should pay attention to early screening for frailty in this population and provide timely and effective interventions to prevent or delay the onset of frailty in patients.

    • Mediating effect of elderly people's evaluation of environmental protection work on the relationship between natural environment conditions in residential areas and subjective well-being

      ObjectiveTo explore the relationship between the natural environment of residential areas and subjective well-being of the elderly and the role of the elderly’s evaluation of the environmental protection work in both. MethodsBased on the China social survey database (CSS) in 2019, Rstudio and Stata software were used to process the data screened according to the restricted conditions, and multi-classification logistic regression analysis and Bootstrap mediation effect test were used to test the effect relationship among variables. ResultsAir pollution, water pollution, noise pollution, land pollution, electromagnetic ionizing radiation pollution and other pollution in residential areas have no direct effect on the subjective well-being of the elderly (P>0.05). The evaluation of the elderly on the environmental protection work had a positive impact on the elderly’s subjective well-being (P<0.01) and played a completely mediating role in the impact of the natural environment in the residential area on the elderly’s subjective well-being. ConclusionThe government should continue to play a leading role in coordinating environmental protection and pollution control work. In addition, establish an open and transparent environmental protection information disclosure system and strengthen communication with the elderly.

    • 維生素D佐治老年人社區獲得性肺炎的臨床研究

      目的探討維生素D(VitD)佐治老年人社區獲得性肺炎(CAP)的臨床療效。 方法選擇老年CAP患者104例, 隨機分為VitD治療組和安慰劑對照組, 每組各52例, 以同期體檢的78例健康老年人作對照。應用酶聯免疫吸附法檢測血清中25羥維生素D3[25-(OH)D3]水平, 應用免疫透射比濁法檢測血清中免疫球蛋白A(IgA)及C反應蛋白(CRP)水平, 記錄和比較CAP患者的臨床癥狀、體征、實驗室、影像學檢查的變化情況及住院天數。 結果CAP患者治療前血清25-(OH)D3、IgA和CRP水平分別為(25.22±8.68) nmol/L、(0.28±0.06) g/L和(119.50±21.76) mg/L, 其中血清25-(OH)D3和IgA水平均明顯低于正常對照組(t=25.213, P<0.05;t=22.280, P<0.05), 而血清CRP水平顯著高于正常對照組(t=-47.114, P<0.05), 且CAP患者血清25-(OH)D3水平與血清IgA水平呈正相關(r=0.829, P<0.05)。同安慰劑對照組相比, VitD治療組患者血清CRP水平降低, 臨床總有效率提高了34.4%, 住院天數縮短了25.1%, 且三項指標組間比較差異均有統計學意義(t=5.017, P<0.05;χ2=4.248, P<0.05;t=6.321, P<0.05)。VitD治療組患者治療后血清25-(OH)D3和IgA水平分別為治療前的2.84倍和1.96倍。安慰劑對照組患者治療前后血清25-(OH)D3和IgA水平差異無統計學意義。 結論VitD缺乏可能是老年CAP的潛在病因, 補充足量VitD可緩解CAP患者臨床癥狀, 明顯縮短病程。

    • Short-term effectiveness of Gamma 3 U-Blade system for osteoporotic intertrochanteric fractures in the elderly

      ObjectiveTo compare the short-term effectiveness between Gamma 3 intramedullary nails and Gamma 3 U-Blade system in the treatment of osteoporotic intertrochanteric fractures in the elderly. Methods A retrospective analysis was conducted on the clinical data of 124 elderly patients with osteoporotic intertrochanteric fractures, who were admitted between February 2020 and February 2023 and met the selection criteria. The fractures were fixed with Gamma 3 intramedullary nails in 65 patients (control group) and Gamma 3 U-Blade systems in 59 patients (UB group). The differences between the two groups were not significant in age, gender, body mass index, American Society of Anesthesiologists (ASA) classification, bone mineral density, time from injury to operation, fracture classification, and affected side (P>0.05). The operation time, intraoperative blood loss, hospital stay, and fracture healing time were recorded; the tip-apex distance, fracture reduction quality, and lag screw position were evaluated on X-ray films at immediate after operation; the lag screw sliding distance and change value of neck-shaft angle were measured on X-ray films at last follow-up. Harris hip score at 1 year after operation and the occurrence of internal fixation-related complications were compared between the two groups. Results The operation time, intraoperative blood loss, and hospital stay in the UB group increased compared to the control group, but the differences were not significant (P>0.05). All patients in both groups were followed up 12-24 months (mean, 17.1 months). At 12 months after operation, there was no significant difference in the Harris hip score between the two groups (P<0.05). Radiological examination showed that there was no significant difference between the two groups (P>0.05) in terms of tip-apex distance, fracture reduction quality, and lag screw position. Fractures healed in both groups, and there was no significant difference in healing time (P>0.05). At last follow-up, the change value of neck-shaft angle and lag screw sliding distance in the UB group were significantly lower than those in the control group (P<0.05). During follow-up, no related complications occurred in the UB group, while 6 cases (9.2%) in the control group experienced complications, and the difference in the incidence was significant (P<0.05). ConclusionFor the osteoporotic intertrochanteric fractures in the elderly, the Gamma 3 U-Blade system fixation can achieve good short-term effectiveness, with better imaging results compared to Gamma 3 intramedullary nails fixation.

    • Correlation between Metabolic Syndrome and Arterial Stiffness in Elderly People: A Population-based Study in Communities of Northern Shanghai

      ObjectiveTo investigate the association between metabolic syndrome and arterial stiffness in elderly people. Methods1 599 participants aged over 65 years old were recruited from 10 communities located in the northern Shanghai. Carotid-femoral pulse wave velocity (cf-PWV) of each participant was measured by SphygmoCor device. Measurements for the diagnosis of metabolic syndrome were all investigated for each participant. SPSS 20.0 was used for data management and statistical analysis. ResultsCf-PWV was significantly associated with metabolic syndrome and its diagnostic measurements (P<0.001). Moreover, with the accumulating diagnostic measurements, cf-PWV increased gradually and significantly. The increasing trend remained significant in all participants, in men and in women (P<0.001). ConclusionArterial stiffness is significantly associated with metabolic syndrome and the accumulation of its diagnostic measurements.
