    • Effects of Body Mass Index and Abdominal Obesity on Prevalence of Diabetes Mellitus and Fasting Blood-glucose in Elderly People in Chengdu

      目的 研究成都地區中老年人群體質量指數(BMI)及腹型肥胖對糖尿病患病率及空腹血糖水平的影響。 方法 2007年5月,采用隨機抽樣方法抽取50~80歲中老年人685人進行心血管危險因素調查,其男394人,女291人,年齡(63.3 ± 0.2)歲。 結果 成都地區中老年人群的超重和肥胖所占的比例較大(約44.3%),按BMI分組(BMI<24 kg/m2;24 kg/ m2≤BMI<28 kg/ m2;BMI≥28 kg/m2)的糖尿病患病率分別為14.0%、18.7%及23.3%,組間差異有統計學意義(P=0.031)。男女分別按腹型肥胖標準分組(男性切點=85 cm,女性切點=80 cm),男性糖尿病患病率在非腹型肥胖及腹型肥胖組分別為17.9%及18.7%組間差異無統計學意義(P=0.849),女性糖尿病患病率分別為9.7%及18.4%,組間差異有統計學意義(P=0.034)。整個人群中,空腹血糖無隨BMI增加而升高的趨勢(P=0.071);女性人群中,空腹血糖隨腰圍的增加而升高(P=0.001);而在男性人群中無此趨勢。在調整相關指標后,logistic回歸分析提示BMI對糖尿病患病率獨立影響,以BMI正常為參照,超重及肥胖的OR值分別為:1.412 [95%CI (0.818,2.437),P=0.215]及2.200 [95%CI(1.034,5.178),P=0.046]。在調整相關指標后,腹型肥胖在女性人群中對糖尿病患病率獨立影響,以非腹型肥胖為參考,腹型肥胖的OR值為:1.394 [95%CI(1.080,3.205),P=0.041],而在男性人群中無此關系。 結論 成都地區中老年人群超重及肥胖所占的比重較大,BMI可影響糖尿病患病率及空腹血糖水平;腹型肥胖能夠影響女性人群糖尿病患病率及空腹血糖水平,但在男性人群中無此關系。

    • Prevalence of Overweight and Obesity and Its Influence Factors among Inhabitants Aged 15-69 Years in Zhuhai

      Objective To understand the prevalence of overweight and obesity and its influence factors in Zhuhai inhabitants. Methods Applying multi-stage cluster random sampling in the three administrative areas of Zhuhai including Xiangzhou, Doumen and Jinwan, A questionnaire-based survey was performed in conjunction of the measurement of height and weight among 961 inhabitants aged 15-69 years. In addition, a multivariate unconditional logistic regression model was employed to analyze the influence factors of overweight and obesity. Results The prevalence and standardized rates of overweight and obesity in the study population were 18.1%, 17.6%, 6.6%, 6.5%, respectively. The standardized rates of overweight and obesity in male and female were 18.4%, 5.8%, 16.5%, 7.2%, respectively. Age, drinking, smoking and regional difference were identified as the 4 risk factors of overweight and obesity, their OR values being 1.028, 1.683, 0.677, 1.404, Plt;0.05, respectively. Conclusion The prevalence of overweight and obesity in Zhuhai’s inhabitants was over the average level of Chinese residents, and overweight and obesity has become a major risk factor influencing the health of Zhuhai’s inhabitants. In view of the influence factors of overweight and obesity, timely and effective prevention and control measures should be taken.

