    • Nursing and Observation of 456 Cases of Premature Rupture of Membranes

      摘要:目的:減少胎膜早破患者產科并發癥的發生。方法:將我院于2005年1月至2006年12月收治的217例胎膜早破的患者設為對照組,將2007年1月~2008年12月收治的248例胎膜早破的患者設為觀察組。對照組采用教科書上傳統的方法進行護理,觀察組正確地判斷胎膜早破,胎兒宮內狀況評估,產前選擇正確的臥位,加強對產前、產時、產后規范的監護,積極預防感染等措施。結果:積極的醫療處理有效地減少了產后出血,胎兒宮內窘迫,切口感染的發生。結論:對胎膜早破的患者,盡早地采取正確、有效的護理干預措施,能減少產科并發癥的發生,保障母兒的健康。Abstract: Objective: To reduce maternal obstetrics complications of premature rupture of membranes occurred. Methods: From in January 2005 to December 2006, treated 217 cases of premature rupture of membranes in pregnant women as control group, from January 2007 to December 2008 treated 248 cases of premature rupture of membranes as observation group. The control group used the traditional textbook approach to care. The observation group to determine the correct premature rupture of membranes, fetal assessment, pregnant women to choose the correct prelying, strengthen the preproduction, the postnatal care norms positive measures such as the prevention of infection. Results:The suitable medication and nursing procedure could effectively reduce postpartum hemorrhage, fetal distress, the occurrence of incision infection. Conclusion: The maternal premature rupture of membranes, as soon as possible to take the correct and effective nursing interventions can reduce the incidence of obstetric  complications to protect the health of mothers and infants.

    • Attitudes toward Reporting Adverse Events among Low-seniority Nurses and the Related Factors

      ObjectiveTo explore low-seniority nurses' attitudes regarding adverse events reporting. MethodA total of 200 low-seniority nurses were investigated with the Chinese version of Reporting of Clinical Adverse Events Scale from October to December 2013. Epidata software was used to collect and manage data and SPSS 17.0 software was applied to analyze the collected data. ResultsThe nurses who once witnessed or experienced adverse events accounted for 76%, of whom 74.3% reported adverse events. The mean score of reporting of clinical adverse events among low-seniority nurses was 48.5±7.2. Age was positively associated with the global scores of reporting of clinical adverse events, when compared with other variables (r=0.20, P=0.01). ConclusionsOverall, low-seniority nurses' attitudes toward reporting adverse events are negative. Age is positively associated with attitudes toward reporting adverse events. Therefore, a non-punitive culture should be established and an efficient reporting system is good to enhance the quality of care.

    • Study on the difference in stress load and professional benefit of hemodialysis nurses between different periods of coronavirus disease 2019 epidemic

      ObjectiveTo investigate the current situation of stress load and professional benefit of hemodialysis nurses during the outbreak period and the remission period of coronavirus disease 2019 (COVID-19) epidemic, and discuss the differences in stress load and professional benefit of hemodialysis nurses between the two periods.MethodsIn February 2020, 119 hemodialysis nurses from three hemodialysis centers in Sichuan Province were selected as the respondents. Using the Stress Overload Scale and the Questionnaire of Nurses’ Perceived Professional Benefits, questionnaire survey was conducted in February 2020 and April 2020, respectively. Paired-sample t test was used for analysis.ResultsA total of 109 valid questionnaires were collected. The scores of stress load of hemodialysis nurses in outbreak period and remission stage were 3.00±1.26 and 2.17±1.16, respectively, of which the scores of event load dimension were 3.49±1.15 and 2.31±1.27, respectively, and the scores of individual vulnerability dimension were 2.59±1.19 and 2.05±1.06, respectively; the professional benefit scores of hemodialysis nurses in outbreak and remission were 4.19±0.83 and 4.21±0.78, respectively. The difference in stress load of the same group of hemodialysis nurses between different time periods was statistically significant (P<0.05), while there was no significant difference in professional benefit (P>0.05).ConclusionsDuring the outbreak of COVID-19 epidemic, the stress load of hemodialysis nurses was ata medium level, and that in the remission stage of COVID-19 epidemic was at a low level; the professional benefit of nurses in the outbreak and remission period was at a high level. The stress load status of hemodialysis nurses was different between different periods of the epidemic. We should take incentive mechanism or targeted psychological intervention measures to improve the professional benefit level of nurses.

