    • Application of taijiquan in exercise rehabilitation of patients with chronic kidney disease

      Chronic kidney diseases (CKD) with long duration and a variety of complications have caused great physical and psychological problems for patients, and the overall quality of life of patients is low. Taijiquan, as a traditional Chinese techniques project, is beneficial to improving cardiopulmonary function, enhancing lower limb muscle strength, and reducing cardiovascular and cerebrovascular risks. We summarized the latest progress in clinical research concerning taijiquan as exercise rehabilitation for patients with CKD, aiming to promote the clinical application of taijiquan and other traditional exercises in the rehabilitation process of CKD patients and improve the overall quality of life of CKD patients.

    • Clinical Efficacy of Cordyceps Sinensis for Chronic Kidney Diseases: A Systematic Review

      Objective To assess the clinical efficacy of Cordyceps sinensis in the treatment of chronic kidney diseases. Methods Randomized or quasi-randomized controlled trials (RCTs or quasi-RCTs) were identified from MEDLINE (1996 to Oct. 2005), EMbase (1984 to Oct. 2005), The Cochrane Central Register of controlled Trials (Issue 3, 2005) and CBMdisc (1978 to Oct. 2005). We also handsearched related published and unpublished data and their references. Data were extracted and evaluated by two reviewers independently with a designed extraction form. RevMan4.2.7 software was used for data analysis. Results One RCT and 5 quasi-RCTs involving 434 patients were included. Results of meta-analysis were presented as follows: ① Response rate: five studies showed that, compared with the blank control group, significant differences were observed after the treatment with Cordyceps (RR 2.13, 95%CI 1.06 to 4.26); one study showed that there was no difference between Cordyceps and Shenyankangfu tablet (RR 2.17, 95%CI 0.73 to 6.460). ② Total effective rate: five studies showed that, compared with the blank control group, significant differences were observed after the treatment with Cordyceps (RR1.94, 95 %CI 1.14 to 3.30); one study showed that there was no difference between Cordyceps and Shenyankangfu tablet (RR 1.39, 95%CI 0.89 to 2.16). ③ Adverse reactions: adverse reactions were rare only with mild symptoms. Conclusions Cordyceps sinensis may improve the response rate and total effective rate of chronic kidney diseases. More high quality trials are required.

    • Prognosis of fetal isolated hyper echogenic kidneys on prenatal ultrasound examination: a meta-analysis

      ObjectiveTo systematically review the prognosis of fetal isolated hyper echogenic kidneys (IHEK) on prenatal ultrasound examination. MethodsPubMed, EMbase, Web of Science, WanFang Data, and CNKI databases were electronically searched to collect cross-sectional studies on the prognosis of fetal IHEK on prenatal ultrasound examination from January 1990 to January 2021. Two reviewers independently screened literature, extracted data, and assessed the risk of bias of included studies; then, meta-analysis was performed by using R 3.5.2 software. ResultsA total of 9 cross-sectional studies involving 348 fetuses were included. The results of meta-analysis showed that the incidence rate of live births was 79% (95%CI 69% to 88%), termination of pregnancy/neonatal mortality (TOP/NND) was 30% (95%CI 15% to 45%), normal kidneys was 34% (95%CI 15% to 53%), autosomal recessive polycystic kidney disease (ARPKD) was 21% (95%CI 12% to 30%), autosomal dominant polycystic kidney disease (ADPKD) was 13% (95%CI 5% to 21%), and multicystic dysplastic kidney (MCDK) was 4% (95%CI 2% to 7%). Subgroup analysis showed that the prognosis of normal amniotic fluid subgroup was significantly superior to that of reduced amniotic fluid subgroup. ConclusionCurrent evidence shows that the incidence of adverse pregnancy outcomes in patients with IHEK on prenatal ultrasound examination is high, the prognosis is superior when IHEK with normal amniotic fluid volume, and is worse when with small amniotic fluid volume. Due to limited quality and quantity of the included studies, more high-quality studies are required to verify the above conclusion.