    • Effects of Body Mass Index on Prevalence of Hypertension and Blood Pressure in the Elderly

      摘要:目的:研究成都地區中老年人群體重指數(BMI)與高血壓患病率及血壓水平的關系。方法:按照隨機抽樣的方法抽取樣本,對711人(平均年齡為63.28±6.25歲;男性占57.8%)進行了相關調查,調查內容中包括身高、體重、血壓及脈搏等。結果:成都地區中老年人群的超重及肥胖所占比重較大(約45%),按BMI分組(lt;18.5 kg/m2,18.5~23.9 kg/m2,24~27.9 kg/m2,≥28.0 kg/m2)的高血壓患病率分別是31.6%,54.8%,64.4%,82.8%,差異有統計學意義。采用logistic回歸分析發現在調整年齡、性別、腰圍及尿酸等后,BMI對高血壓的患病率有獨立影響。在整個人群及女性病人中,血壓隨著BMI的升高而有升高的趨勢,差異有統計學意義。結論:成都地區中老年人群超重及肥胖所占比重較大。BMI可以影響高血壓的患病率及影響女性病人的血壓水平,是高血壓的獨立危險因素。Abstract: Objective: To investigate the effects of body mass index on prevalence of hypertension and blood pressure in the elderly. MethodsA survey, including height, weight, blood pressure and pulse, was carried out in a general population of Chengdu. A total of 711 subjects (average age: 63.28±6.25 years; male: 57.8%) were recruited by random sampling method. Results:The proportion of overweight and obesity was about 45%. The hypertension prevalence rate was significantly positively correlated with BMI (Plt;0.01), and that was also seen in the level of SBP and DBP for the female (Plt;0.05). In logistic regression analysis adjusting for age, gender, waist, uric acid, the standardized OR for higher BMI (≥28.0 kg/m2) as a risk factor of hypertension was 5.140. Conclusion:The proportion of overweight and obesity was great in Chengdu area. BMI can affect the prevalence rate of hypertension and the level of blood pressure.

    • The prevalence of osteoarthritis in Chinese aged 40 and over: a meta-analysis

      ObjectiveTo systematically review the prevalence of osteoarthritis in Chinese aged 40 and above from January 2000 to December 2019.MethodsPubMed, ScienceDirect, Wiley Online Library, The Cochrane Library, CBM, VIP, CNKI and WanFang Data databases were electronically searched to collect cross-sectional studies on osteoarthritis in Chinese aged 40 and above from January 1st, 2000 to December 31st, 2019. Two reviewers independently screened literature, extracted data and assessed risk of bias of included studies. Meta-analysis was then performed using R 3.5.2 software.ResultsA total of 29 cross-sectional studies with a total sample of 60 711 cases and 19 707 patients were included. The results of meta-analysis showed that, the crude prevalence of osteoarthritis in Chinese aged 40 and over was 38.46% (95%CI 24.31% to 46.22%). Subgroup analysis results showed that the prevalence of lumbar osteoarthritis was the highest (24.79%, 95%CI 13.28% to 27.37%), followed by knee osteoarthritis prevalence (20.50%, 95%CI 14.51% to 27.23%) which increased with age. The prevalence of knee osteoarthritis in females (25.14%, 95%CI 19.54% to 31.19%) was higher than that in males (18.99%, 95%CI 13.86% to 24.71%). The prevalence of knee and lumbar osteoarthritis in rural areas was higher than that in urban areas. The prevalence of knee osteoarthritis in western China (23.59%, 95%CI 18.34% to 30.35%) was higher than that in eastern China (18.36%, 95%CI 12.43% to 27.92%) and central China (15.54%, 95%CI 11.22% to 21.53%). The prevalence of lumbar osteoarthritis in western China (31.17%, 95%CI 19.21% to 50.60%) was higher than that in eastern China (24.38%, 95%CI 16.26% to 36.54%). The incidence of cervical osteoarthritis in the eastern China (20.49%, 95%CI 13.90% to 30.21%) was higher than that in the western China (12.32% 95%CI 8.09% to 18.75%). The prevalence of hand osteoarthritis in western China (6.85%, 95%CI 2.71% to 8.13%) was higher than that in eastern China (2.7%, 95%CI 1.33% to 5.48%).ConclusionsCurrent evidence shows that the prevalence of osteoarthritis in Chinese aged 40 and above is high, and the prevalence of lumbar osteoarthritis is the highest, and the prevalence in western China is higher than that in eastern and central China, followed by knee osteoarthritis, in which the prevalence in females, rural areas, and western China is high. Due to limited quality and quantity of the included studies, more high quality studies are required to verify above conclusion.