    • Application of Cluster Intervention in the New Nurses' Occupation Training

      ObjectiveTo explore the effect of the cluster intervention in new nurses in the Department of Neurosurgery in occupation training, so as to provide reference for the clinical training of new nurses. MethodsEight nurses who entered the Department of Neurosurgery in January 2013 were set as control group and the 8 new nurses entering in January 2014 as the observation group. The control group was adopted the traditional training methods, while the observation group underwent used extra cluster intervention. The differences in the results of theory, technology, operation and the 360-degree evaluation between the two groups were analyzed at the end of year. ResultsIn the observation group, the median theory examination score was 91.50, median operation assessment grades was 95.00, which were higher than those in the control group (82.00 and 83.00). The average scores of 360-degree evaluation in the observation group were higher than those in the control group with a significant difference (P < 0.01). ConclusionsIntensive intervention should be used for new nurses' training. It helps to improve the training effect and new nurses' ability.

    • Study on the Effect of Preoperative Application of Doctor-nurse Double Check Table

      ObjectiveTo evaluate the infiuence of doctor-nurse double check table applied before operation on the completion of preoperative preparation in gastrointestinal surgery department of class-three grade-one hospitals. MethodsA total of 647 selective operation patients from April to September 2013 in the Department of Gastrointestinal Surgery were divided into observation group (n=315) and control group (n=332) based on admission time. After training for medical staff, the check tables were filled, and relatively high frequency issues were followed up for quality tracking. The completion of preoperative preparation was compared between the two groups after operation. ResultsCompared with the control group, the completion of preoperative preparation and satisfaction of patients of the observation group were significantly higher and the operation delay was significantly lower (P<0.05). ConclusionPreoperative application of doctor-nurse double check table can significantly improve the completion rate of preoperative preparation, the operation delay phenomenon and satisfaction of patients, promote the communication between doctors and nurses, reduce the risk of operation, and ensure the safety of surgical patients.

    • The impact of integration of doctors-nurses-therapists continuous nursing management on rehabilitation of patients with spinal cord injury

      ObjectiveTo explore the application and effect of integration of doctors-nurses-therapists continuous nursing management in patients with spinal cord injury.MethodsThe patients with spinal cord injury from June to December 2018 in the Department of Rehabilitation Medicine of West China Hospital of Sichuan University were selected. A set of numbers was generated by a computer. And the patients were randomly divided into the trial group and the control group. Patients in the control group were given routine discharge continuous nursing management, and patients in the trial group adopted the integration of doctors-nurses-therapists continuous nursing management to implement post-discharge guidance. The activity of daily living (ADL), quality of life or psychological status of the two groups were compared at discharge and 3 months after discharge. The compliance, satisfaction and readmission rates between the two groups were also compared 3 months after discharge.ResultsA total of 60 patients were included, with 30 cases in each group. There was no significant difference in the scores of ADL, quality of life or Huaxi Xinqing index between the two groups (P>0.05). Three months after discharge, the score of ADL (71.65±1.87 vs. 62.70±2.29), quality of life (302.90±2.71 vs. 292.95±3.39), compliance rate of medication on time (86.67% vs. 63.33%), compliance rate of exercise on time (86.67% vs. 60.00%), compliance rate of follow-up visit on time (90.00% vs. 63.33%), compliance rate of reasonable diet (83.33% vs. 60.00%), and satisfaction (90.45±1.82 vs. 79.55±1.39) of patients in the trial group were higher than those in the control group (P<0.05). There was significant difference in Huaxi Xinqing index between the trial group and the control group (14.57±1.36 vs. 21.60±1.88, P<0.001). The readmission rate of the trial group was lower than that of the control group (6.67% vs. 26.67%), but the difference was not statistically significant (P>0.05). Three months after discharge, the ADL and quality of life in both groups were significantly different from those at discharge (P<0.001); the score of Huaxi Xinqing index in the control group was higher than that at discharge (t=-17.971, P<0.001), which in the trial group was also higher than that at discharge, but the difference was not statistically significant (t=-1.352, P=0.187).ConclusionThe integration of doctors-nurses-therapists continuous nursing management can effectively improve the ADL, quality of life and compliance of discharged patients with spinal cord injury, improve the mental state of patients, and improve patient satisfaction, which is helpful for the rehabilitation of patients with spinal cord injury outside the hospital.

