    • Effects of Recombinant Human Brain Natriuretic Peptide on Chinese Patients with Heart Failure: A Meta-analysis

      Objective To evaluate the efficacy of recombinant human brain natriuretic peptide (rhBNP) on Chinese patients with congestive heart failure by meta analysis. Methods Both foreign language databases including PubMed, EMbase, The Cochrane Library (Issue 3, 2009) and Chinese databases involving CBM, VIP and CJFD were searched to identify randomized controlled trials (RCTs) that reported the effect of rhBNP on the heart function (left ventricular ejection fraction (LVEF) and the recent level of improvement in cardiac function) and its side effects of Chinese patients with congestive heart failure. Two reviewers assessed the quality of each trial and extracted data independently. The Cochrane Collaboration’s RevMan 4.2.8 software was used for statistical analysis. Results Nineteen RCTs were included, all of which came from internal. The methodological quality of the included studies was good. The baseline data of each trial were comparable. The results of meta-analyses showed: (1) the improvement of LVEF was higher in the rhBNP group than that in the blank control group (WMD=7.22, 95%CI 3.15 to 11.291, P=0.000 5). The level of improvement in cardiac function was better in the rhBNP group than those in the blank control group (OR=5.48, 95%CI 1.61 to 18.65, P=0.007), the nitroglycerin group (OR=3.60, 95%CI 2.02 to 6.41, Plt;0.000 1), and the sodium nitroprusside group (OR=3.21, 95%CI 0.12 to 85.20, P=0.49). The incidence of side effects was lower in the rhBNP group than that in the nitroglycerin group (OR=0.23, 95%CI 0.11 to 0.47, Plt;0.000 1), and the sodium nitroprusside group (OR=0.30, 95%CI 0.11 to 0.82, P=0.02). Moreover, the results of sensitivity analysis were also consistent with the above findings. Conclusion Recombinant human brain natriuretic peptide can effectively improve the hemodynamics and cardiac function level of Chinese population of patients with heart failure. The treatment doses are safe and tolerant, so it is recommended to clinical use.

    • Changes of fibrinogen and collagen metabolism after cardiac surgery and their relationship with postoperative atrial fibrillation: A prospective cohort study

      ObjectiveTo investigate the changes of fibrinogen and classical markers of collagen metabolism [carboxy-terminal propeptide of type Ⅰ procollagen (PICP) and carboxy-terminal cross-linked peptide of type Ⅰ collagen (ICTP)] in peripheral blood and pericardial drainage after coronary artery bypass grafting (CABG) and/or heart valve replacement (VR), and to evaluate their relationship with postoperative atrial fibrillation (POAF) after cardiac surgery. MethodsPatients who underwent CABG and/or VR in the Heart Center of Beijing Chao-Yang Hospital from March to June 2021 were included. Peripheral blood and pericardial drainage fluid samples were collected before surgery and at 0 h, 6 h, 24 h and 48 h after surgery to detect PICP, ICTP and fibrinogen levels, and preoperative, intraoperative and postoperative confounding factors were also collected. PICP, ICTP and fibrinogen levels were measured by enzyme-linked immunosorbent assay (ELISA). ResultsA total of 26 patients with 125 blood samples and 78 drainage samples were collected. There were 18 males and 8 females with an average age of 64.04±7.27 years. The incidence rate of POAF was 34.6%. Among the factors, the fibrinogen level in pericardial drainage showed two peaks within 48 h after operation (0 hand 24 h after operation) in the POAF group, while it showed a continuous downward trend in the sinus rhythm (SR) group, and the change trend of fibrinogen in pericardial drainage was significantly different over time between the two groups (P=0.022). Fibrinogen in blood, PICP and ICTP in blood and drainage showed an overall decreasing trend, and their trends over time were not significantly different between the two groups of patients (P>0.05). Univariate analysis showed that fibrinogen at 24 h and 48 h after pericardial drainage, fibrinogen in preoperative blood, PICP immediately after surgery and right atrial long axis diameter were significantly higher or longer in the POAF group than those in the SR group. Multiple regression showed that fibrinogen≥11.47 ng/mL in pericardial drainage 24 h after surgery (OR=14.911, 95%CI 1.371-162.122, P=0.026), right atrial long axis diameter≥46 mm (OR=10.801, 95%CI 1.011-115.391, P=0.049) were independent predictors of POAF. ConclusionThis study finds the regularity of changes in fibrinogen and collagen metabolic markers after CABG and/or VR surgery, and to find that fibrinogen in pericardial drainage 24 h after surgery is a potential novel and predictive factor for POAF. The results provide a new idea for exploring the mechanism of POAF, and provide a research basis for the accurate prediction and prevention of clinical POAF.