    • Expression and significance of irisin in rats model with adenine-induced chronic kidney disease

      Objective To explore the expression and changes of serum irisin in adenine-induced chronic kidney disease (CKD) model, and the role of irisin and related pathway in CKD renal fibrosis. Methods Twenty male SD rats were randomly divided into a control group and a model group (CKD group) using a simple randomization method, with 10 rats in each group. At the end of the 2nd and 4th week, biochemical indicators, serum irisin and serum bone morphogenetic protein 7 (BMP7) levels, renal pathologic changes and interstitial fibrosis of renal tubules were measured in two groups of rats. The protein expression levels and messenger RNA (mRNA) expression levels of alpha-smooth muscle actin (α-SMA), collagen type I (Col-Ⅰ), BMP7, and Smad1 in rat kidney tissue were detected and compared. Results Compared with the control group at the end of the 2nd and 4th week, the CKD group showed that the serum creatinine (Scr), serum urea nitrogen (BUN), and 24-hour urinary protein level were increased (P<0.05), the protein expression levels and mRNA expression levels of α-SMA and Col-Ⅰ were increased (P<0.05), while the serum irisin and serum BMP7 were decreased (P<0.05), the protein expression levels and mRNA expression levels of BMP7 and Smad1 were reduced (P<0.05). Compared with the end of the 2nd week, the CKD group at the end of the 4th week showed that the serum Scr, serum BUN, and 24-hour urinary protein level were increased (P<0.05), the protein expression levels and mRNA expression levels of α-SMA and Col-Ⅰ were increased (P<0.05), while the serum irisin and serum BMP7 were decreased (P<0.05), the protein expression levels and mRNA expression levels of BMP7 and Smad1 were reduced (P<0.05). Compared to the control group, the renal tissue structure of the CKD group showed significant structural disorders and interstitial fibrosis of the renal tissue, which worsened over time. Serum irisin was negatively correlated with α- SMA and Col - Ⅰ (r=?0.917, ?0.902, P<0.001) respectively, while serum irisin was positively correlated with serum BMP7 (r=0.842, P<0.001); Kidney tissue BMP7 was positively correlated with Smad1 (r=0.884, P<0.001). The cluster heat map showed that compared with the control group, BMP7 and recombinant fibronectin type Ⅲ domain containing were significantly decreased, α-SMA and Col-Ⅰ were significantly increased in CKD group; recombinant fibronectin type Ⅲ domain containing were positively correlated with BMP7, and negatively correlated with α-SMA and Col-Ⅰ. Conclusions irisin may be involved in the process of renal fibrosis in adenine-induced CKD via the BMP7/Smad1 axis. This will provide new ideas for the prevention and treatment of renal fibrosis.

    • Value of CT enhanced image texture analysis in diagnosis of acute pancreatitis with acute kidney injury

      ObjectiveTo determine value of texture analysis based on bi-phasic enhanced CT images in diagnosis of acute pancreatitis (AP) with acute renal injury (AKI).MethodsA total of 62 patients with clinically proven AP including 39 patients with AKI and 23 patients without AKI were analyzed retrospectively. The region of interest (ROI) was chosen at the axial CT-enhanced images of bilateral kidneys using the ITK-Snap software and the texture analysis was performed by the Analysis-Kinetics (A.K.) analysis software. Using the Analysis of Variance, Mann-Whitney U test, Spearman correlation analysis and LASSO regression to reduce the features dimension, and screening out the textures by the logistic regression. The receiver operating characteristic (ROC) curve was established to determine the diagnostic performance of the features.ResultsIn the total of 396 image histological features originally extracted from the texture analysis, 6 features were finally screened out through the dimensionality reduction, involving the Haralick correlation, Inertia, Mean value, Cluster prominence, Short run high grey level emphasis, and Surface area. The area under curve (AUC), threshold, sensitivity, specificity, and accuracy in diagnosing of AP with AKI respectively was 0.926, 0.619, 89.4%, 71.4% and 82.7% by the Haralick correlation; which respectively was 0.790, 0.665, 59.6%, 82.1%, 68.0% by the Inertia; which respectively was 0.983, 0.662, 89.4%, 100%, 93.3% by the Mean value; which respectively was 0.903, 0.696, 80.9%, 85.7%, 82.7% by the Cluster prominence; which respectively was 0.980, 0.778, 76.6%, 100%, 85.3% by the Short run high grey level emphasis; which respectively was 0.819, 0.604, 78.7%, 75.0%, 77.3% by the Surface area.ConclusionTextures of contrast-enhanced CT images have better resolving ability and higher accuracy in diagnosis of AP with AKI and diagnostic efficiency of Mean value is the best.