    • Prevalence and risk factors of asthma in Chengdu among adolescents aged over 14 years and adults

      ObjectiveIn order to improve the prevention and treatment of bronchial asthma, the prevalence and risk factors of asthma in Chengdu among residents over 14 years old were investigated.MethodsA cross-sectional survey was conducted in Chengdu. The inhabitants (age > 14 years) recruited in this household questionnaire survey were through multi-stage cluster random sampling. Univariate and multivariate logistic regression were used to analyze the risk factors of asthma.ResultsA total of 3 477 subjects were finally recruited in this study. Of them, 131 were asthmatic patients; and the prevalence rate was 3.8%. There were significant differences observed in the prevalence of asthma among people of different ages, residences, occupations and educational levels (χ2=191.084, P<0.05; χ2=9.114, P<0.05; χ2=114.268, P<0.05; χ2=62.123, P<0.05). Univariate regression analysis showed that the risk factors of asthma included five factors (measles, chickenpox, pneumonia, tracheobronchitis and intestinal parasitic diseases) related to childhood illness, and two factors (asthma and chronic bronchitis) related to the first-degree relatives (P<0.05). In addition, active smoking history was a risk factor for asthma in men (P<0.05). Multivariate logistic regression indicated that measles, pneumonia, tracheobronchitis, intestinal parasitic diseases in childhood and first-degree relatives suffering from asthma were independent risk factors for asthma.ConclusionsThis study describes the epidemiological characteristics of asthma in Chengdu among adolescents (age>14 years) and adults. The history of measles, pneumonia, tracheobronchitis, and intestinal parasitic diseases in childhood, and first-degree relatives suffering from asthma are the independent risk factors for asthma. In addition, active smoking history is a risk factor for asthma in men.

    • Meta-analysis of prevalence and risk factors of chronic obstructive pulmonary disease in China plateau

      Objective To analyze the prevalence rate of chronic obstructive pulmonary disease in people over 40 years old in China plateau (altitude≥1 000 m) in recent years. Methods PubMed, Embase, Cochrane Library, China National Knowledge Infrastructure, WanFang Data, VIP databases and Chinese Medical Database were searched, and the articles published between January 1, 2010 and December 5, 2022 were collected. Two reviewers independently reviewed the literature, extracted data, and assessed the risk of bias of included articles. Meta-analysis was performed with Stata 17.1. Results A total of 13 articles were included, with a total sample size of 37 230 people, including 3 697 chronic obstructive pulmonary disease patients. The prevalence rate of chronic obstructive pulmonary disease in people over 40 years old in China plateau was 9.0% [95% confidence interval (0.07, 0.11), P<0.001]. The results of subgroup analysis showed that males (12.4%), smokers (13.4%), 61-70 years old (15.1%), primary school and below (10.3%), low body weight (12.2%) and Yunnan (11.5%) had the highest prevalence rates within the subgroup (P<0.05). Conclusions The prevalence of chronic obstructive pulmonary disease in China plateau is still high. Gender, age, smoking, education level, body mass index and region are still important influencing factors of chronic obstructive pulmonary disease.