    • STUDIES ON THE CHANGES OF MORPHOMETRY AND NEUROPEPTIDE OF SPINAL NEURONS AFTER PERIPHERAL NERVE INJURY

      To observe the change of morphology and neuropeptide in the spinal neurons in order to clarify the functional state after injury of peripheral nerves is especially in the late stage. Sciatic nerves were cut with their proximal segments in the preparation of a model of peripheral nerve injury. Combination of horseradish peroxidase retrograde tracing immunohistochemistry and computer image analysis the changes in the morphometry of the perikarya of ventral horn neurons of the spinal cord, the quantitative changes of substance P (SP). Calcitonin gene-related peptide (CGRP) in dorsal horn and CGRP and choline acetyransferase (CHAT) in ventral horn of the spinal cord were examed. The results showd: (1) At the 3rd week after injury, swollen perikarya of the ventral horn neurons were observed, subseauently the swelling of perikarya was decreased tile the 6th week the neurons recovered to their normal size. At the 12th week the neurons were generally stable in their size, shortening of the dendrites was seen in 27% of the neurons. (2) The dendrites of the neurons progressively contracted till at the 12th week 53% of them were degenerated. The results of the 24th week were similar to the that at the 12th week. (3) CGRP in the ventral horn of the spinal cord was elevated to the highest point after 1 week of injury, that lasting for 4 weeks and 8 weeks later, the lever of CGRP returned to normal. From 20th to 24th week, there was no obvious changes of CHAT in the ventral horn of the spinal cord during observation. (4) SP went to the lowest point in the dorsal horn during 2-6 weeks, then recovered slowly, and beiny normal again after 16 weeks, however, CGRP was changed slightly. The results indicated that although a series of degenerating changes occurred in the neurons of the spinal cord during the late peripheral nerve injury, but the functional activity of the central meurons still was maintained at a certain level.

    • A comparative study on anti-endotoxin activity of three lipopolysaccharide binding protein/CD14 mimic peptide

      Objective To compare the in vitro inhibition activity of three mimic peptides at lipopolysaccharide binding protein (LBP)/CD14 binding sites for LPS-induced inflammatory response.Methods Enzyme-linked immunoSorbent assay(ELISA)was applied for the detection of the affinity between the mimic peptides and CD14 as well as the competitive inhibition activity of LBP.Mature U937 cells induced by PMA were co-cultured with LPS and intervened by mimic peptide.The effect of the mimic peptide on the TNF-α expression of U937 was detected by RT-PCR.Alveolar macrophages(AMs)of rats were co-cultured with FITC-LPS,and mimic peptide intervention was conducted.The effect of the mimic peptide on combination of LPS and AMs was observed by fluorescence microscope . Results Affinity between No.1 mimic peptide(FHRWPTWPLPSP,10 μg/mL)and CD14 was significantly higher than those of No.2 and No.3(20.3±4.1 11.8±2.4 and 13.7±3.3,Plt;0.01 or Plt;0.05).The competitive inhibitory activity of No.1 mimic peptide(10 μg/mL)for LBP was higher than those of No.2 and No.3[(57.2土11.2)% vs(39.4±9.7)% and(37.9±8.3)% ,Plt;0.01].All of the three mimic peptides(10 μg/mL)could significantly inhibit the LPS-induced expression of TNF-α by U937 at mRNA level(Plt;0.01 or Plt;0.05),moreover,the inhibitory activity of No.1 peptide was the highest(0.239±0.053 vs 0.406±0.112 and 0.493±0.121,Plt;0.01).In addition,No.1 mimic peptide markedly inhibited LPS combination with rat lung AMs(2157±514 vs 2763±453,plt;0.01).Conclusion No.1 mimic peptide(FHRWPTWPLPSP)have a relatively higher affinity with CD14 and high competitive inhibition activity for LBP,therefore it have the potential ability of anti-inflammatory response induced by LPS.

