    • Static three-dimentional reconstruction of sonography in rhegmatogenous retinal detachment

      Objective To present stereoscopic image of rhegmatogenous retinal detachment with three-dimentional reconstruction of sonography. Methods Ultrasound data were collected by Hpsonos 1500 and 7.0MHz transducter with the motor controlled by computer.Three-dimentional image were reconstructed with Tomtec echoscan. Results Three-dimentional image were successfully reconstructed in 14 eyes of 13 cases of retinal detachment include 3 eyes of 3 cases with opaque refractive medium showing stereosopic image of retina and some retinal tears. Conclusion Static three-dimentional reconstruction of sonography might enhance the ability to visulize spatial anatomic structure of retina and offer a new method to find retinal tears in patients with opague refractive medium.     • Correlation between Intima-media Thickness of Carotid Artery in Color Ultrasonography and Heart Rate Variability

      ObjectiveTo investigate the correlation between intima-media thickness (IMT) of carotid artery in color ultrasonography and the heart rate variability. MethodsA retrospective analysis was performed in 64 patients from West China Hospital of Sichuan University between March and May 2013. Carotid intima-media thickness was measured with color ultrasonography and dynamic electrocardiogram, and the heart rate variability was assayed at the same time. ResultsIMT in the cardiovascular disease group, combination group, coronary heart disease group and hypertension group was significantly thicker than the control group (P<0.05). The differences of SDNN and SDANN were statistically significant (P<0.05) between the combination group and the control group. There were 23 cases with IMT ≥ 1.0 mm in the cardiovascular disease group including 8 cases in the combination group, 10 cases in the coronary heart disease group and 5 cases in the hypertension group. IMT in those groups were all significantly higher than that in the control group with only 2 cases having IMT ≥ 1.0 mm (P<0.05). There were 18 cases with SDNN<100 ms in the cardiovascular disease group including 7 cases in the combination group, 6 cases in the coronary heart disease group and 5 cases in the hypertension group, but there was no statistically significant difference compared with that in the control group with only 11 cases (P>0.05). Negative correlation was found between IMT and SDNN, SDANN in the cardiovascular diseases group (r=-0.574, -0.544; P<0.01) and negative correlation was found between IMT and SDANN in the control group (r=-0.392, P<0.05). ConclusionThe carotid artery lesions and autonomic nerve especially sympathetic nerve dysfunction are obvious in patients with cardiovascular diseases and there is a negative correlation between them.

    • Color Doppler imaging analysis of retrobulbar blood flow velocities in primary open-angle glaucomatous eyes: a meta-analysis

      ObjectiveTo observe the hemodynamic parameters of retrobulbar vessels of eyes with primary open-angle glaucoma (POAG) by using color Doppler imaging (CDI) technique. Methods Pertinent publications were retrieved from the PubMed of The National Library of Medicine, the ISI Web of Knowledge of The Institute for Scientific Information, and Cochrane Central Register of Controlled Trials. Case control studies involved POAG patients were included. Changes in retrobulbar blood flow parameters including peak systolic velocity (PSV), end diastolic velocity (EDV) and resistive index (RI) of the ophthalmic artery (OA), central retinal artery (CRA) and short posterior ciliary artery (SPCA) were evaluated by CDI. The searching time was from the data base established up to April, 2014. Meta analysis was used on the included articles, the mean difference (MD) along with 95% confidence interval (95% CI) of the blood flow parameters were calculated. ResultsTwenty-four articles were retrieved, including 1336 eyes as cases, 1102 eyes as controls. PSV of POAG eyes was statistically signiflcantly lower than controls in the OA (MD=-3.05, 95%CI:-4.49--1.61, P < 0.001), CRA (MD=-1.66, 95%CI:-1.95--1.38, P < 0.001), SPCA (MD=-0.87, 95% CI:-1.49--0.26, P=0.005). EDV of POAG eyes was statistically significantly reduced than controls in the OA (MD=-1.78, 95%CI:-2.14--1.41, P < 0.001), CRA (MD=-0.95, 95%CI:-1.17--0.74, P < 0.001), SPCA (MD=-0.53, 95%CI:-0.71--0.36, P < 0.001). Statistically significant increases in RI of POAG eyes than controls in the OA (MD=0.04, 95%CI: 0.03-0.05, P < 0.001), CRA (MD=0.06, 95%CI: 0.05-0.07, P < 0.001), SPCA (MD=0.04, 95%CI: 0.03-0.06, P < 0.001). ConclusionThis meta-analysis suggests that significant decreased velocity and increased resistance of retrobulbar blood flow are found in POAG eyes.

