      冷凍治療早產兒視網膜病變(ROP)研究(CRYO-ROP)為美國國立眼科研究所于1986年發起的一項設計嚴密、覆蓋面廣的前瞻性多中心、隨機對照臨床試驗。該研究觀察周期長達15年,開創了ROP臨床研究的先河。雖然冷凍治療現已被激光光凝治療逐步取代,但該研究得到的許多客觀可信的結果對目前認識ROP仍具有指導意義;該研究的設計理念和研究方法更值得借鑒和參考。

    • 感染和炎癥與早產兒視網膜病變

      早產兒視網膜病變(ROP)的發生發展受到多種細胞因子、細胞外基質成分以及生長因子網絡等的緊密調控。除了低出生體重、小胎齡和分娩后補充用氧以及維生素E水平不足、吲哚美辛治療動脈導管未閉、高碳酸血癥、輸血及血液制品、孕母服用阿司匹林等因素與其發生發展具有較大的相關性之外,近年研究發現,產前宮內感染和胎兒炎癥反應是早產以及新生兒肺和大腦功能障礙的重要致病因素;胎兒暴露于感染以及胎盤和(或)新生兒體液中高濃度的炎癥相關蛋白可增加發生早產、早產兒支氣管肺發育不良以及早產兒顱腦損傷和伴隨殘疾的風險;感染和炎癥對ROP以及其他視覺功能障礙均可能存在不良影響。

    • Progress on numerical simulation of the deposition of inhaled particles in human pulmonary acinus region

      The inhalation and deposition of particles in human pulmonary acinus region can cause lung diseases. Numerical simulation of the deposition of inhaled particles in the pulmonary acinus region has offered an effective gateway to the prevention and clinical treatment of these diseases. Based on some important affecting factors such as pulmonary acinar models, model motion, breathing patterns, particulate characteristics, lung diseases and ages, the present research results of numerical simulation in human pulmonary acinus region were summarized and analyzed, and the future development directions were put forward in this paper, providing new insights into the further research and application of the numerical simulation in the pulmonary acinus region.

    • Evaluation of Effects of Health Education Pathway Intervention on the Mental Status and Coping Capacity in Family Members of Brain Injury Patients

      ObjectiveTo investigate the effects of health education pathway intervention on the mental status and coping capacity in family members of brain injury patients receiving surgery. MethodsOne hundred and eighty-eight family members were randomly divided into control group (n=93) and intervention group (n=95) between September 2013 and October 2014. The control group received conventional health education only, while the intervention group was given health education pathway intervention. The mental status and coping capacity of family members in the two groups on admission and at discharge were surveyed and compared based on symptom checklist 90 (SCL-90) and coping styles questionnaire (CSQ). The hospitalization stay and expenditure and the satisfaction degree were also compared. ResultsThere was no significant differences in mental status and coping capacity in family members between the two groups on admission (P > 0.05). After health education pathway intervention, the positive rate of SCL-90 in the control group was significantly higher than that in the intervention group, and the total score and score for each factor were also obviously higher (P < 0.05). As for coping capacity, the scores of self-blaming, avoidance, fancy and rationalization of CSQ in the control group were significantly higher than those in the intervention group, and the scores of appealing help and resolving problems were obviously lower (P < 0.05). Furthermore, the hospitalization stay and expenditure were significantly shorter or lower in the intervention group than those in the control group, and the satisfaction degree on nursing service was obviously higher (P < 0.05). ConclusionThe health education pathway intervention can greatly improve mental status and coping capacity in family members of brain injury patients.

    • Experimental research on the effect of functional residual capacity on the deposition of inhalable particles in human alveoli region

      Research on the deposition of inhalable particles in the alveoli of the lungs is important to the causes, development for common respiratory diseases such as emphysema, and even the optimization of clinical treatment and prevention programs of them. In this paper, an in vitro experimental model was established to simulate the deposition of terminal bronchioles and pulmonary acinus particles. The deposition rate of inhalable particles with different particle sizes in the pulmonary acinus was studied under different functional residual capacity. The results showed that the particle diameter was an important factor affecting the deposition of particles in the lung alveoli. Particles with 1 μm diameter had the highest deposition rate. With the functional residual capacity increasing, particulate deposition rate significantly reduced. The results of this study may provide data support and optimization strategy for target inhalation therapy of respiratory diseases such as emphysema and pneumoconiosis. The established model may also provide a feasible in vitro experimental model for studying the deposition of inhalable particles in the pulmonary alveoli.