    • Epidemiology Study of Allergic Diseases in Liangshan District

      【摘要】 目的 調查涼山地區青少年四種常見變應性疾病——濕疹、蕁麻疹、變應性鼻炎(allergic rhinitis,AR)、支氣管哮喘(bronchial asthma,BA)的發病情況,及農村與城區患病差異。 方法 采用分層整群抽樣方式確定調查點。受調查者填寫調查問卷,接受皮膚科、耳鼻喉科、內科專科醫師檢查。 結果 濕疹調查8 113人,發現患者810例,患病率10.0%,標準化患病率9.3%;男性患病率8.1%,女性患病率11.2%(Plt;0.01);城區與農村患病率均為10.0%。蕁麻疹調查8 113人,發現患者513例,患病率6.3%,標準化患病率6.5%;男性患病率4.6%,女性患病率7.5%(Plt;0.01);農村患病率6.9%,城區患病率4.9% (Plt;0.01)。AR調查7 977人,發現患者741人,患病率9.3%,標準化患病率9.1%;男性患病率9.4%,女性患病率9.2%(Pgt;0.05);農村患病率9.0%,城區10.1%(Pgt;0.05)。BA調查7 741人,發現患者296例,患病率3.8%,標準化患病率4.0%;男性患病率3.2%, 女性患病率4.2%(Plt;0.05);農村患病率4.1%,城區患病率2.9%(Plt;0.05)。 結論 通過調查獲得了涼山地區青少年四種變應疾病患病數據。發現女性濕疹、蕁麻疹、BA患病率均高于男性,AR患病率相近。農村濕疹、蕁麻疹、支氣管哮喘患病率均高于城區,AR患病率相近。【Abstract】 Objective To obtain the prevalence rate of four common allergic diseases, including eczema, urticaria, allergic rhinitis, and bronchial asthma, in adolescent of Lianshan district; and to explore the differences of these allergic diseases prevalence rate between rural area and urban area. Methods A survey was conducted in adolescent. The cluster sampling was used. The subjects were required to fulfill the self-report questionnaires and also received physical examinations by doctors. Results A total of 8 113 individuals were included in eczema studis, in whom eczema were present in 810 with a morbidity of eczema of 10.0% (9.3% after standardized). The prevalence rate of eczema in males and females were 8.1% and 11.2%, respectively (Plt;0.01). The prevalence rate of eczema in urban area was the same as that in rural area (10.0%). A total of 8 113 people were included in the urticaria study, in whom urticaria were found in 513. The prevalence rate was 6.3% (6.5% after standardized). The prevalence rate of urticaria in males and females were 4.5% and 7.6%, respectively (Plt;0.01), and were 6.9% and 4.9% respectively in rural area and urban area (Plt;0.01). A total of 7 977 people were included in AR studis. AR was present in 741 people, and the prevalence rate of AR was 9.3% (9.1% after standardized). The prevalence rate of AR in males and females were 9.4% and 9.2% respectively (Pgt;0.05), and were 10.1% and 9.0% respectively in urban area and rural area (Pgt;0.05). A total of 7 741 people were included in BA study, and BA were present in 296 people. The prevalence rate of BA were 3.8% (4.0% after standardized). The prevalence rate in males and females were 3.2% and 4.2% respectively (Plt;0.05), and were 2.9% and 4.1% respectively in urban area and rural area (Plt;0.05). Conclusion The prevalence rate of these commom allergic diseases in adolescent of Liangshan district was obtained. The females have a higher prevalence rate of eczema, urticaria and BA than that in the males; the prevalence rate of AR in males is similar to which in the females. A higher prevalence rate of eczema, urticaria and BA is found in rural area than that in urban area; the prevalence rate of AR in urban area is similar to which in rural area.

    • The prevalence and related factors of diabetic retinopathy in Shanghai Songnan community