    • Perioperative Change Characteristics and Clinical Meanings of Natriuretic Peptide after Offpump Coronary Artery Bypass Grafting

      Objective To investigate the change characteristics of brain natriuretic peptide(BNP) after offpump coronary artery bypass grafting (OPCAB), and observe the relationship of BNP with atrial fibrillation, intensive care unit(ICU) stay time, time of withdrawing trachea tube, and usage of intraaortic balloon pump(IABP). Methods We retrospectively analyzed the clinical data of 168 consecutive patients with coronary artery diseases who underwent OPCAB in Beijing Anzhen Hospital from September 2009 to March 2010. There were 133 males and 35 females with an age of 60.77±9.32 years. BNP was determined before operation, on the operative day just after operation, and day 1, 2 and 3 after operation. At the same time, the relationship between BNP and ICU stay time, time of withdrawing trachea tube, occurrence of atrial fibrillation, and usage of IABP were also recorded. Results There were two inhospital deaths, due to low cardiac output syndrome, infection and circulatory failure respectively. BNP increased significantly after operation with the peak level occurring 1 day after operation, after which it decreased gradually. BNP level in patients with their left ventricular ejection fraction(LVEF) under 50% was significantly higher than that in patients with LVEF equal to or above 50% before and after operation(the median BNP level at the second postoperative day: 2 198.20 pg/ml vs. 531.65 pg/ml, Plt;0.05). BNP in patients using IABP was significantly higher than that in patients without using IABP before and after operation(the median BNP level at the first postoperative day: 5 066.75 pg/ml vs. 745.20 pg/ml, Plt;0.05). The ROC value was 0.834 when perioperative BNP was used to predict the possibility of using IABP. BNP in patients with postoperative atrial fibrillation was also significantly higher than that in patients without atrial fibrillation(the median BNP level at the first postoperative day: 1 070.60 pg/ml vs. 747.80 pg/ml, Plt;0.05). BNP was positively correlated with using IABP(at the third postoperative day: r=0.437, Plt;0.05), prognosis(at the first postoperative day:r=0.224, Plt;0.05), time of withdrawing trachea tube(at the third postoperative day: r=0.440, Plt;0.05), ICU stay time(at the third postoperative day: r=0.477, Plt;0.05). Conclusion BNP can be considered as one of the diagnostic criteria for ventricular dysfunction, and preoperative high BNP level is a risk factor for using IABP and ICU stay time equal to or longer than 3 days.

    • The Value and Significance of Brain Natriuretic Peptide in Evaluating the Heart Function of Functional Single Ventricle after TCPC Procedure

      Objective To investigate the value and significance of the changes of plasma level of brain natriuretic peptide(BNP) in evaluating ventricle performance of functional single ventricle after total cavopulmonary connection (TCPC). Methods We studied 11 patients with functional single ventricle undergone TCPC procedure after 2.1 years, who were followed-up at our ward between April 2004 and November 2004, 7 of them were males and 4 of them were females (TCPC group). The clinical heart function of patients was scored according to the modified scoring system described by Ross. We obtained 3ml blood samples from the extremital vein of all subjects. Blood was collected into chilled tubes containing EDTA and aprotinin (4.5mg and 1 500u/ml blood, respectively). The blood samples were promptly centrifuged (-4℃, 3 000r/min for 10 min) and the plasma was separated. BNP concentration was determined using immunoradiometric assay kits. Magnetic resonance imaging (MRI) examination was undertaken in 6 patients of TCPC group to analyse the relative factors with the change of BNP. Control group included 9 healthy children. Results (1) Median plasma BNP level for TCPC group and control group was 400pg/ ml (IQR200-690) and 110 pg/ml (IQR90-190), respectively. There was a significant difference in plasma BNP between them (P=0. 003). (2) The results of the index of heart function of TCPC group determined by MRI were 65. 76±8. 65 ml/m2 in end-diastolic volume index, 31. 90±6. 36ml/m2 in end-systolic volume index, 39.09±11.76ml/m2 in stroke volume index, 0. 52± 0. 06 in ejection fraction(EF), 2.38±0.58L/min·m2 in cardiac index (CI), 103.49±21.57g/m2 in mass index and 1.57±0.24 in mass/EDV. (3) The plasma BNP level for TCPC group was significant correlation with operation ages(r=0.632, P=0.041 ). There was no correlation between plasma BNP level with EF, CI, score of Ross, gender, ages, percutaneous oxygen saturation(SpO2) and the type of dominant ventricle, respectively. Conclusions Raised concentration of plasma BNP in patients 2 years after undertaken TCPC procedure indicates that nervous-endosecretory system is still under stress condition. This pattern suggests that neurohormonal activation is primarily related to the altered postoperative physiology. the significance of BNP in patients of functional single ventricle after undertaken TCPC is different from tat in biventricular physiology patiens. The plasma BNP level could not be correctly evaluated the cardiac function after TCPC operation.