    • Value of Endorectal Ultrasonography in Preoperative Staging of Rectal Cancer

      Objective To determine the accuracy of endorectal ultrasonography (ERUS) in preoperative staging of rectal cancer and investigate the limitations and pitfalls of ERUS. Methods Ninety-four patients with rectal cancer were examined preoperatively by ERUS between September 2008 and November 2009 in this hospital. The size, shape, echo pattern, infiltration depth, and extra-rectal invasion of lesions were observed. The results of ERUS staging were compared with histopathology findings of the resected specimens. Results The overall accuracy of ERUS in T staging was 63.8% (60/94). The accuracies of ERUS for pT1, pT2, pT3, and pT4 tumor were 87.2% (82/94), 76.6% (72/94), 76.6% (72/94), and 97.9% (92/94), respectively. The sensitivity, specificity, and accuracy of ERUS for advanced rectal cancer (pT3+pT4) were 70.8% (34/48), 78.3% (36/46), and 74.5% (70/94), respectively. The sensitivity, specificity, and accuracy of ERUS in lymph node metastasis were 75.0% (42/56), 42.1% (16/38), and 61.7% (58/94), respectively. There was no significant difference of accuracy among various tumor locations above anocutaneous line (P=0.495). The accuracy of ERUS for T staging improved with experience, the T staging accuracy improved from 40.0% after assessment of 30 cases to 81.3% after 94 cases were examined (P=0.026). Conclusions The ERUS provides a good accuracy rate for assessment of the depth of tumor invasion and lymph node metastasis of rectal cancer, and has become an important imaging tool for preoperative staging rectal cancer. The operator experience, peritumoral inflammation mainly influences the accuracy of ERUS.

    • CLINICAL ANALYSIS OF DIAGNOSIS AND TREATMENT OF ACRAL GLOMUS TUMOR

      ObjectiveTo summarize the characteristics, diagnosis, and treatment of acral glomus tumor in order to improve the level of diagnosis and treatment. MethodsThe clinical data from 70 cases of acral glomus tumor treated between June 2004 and October 2013 were analyzed retrospectively. There were 11 males and 59 females with an average age of 41 years (range, 18-67 years). The disease duration ranged from 4 months to 30 years, with a median duration of 5 years. Sixty-nine cases had solitary tumors and only 1 patient had more than 1 lesion. The tumors were located on the finger in 66 patients (67 fingers) and the toe in 4 patients (4 toes); among them, the subungual glomus tumor happened in 44 patients (44 fingers and 1 toe). All patients suffered from paroxysmal pain and pinpoint pain with positive Love's pin test, and 29 patients (28 fingers and 1 toe) had positive cold sensitivity. Fifty-two patients (48 fingers and 4 toes) were found to have glomus tumor according to the high-frequency color doppler ultrasonography. X-ray films revealed depression on the phalanx in 16 patients (14 fingers and 2 toes). ResultsNo patient suffered from delayed incision healing, and infection after surgical treatment. The follow-up time was from 1 month to 9 years and 2 months with a median follow-up time of 20 months. The clinical symptoms disappeared after surgery with no dysfunction or recurrence. ConclusionThe diagnosis of acral glomus tumor is easy because of the typical symptoms:paroxysmal pain, pinpoint pain, and cold sensitivity. High-frequency color doppler ultrasonography may play an important role in the preoperative assessment of glomus tumors with accurate localization.

    • Comparative Ultrasonography with Radionuclide Imaging Evaluating Parotid of Sjogren Syndrome

      摘要:目的:探討超聲與核素顯像在評價干燥綜合征(SS)腮腺受累情況中的價值和作用。方法:對65例SS病人分別進行超聲和核素顯像檢查。結果:超聲判為腮腺功能0級、I級、ⅡⅢ級、Ⅳ級的能力與核素顯像判為正常(χ2=0.075,Pgt;0.05)、輕度(χ2=0.12,Pgt;0.05)、中度(χ2=0.27,Pgt;0.05)、重度(χ2=0.097,Pgt;0.05)受損的能力一致,差異無統計學意義;核素發現單純攝取功能受損的超聲多表現為不均勻型、單純排泌功能受損的超聲多表現為結節型、攝取和排泌功能均受損的超聲多表現為纖維化型。結論:超聲檢查可作為SS患者腮腺受累情況評價的良好手段。Abstract: Objective: To quantitatively evaluate ultrasonography and radionuclide imaging in thediagnosis of parotid gland involvement in Sjogren syndrome (SS). Methods: Ultrasonography and radionuclide imaging were conducted on 65 cases with primary Sjogren syndrome. Results: There was no significant difference statistically between the ultrasonographic appearance of the parotid gland and radionuclide imaging (P gt;0. 05). Conclusion: As for diagnosis and evaluating the parotid gland of Sjogren syndrome, ultrasonography may be the useful choice.