      ObjectiveIn order to provide guidance for early interference of diabetic retinopathy (DR) in patients with diabetes mellitus (DM), we surveyed the prevalence and analysis the related factors of DR in Shanghai Songnan community. MethodsBased on an established resident health database, an epidemiology study was performed on the residents with DM in Shanghai Songnan community.1177 patients completed questionnaire survey, and received physical examination and laboratory tests. The diagnosis and grading of DR were established based on the ocular fundus images acquired by digital non-mydriasis fundus camera. Patients with incomplete questionnaires were excluded. ResultsThere were 1120 DM patients with valid questionnaires. DR was found in 264 (23.57%) patients. The prevalence of mild, moderate, severe non-proliferative DR and proliferative DR was 17.05%, 5.09%, 1.16% and 0.27% respectively. There was significant differences in age, disease course, systolic blood pressure, insulin usage between the DR group and NDR group(t=-2.647, 2.688, 2.204, 2.291;χ2=12.527;P=0.008, 0.007, 0.028, 0.022, 0.000). There was significant differences in fasting blood-glucose and insulin usage between the mild, the moderate and the severe DR group(t=21.964, χ2=14.996;P=0.000, 0.001). Stepwise logistic analysis identified that age, DM course, and insulin usage were the related factors of DR (OR=0.769, 1.239, 1.100, 1.071, 1.682;P=0.001, 0.043, 0.176, 0.097, 0.005). ConclusionThe age, DM course, and insulin usage were the related factors of DR. The high prevalence of DR indicated the importance of the management of diabetic patients.

    • The prevalence of asthma among adults in China: a meta-analysis

      ObjectivesTo systematically review the prevalence of asthma in adults (>14 years old) in China.MethodsPubMed, Web of Science, CENTRAL, CINAHL, WanFang Data, CBM, CNKI, and VIP databases were electronically searched to collect cross-sectional studies on the prevalence of asthma in Chinese adults from January 2000 to December 2019. Two reviewers independently screened literature, extracted data and assessed the risk of bias of included studies, then, meta-analysis was performed by using Stata 15.0 software.ResultsA total of 67 cross-sectional studies, involving 1 381 673 subjects and 19 871 adult asthma patients were included. The results of meta-analysis showed that the prevalence rate of asthma in adults in China was 1.81% (95%CI 1.59% to 2.01%). The subgroup analysis showed that the prevalence rates of asthma in males and females were 1.87% and 1.76%, respectively. The prevalence rates were 2.16% in the northeast region, 1.34% in the central region, 1.63% in the eastern region and 2.23% in the western region. The prevalence rates were 0.73%, 1.53%, and 2.81% in 15-40 years old, 41-60 years old, and >60 years old, respectively. The prevalence rates were 3.07% in illiteracy population, 1.69% in primary school population, 1.46% in junior middle school population, 1.01% in senior high school population and 0.98% in college and above population. The prevalence rates were 2.23%, 2.10%, 1.17%, 1.45%, 1.09%, and 1.41% in agriculture, forestry and animal husbandry personnel, workers, public officials, business and service personnel, military personnel, and other occupations, respectively. The prevalence rates were 1.41% and 1.59% in Han and other ethnic populations, respectively. The prevalence rates were 2.03% in cities and 1.99% in rural areas. And the prevalence rates were 1.22% in 2000 to 2005, 2.07% in 2006 to 2010, 2.25% in 2011 to 2015, and 2.62% in 2016 to 2019.ConclusionsThe prevalence rate of asthma in Chinese adults is higher, which is different due to the influence of age, sex, region, education, and occupation.

    • Analysis on Needs, Demands and Utilization of Health Services of Urban and Rural Residents in Chongqing

      Objective To understand the status of needs, demands and utilization of health services of urban and rural residents in Chongqing, so as to provide references for the evaluation of health services status and policy making and regulating. Methods The data from family health questionnaire of health service survey in Western China in 2008 were descriptively analyzed. Results The two-week prevalence rate was 216.9‰ and the two-week hospital visit rate was 211.5‰. The sick people who did not seek medical care accounted for 56.2% among the sick population. The chronic disease prevalence rate was 226.4‰. The annual hospitalization rate was 77.1‰. Conclusion During the past five-year from 2003 to 2008, the needs of health services in Chongqing have had no big change, but the chronic disease prevalence rate has been in uptrend, and the utilization has obviously increased. And the economic factor is still the major cause for impeding residents to seek medical care. So it’s necessary to strengthen the construction of primary health care institutions, to improve the level of health insurance system, and to decrease the disparity in urban and rural areas.
