目的探討重癥肺炎患者中 D-二聚體、纖維蛋白原、白細胞介素 -6（IL-6）的水平變化及在臨床中的應用價值。方法采用回顧性分析的方法，選取 2014 年 10 月至 2016 年 3 月我院呼吸內科確診的 78 例重癥肺炎患者、78 例普通社區獲得性肺炎患者以及同期 78 例正常體檢者，分別收集患者入院時和正常體檢者血清中的 D-二聚體、纖維蛋白原和 IL-6。根據預后將重癥肺炎患者分為痊愈組與病死組兩個亞組。分析 D-二聚體、纖維蛋白原、IL-6 在三組中差異、在病死組與痊愈組中的差異，以及與肺炎嚴重度評分（PSI 評分）及其相互之間的相關性。結果重癥肺炎組中 D-二聚體、纖維蛋白原、IL-6 明顯高于其他兩組（P＜0.05），普通肺炎組高于正常組（P＜0.05）；病死組 D-二聚體、纖維蛋白原、IL-6 均較痊愈組高（P＜0.05）；重癥肺炎組 D-二聚體、纖維蛋白原、IL-6 均與 PSI 評分呈正相關性（r₁=0.765，P＜0.05；r₂=0.736，P＜0.05；r₃=0.675，P＜0.05）；且兩兩之間均呈正相關性（r₁₂=0.654，P＜0.05；r₁₃=0.532，P＜0.05；r₂₃=0.524，P＜0.05）。結論根據重癥肺炎患者中 D-二聚體、纖維蛋白原、IL-6 水平變化可評估重癥肺炎病情嚴重程度，并判斷患者預后情況。