    • CONSTRUCTION AND IDENTIFICATION OF Nogo EXTRA CELLULAR PEPTIDE RESIDUES 1-40 GENELENTIVIRAL VECTOR

      【 Abstract】 Objective To construct a lentiviral expression vector carrying Nogo extra cellular peptide residues 1-40(NEP1-40) and to obtain NEP1-40 efficient and stable expression in mammalian cells. Methods The DNA fragment ofNEP1-40 coding sequence was ampl ified by PCR with designed primer from the cDNA l ibrary including NEP1-40 gene, and then subcloned into pGC-FU vector with in-fusion technique to generate the lentiviral expression vector, pGC-FU-NEP1-40. The positive clones were screened by PCR and the correct NEP1-40 was confirmed by sequencing. Recombinant lentiviruses were produced in 293T cells after the cotransfection of pGC-FU-NEP1-40, and packaging plasmids of pHelper 1.0 and pHelper 2.0. Green fluorescent protein (GFP) expression of infected 293T cells was observed to evaluate gene del ivery efficiency. NEP1-40 protein expression in 293T cells was detected by Western blot. Results The lentiviral expression vector carrying NEP1-40 was successfully constructed by GFP observation, and NEP1-40 protein expression was detected in 293T cells by Western blot. Conclusion The recombinant lentivirus pGC-FU-NEP1-40 is successfully constructed and it lays a foundation for further molecular function study of NEP1-40.

    • BIOCOMPATIBILITY EVALUATION OF NANO TCP/ GELATIN/ VELVET ANTLER POLYPEPTIDE MATERIAL

      Objective To evaluate the biocompatibil ity of a new nano TCP/ gelatin / velvet antler polypeptide material. Methods The nano TCP/ gelatin / velvet antler polypeptide material was prepared, and the morphous was observed by scanning electron microscope. L929 and NIH/3T3 cell l ines were cultured conventionally. Acute toxicity test, hemolysis test, cell prol iferation and cytotoxicity test were used to evaluate the biocompatibil ity of the material. Results The compositemicrosphere material was about 10 μm in diamerter and had good spherical geometry, high monodispersity with nanometer size holes on the surface. Toxic symptoms such as hyperspasmia, palsy and death did not appear during the observing stage in acute toxicity test. Maximum hemolysis rate of the material was less than 5% which met the requirement of hemolysis test standard as a medical material. Different concentrations of the materials leaching l iquor could enhance the prol iferation of NIH/3T3 cells, which showed the good biologic activity. Toxicity grade was 0, and the material was no cytotoxic. Conclusion Nano TCP/ gelatin / velvet antler polypeptide material has good biocompatibil ity.

    • RESEARCH PROGRESS OF C TERMINAL PROPEPTIDE OF COLLAGEN TYPE II

      Objective To review the research progress of C terminal propeptide of collagen type II (CTX-II), a osteoarthritis (OA) biomarker. Methods Domestic and international l iterature about CTX-II was reviewed extensively and summarized. Results CTX-II is investigated broadly and has the best performance of all currently available biomarkers. CTX-II is a truly useful biomarker for early diagnosis, prognosis, and measurement of treatment response in OA. Conclusion Single CTX-II may be not sufficient for early diagnosis and prognosis of OA, so a combination of CTX-II and other biomarkers or diagnosis methods is needed.

    • Influence of Sleeve Gastrectomy on Blood Glucose, Ghrelin, and Glucagon Like Peptide-1 Levels in GK Rats

      ObjectiveTo investigate the effect and mechanism of sleeve gastrectomy (SG) on reducing blood glucose level. MethodsThirty GK rats were randomly divided into SG group, sham operation (SO) group, pair-fed (PF) group, and blank control (BC) group. The changes of weight, fasting blood glucose, glucose tolerance (oral glucose tolerance test, OGTT), insulin tolerance (insulin tolerance test, ITT), plasma insulin, ghrelin, and glucagon like peptide-1 (GLP-1) were monitored before and 24 weeks after operation respectively. ResultsFrom the 4th week after operation, weight gain in SG group and PF group began to decrease significantly compared with SO group (Plt;0.01). From the 2nd week after operation, fasting blood glucose level in SG group was lower than that in SO, PF, and BC groups (Plt;005), and the glucose tolerance in SG group obviously improved compared with preoperation and the other 3 groups (Plt;0.01). On the 6th week after operation, the insulin sensitivity in SG group obviously improved compared with SO group (Plt;0.05, Plt;0.01). There was no significant difference of insulin level between SG group and SO group (Pgt;0.05), ghrelin level significantly decreased (Plt;0.01) while GLP-1 level significantly increased (Plt;0.01) in SG group compared with SO group during 2-24 weeks after operation. ConclusionsThe effect of SG on reducing blood glucose is definite. SG can directly lower blood glucose independent with weight loss. Postoperative decreased ghrelin level and increased GLP-1 level may be its primary mechanism.