    • A multicenter survey on the current status of delirium knowledge among geriatric nurses in Sichuan province

      Methods To explore the level of delirium knowledge of geriatric nurses in Sichuan province and analyze the factors, so as to provide the basis for systematic and targeted knowledge training on delirium and clinical management. Methods Using the self-designed “the Questionnaire of Elderly Delirium Knowledge”, geriatric nurses from 22 hospitals in Sichuan province were investigated through a convenient sampling method from September 2018 to February 2019. Results A total of 475 geriatric nurses were investigated. The average delirium knowledge score of the 475 geriatric nurses was 69.51±12.42. Multiple linear regression analysis showed that the main factors affecting the score of delirium-related knowledge were the education of nurses (P=0.037), technical title (P<0.001), years of working in the geriatric department (P=0.001), and the level of working hospital (P=0.001). Conclusions The level of delirium knowledge of geriatric nurses is low and can not meet the needs of clinical work. Nursing managers should carry out delirium knowledge training according to the different characteristics of nurses.

    • Investigation on Emergency specialized nurses’ core competencies and the influencing factors

      Objective To investigate the core competencies of emergency specialized nurses and the influencing factors in Sichuan Province so as to provide a basis for improving the training systems. Methods The trainees who received specialized training in West China Hospital every March and September between 2012 and 2014 were investigated with questionnaire survey. Results A total of 270 questionnaires were given out, and 246 valid questionnaires were retrieved. The scores of emergency specialized nurses’ core competencies ranged from 165 to 258, with an average of 214.55±22.56. According to the scores, 4.88% of the emergency specialized nurses’ core competencies were at a low level, 67.07% were at a middle level and 28.5% were at a high level. The influencing factors of core competencies included education, professional title, position, level of hospitals and years of working experience in the emergency department. Conclusion Core competencies of emergency specialized nurses need to be further improved and the training systems need to be improved consistently.

    • Study of the Attitude, Subjective Norm and Behavioral Intention toward Caring Mechanically Ventilated Patients among Nurses in ICU Settings in Chengdu

      Objectives To describe the attitude , subjective norm and behavioral intention of ICU nurses toward mechanically ventilated patients in Chengdu. Methods The modified version of Attitude, Subjective Norm and Behavioral Intention of Nurses Toward Mechanically Ventilated Patients (ASIMP) was used to investigate ICU nurses in three tertiary-level hospitals in Chengdu. Results The attitude, subjective norm and behavioral intention among ICT nurses respectively toward mechanically ventilated patients were 69.1%, 91.3%, and 95.9%. Conclusion The attitude, subjective norm and behavioral intention of most ICT nurses toward mechanically ventilated patients were positive.

    • Effect of Balint groups on promoting mental health in oncology nurses

      Objective To discuss the effect of Balint groups on promoting mental health of oncology nurses through developing Balint group activities. Methods From December 2015 to December 2016, a series of Balint group activities were carried out among 63 oncology nurses in West China Hospital of Sichuan University. Huaxi Emotion Index was used to investigate the psychological status in the nurses before and after the activities of Balint groups, and then the data was analyzed statistically. Result Except the scores of nurses aged over 41 and nurses above the primary and secondary level (P>0.05), all other scores had significant differences (P<0.05). Conclusions Balint group activities can significantly relieve oncology nurses’ negative emotions such as anxiety, tension and depression, alleviate body discomfort symptoms and improve interpersonal relationships. Huaxi Emotion Index is easy to understand, which helps rapid clinical assessment and nurses’ psychological problems screening. It is suitable for popularization and application clinically.
