目的探討食管癌、賁門癌術后胸內食管胃吻合口瘺再手術治療的適應證、手術方法和臨床效果。方法回顧性分析2008年1月至2011年6月于林州市人民醫院行食管癌、賁門癌開胸手術后發生胸內食管胃吻合口瘺39例患者的臨床資料。其中17例患者行再次開胸手術治療(再手術組)，男14例、女3例，年齡(59.76±7.54)歲；其余22例患者行傳統經皮置管胸腔引流治療(傳統引流組)，男17例、女5例，年齡(59.18±7.24)歲。比較兩組患者相關臨床資料。結果傳統引流組治愈20例，死亡2例(9%)；再手術組治愈17例，無死亡病例。兩組死亡率差異無統計學意義(9% vs.0%，P>0.05)。再手術組患者第一次手術后住院時間[(29.76±7.92)d vs.(41.58±8.73)d，P<0.001]、體重下降[(4.83±5.47)kg vs.(10.05±6.21)kg，P<0.05]均優于傳統引流組。結論胸內食管胃吻合口瘺后再手術治療可選擇應用于大部分診斷明確的患者，合理的手術治療效果優于傳統引流治療。