    • Interpretation of guidelines on nutritional management of chronic kidney disease

      Chronic kidney disease (CKD) has become an important public health problem that seriously affects the physical and mental health of Chinese people. Nutritional therapy has become one of the important treatments for CKD. Domestic and international guidelines for CKD clinical nutrition therapy have been introduced, and the intake of various nutrients in CKD patients, especially protein intake, has been guided. In recent years, with the continuous development of nutrition assessment and treatment, the content of the guidelines has gradually improved, but some views are still controversial. This article explains the relevant guidelines for the treatment of CKD at home and abroad to provide reference for the clinical nutritional treatment of CKD patients.

    • Application of plasmapheresis in perioperative period of kidney transplantation

      Kidney transplantation is an ideal treatment for patients with end-stage renal disease. Circulating alloantibodies against donor human leukocyte antigens and blood group antigens can impair allografts, shorten allograft survival, and limit access to kidney transplantation. Furthermore, the presence of donor specific antibodies is associated with increased incidence of antibody-mediated rejection and decreased graft survival following transplantation. Plasmapheresis, an extracorporeal therapy directed at removing plasma proteins that has been found to minimize the effects of perioperative sensitization in kidney transplantation. Plasmapheresis enables transplantation across the barrier of ABO blood group incompatibility. In addition, it is also an important approach for the treatment of antibody-mediated rejection. Therefore, studying the application of plasmapheresis in perioperative period of kidney transplantation is expected to increase the chance of transplantation and improve the outcomes following transplantation. This article introduces the application of plasmapheresis in the perioperative period of kidney transplantation.

    • Impact of Preoperative Coronary Angiography on Postoperative Acute Kidney Injury in Patients with Valve Replacement

      ObjectiveTo explore whether preoperative coronary angiography could increase the incidence of postoperative acute kidney injury for patients with valve replacement. MethodsA total of 638 patients underwent routine cardiac valve replacement in our hospital from January 2013 through September 2015. There were 118 patients with preoperative coronary angiography (a coronary angiography group), and 520 patients without coronary angiography (a non-coronary angiography group). Serum creatinine (Scr), urea nitrogen(Bun), brain natriuretic peptide (BNP), creatine kinase myocardial band (CK-MB), cardiac troponin I (cTnI) values were recorded at 4 time points:before surgery (T0), after surgery 12 h (T1), 24 h (T2), 48 h (T3). The number of patients with acute kidney injury at the time of 48 hours after surgery was recorded. ResultsScr values (91.6±37.7 μmol/L vs. 81.0±27.4 μmol/L, 84.9±23.6 μmol/L vs. 73.5±25.3 μmol/L) increased in the patients who did not undergo coronary angiography at the time of 24 hours and 48 hours after cardiac surgery compared with the patients with coronary angiography with statistical differences. While there was no statistical difference in the incidence of acute kidney injury between the two groups. The cardiac enzymes had no statistical difference between the two groups. ConclusionPreoperative coronary angiography does not increase the probability of postoperative acute kidney injury.

    • Development and prospect of tissue engineering in urology

      Tissue engineering technology and stem cell research based on tissue engineering have made great progresses in overcoming the problems of tissue and organ damage, functional loss and surgical complications. Traditional method is to use biological substitute materials to repair tissues, while tissue engineering technology focuses on combining seed cells with biological materials to form biological tissues with the same structure and function as its own to repair tissue defects. The advantage is that such tissue engineering organs and tissues can solve the problem that the donor material is limited, and effectively reduce complications. The purpose of tissue engineering is to find suitable seed cells and biomaterials which can replace the biological function of original tissue and build suitable microenvironment in vivo. This paper mainly describes current technologies of tissue engineering in various fields of urology, and discusses the future trend of tissue engineering technology in the treatment of complex urinary diseases. The results of this study show that although there are relatively few clinical trials, the good results of the existing studies on animal models reveal a bright future of tissue engineering technology for the treatment of various urinary diseases.

    • Management throughout the whole course of acute kidney injury

      The high incidence and mortality of acute kidney injury (AKI) have brought great challenges to global health. In recent years, China has made some achievements in the epidemiology, risk factors and treatment of AKI. However, further prevention and treatment are still facing difficulties. Based on current new ideas and research progress, this paper summarized and analyzed the management throughout the whole course of AKI, including AKI risk assessment, early prevention, early identification, treatment and follow-up. The aim is to make Chinese nephrologists realize the focus of AKI prevention and treatment, standardize the management of AKI, and explore the prevention and treatment strategy suitable for AKI in China.