    • Grading method of inhomogeneity of contrast-enhanced ultrasound for rectal tumors based on gray level co-occurrence matrix

      Transrectal contrast-enhanced ultrasound (CEUS) is an important examination for rectal tumors. The inhomogeneity of the CEUS images has important clinical significance. However, there is no objective method to evaluate this index. In this study, a method based on gray-level co-occurrence matrix (GLCM) is proposed to extract texture features of images and grade these images according the inhomogeneity. Specific processes include compressing the gray level of the image, calculating the texture statistics of gray level co-occurrence matrix, combining feature selection and principal component analysis (PCA) for dimensionality reduction, and training and validating quadratic discriminant analysis (QDA). After ten cross-validation, the overall accuracy rate of machine classification was 87.01%, and the accuracy of each level was as follows: Grade Ⅰ 52.94%, Grade Ⅱ 96.48% and Grade Ⅲ 92.35% respectively. The proposed method has high accuracy in judging grade Ⅱ and Ⅲ images, which can help to identify the grade of inhomogeneity of contrast-enhanced ultrasound images of rectal tumors, and may be used to assist clinical doctors in judging the grade of inhomogeneity of contrast-enhanced ultrasound of rectal tumors.

    • Ultrasound biomicroscopy in 34 cases of iris and ciliary tumor

      Objective To probe the significance of application of ultrasound biomicroscopy (UBM) in the diagnosis and management of the iris and ciliary tumors. Methods UBM (Mode 840, Humphrey, 50 MHz 5 mm×5 mm) was done in 34 cases (35 eyes) of iris and ciliary body tumors, and some of the affected eyes underwent B-scan or Doppler ultrasound and CT scan. Histopathological examination of the resected tumor tissues was performed in 21 eyes of the operation. Results Among this series of 35 eyes with iris and ciliary body tumors detected by UBM, the characteristics of locality and solidity of the tumors, i,e., anterior chamber in plantation cyst, cyst behind the iris, and solid tumors of iris and ciliary body, of 21 eyes undergone surgical treatment revealed the same results both in UBM and histopathological examinations. Conclusion UBM can supply precise informations in diagnosis and treatment of tumors of iris and ciliary body. (Chin J Ocul Fundus Dis, 2002, 18: 128-130)

    • Measurement of ocular hemodynamics in retinal vein occlusion using color Doppler imaging

      Objective To investigate the changes of ocular hemodynamics in patients with retinal vein occlussion(RVO). Methods The hemodynamic parameters(PSV,EDV,PI,Vmax)of central retinal artery(CRA)and central retinal vein(CRV)were measured in the involved eyes(n=48) with RVO and the contralateral clinically healthy eyes(n=39) and in the control eyes(n=40) by color Doppler imaging (CDI)(ATLHDI3000). Results Peak systolic velocity (PSV) and end diastolic velocity (EDV) were significantly lower in the CRA of involved eyes and clinically healthy eyes of patients with RVO compared with control eyes,and pulsatility index(PI)was significantly higher in the CRA of involved eyes of patients with RVO compared with control eyes.PSV were significantly lower in the CRA of involved eyes of patients with RVO compared with their clinically healthy eyes.Pulsatility index(PI)was significantly higher in the CRA of involved eyes of patients with RVO compared with their clinically healthy eyes.Maximun vein velocity (Vmax) was significantly lower in the CRV of involved eyes and clinically healthy eyes of patients with RVO compared with control eyes. Conclusion The changes of hemodynamics in CRA,CRV of involved eyes of patients with RVO may invade their clinically healthy eyes.CDI may be helpful to early diagnosis for RVO.     • Application of MRA, IPVG and DUS in Hemodynamics Evaluation for Portal Hypertension

      【Abstract】ObjectiveTo evaluate the role of MRA, IPVG and DUS in the hemodynamics studies of portal hypertension. MethodsThirtyeight patients with portal hypertension were examined with Philips Gyroscan 1.0 Tesla MR imaging system. 3DDCE MRA and 2DPC MR were used for study of portal venous anatomy and its hemodynamics. The results were compared with those obtained from IPVG and DUS. Results3DDCE MRA could clearly display the anatomical imaging of portal venous system and its imaging quality was better than that of IPVG. The data of hemodynamics from 2DPC MR including diameter, blood velocity and blood flow were closely correlated to those from DUS. ConclusionAs a noninvasive technique, MRA can display the anatomy of portal venous system and measure its hemodynamics exactly. It should be applied as the first choice in hemodynamics evaluation for portal hypertension.