    • Numerical simulation on the deposition characteristics of inhaled particles in human pulmonary acinus region under the influence of multi-factors

      Research on the deposition of inhaled particles in human pulmonary acinus region is important to the pathogenesis investigation, prevention and treatment of lung diseases. Most of the current research focus on the final deposition fraction of inhaled particles in human acinar region, but little is involved in their dynamic deposition characteristics. In this paper, five multi-alveolar models, G3?G7, were built. The evaluation parameter 1/4 deposition time was introduced to study the particle deposition speed. The deposition characteristics of particles in the diameter ranging 0.1?5 μm were numerically simulated and summarized under the influence of factors such as the generation and structure of model, particle diameter and respiratory mode, shedding some new light on the further research of transport of inhaled particles. The results showed that the generation and structure of model had a significance effect on the deposition of particles. 0.1 μm particles were dominated by Brownian diffusion, which experienced a high deposition fraction, a fast deposition speed and a logarithmic deposition curve, while 5 μm particles were dominated by gravitational sedimentation, with a high deposition fraction, a fast deposition speed and an S-shaped deposition curve. The deposition of 0.3?1 μm particles were influenced greatly by convention and varied with the change of respiratory mode. The research methods and results in this paper can provide theoretical basis and data support for the further exploration of the mechanism, prevention and treatment of lung diseases.

    • Progress in numerical simulation and experimental study on inhalable particles deposition in human respiratory system

      Inhalable particles deposition in the human respiratory system is the main cause of many respiratory and cardiovascular diseases. It plays an important role in related disease prevention and treatment through establishing computer or external entity models to study rules of particle deposition. The paper summarized and analyzed the present research results of various inhalable particle deposition models of upper respiratory tract and pulmonary area, and expounded the application in the areas of disease inducement analysis, drug inhale treatment etc. Based on the review, the paper puts forward the problems and application limitations of present research, especially pointing out future emphasis in development directions. It will have a value of reference guidance for further systematic and in-depth study on the inhalable particle deposition simulation, experiment and application.

    • 來曲唑微刺激促排卵方案在卵巢反應低下患者的臨床應用探討

      目的探討來曲唑微刺激方案在卵巢低反應患者的臨床應用價值。 方法選取2009年9月-2011年12月期間就診的第1周期采用常規超促排卵方案,即常規長方案超促排卵行體外受精/卵胞漿內單精子注射助孕的卵巢低反應且助孕失敗的74例患者,第2周期采用微刺激方案,即采用來曲唑微刺激促排卵方案治療,進行前后自身對照研究。比較前后2次促排卵方案中促性腺激素釋放激素(Gn)用量、使用Gn的時間以及注射人絨毛膜促性腺激素(HCG)日的雌激素水平、獲卵數、受精率、優質胚胎率和周期取消率。 結果第1周期常規超促排卵方案的Gn用量及使用Gn的時間均高于第2周期微刺激方案,差異有統計學意義(P<0.05)。但兩個周期在HCG日的雌激素水平、獲卵數、受精率、優質胚胎數和周期取消率,差異無統計學意義(P>0.05)。 結論對于卵巢低反應患者采用微刺激方案較經濟,且治療周期短,并且可連續重復進行,有利于提高累計妊娠率。

    • The incidence and influencing factors of early enteral nutritional feeding interruptions in critically ill patients: a meta-analysis

      ObjectiveTo systematically review the incidence and influencing factors of early enteral nutritional feeding interruptions in critically ill patients. MethodsThe PubMed, Web of Science, Embase, Cochrane Library, CNKI, WanFang Data and CBM databases were electronically searched to collect observational studies on the early enteral nutritional feeding interruptions in critically ill patients from inception to January 2, 2024. Two reviewers independently screened literature, extracted data and assessed the risk of bias of the included studies. Meta-analysis was then performed by using Stata 17.0 software. ResultsA total of 12 studies including 1 121 patients were included. Meta-analysis showed that the incidence of early enteral nutritional feeding interruptions in critically ill patients was 75.0% (95%CI 64.0% to 84.0%). Influenced by feeding intolerance, airway management, tube problems, radiological examination, and endoscopy, surgery and so on, interruptions of early enteral nutritional feeding frequently occur in critically ill patients. ConclusionCurrent evidence shows that early enteral feeding interruptions in critically ill patients are affected by many factors, and the incidence is high. Due to the limited quality and quantity of the included studies, more high-quality studies are needed to verify the above conclusion.
