In order to explore retinal toxicity and to estimatequot;safe dosagequot;of intravitreal gentamicin 21 Dutch-belted rabbits divided into 5 dosage groups and 1 control group were investigated.The results showed that, in 3 000mu;g group, optic disc swelling and vein dilating were observed ophthalmoscopieally. Histopathologie study displayed the retinal necrolysis. In 50~500mu;g groups,the retinal pigment change was revealed within 3~14 days after the injection.Histopathologic study displayed that the retinal damages were confined in outer layers of retina in early stage. Inner layers of retina were also influenced in late stage. The results demontrated that with increasing intravitreal gentamiein,retinal damages are gradully aggravated,and even if intravitreal gentamicin was in minimum dose,retinal damages was still observed. （Chin J Ocul Fundus Dis,1994,10:167-169）
Objective To probe the association between HLA-B27 subtype and acute anterior uveitis(AAU). Methods The HLA-B27 gene of 49 AAu patients were inspected by means of polymerase chain reaction(PCR), and the B27 subtypes of AAU patients were analyzed by means of DNA sequencing and simulated nucleotide hybridization in computer. Results Twenty-nine(59.39%)of 49 AAU patients were B27 positive, and among them 13 (44.00%)were B2704 and 16(56.00%)B27052 in subtypes．No statistically significant difference in most clinical features were found between the patients with B2704 and B27052.In B27 pasitive patients,8 (seven is B27052 and one is B2704) of them were complicatde with ankylosing spondylitis(AS). Conclusions The AAU patients with either B2704 or B27052 have similar clinical feature.The B27052 gene may be an important factor for complicated AS in AAU patients. Objective To present stereoscopic image of rhegmatogenous retinal detachment with three-dimentional reconstruction of sonography． Methods Ultrasound data were collected by Hpsonos 1500 and 7.0MHz transducter with the motor controlled by computer．Three-dimentional image were reconstructed with Tomtec echoscan． Results Three-dimentional image were successfully reconstructed in 14 eyes of 13 cases of retinal detachment include 3 eyes of 3 cases with opaque refractive medium showing stereosopic image of retina and some retinal tears． Conclusion Static three-dimentional reconstruction of sonography might enhance the ability to visulize spatial anatomic structure of retina and offer a new method to find retinal tears in patients with opague refractive medium． (Chin J Ocul Fundus Dis,1998,14:24-25)
Objetive To observe the incidence of extraocular extension of malignant uveal melanoma, the relationship between the incidence and histopathologic type, the characters of clinical manifestation and iconography, and the factors influencing prognosis. Methods Nine cases of extraocular extension of malignant uveal melanoma within last 20 years were analyzed retrospectively. All cases were diagnosed by histopathological examination and most of them had undergone ultrasound and CT examinations. Results The incidence of extraocular extension of malignant uveal melanoma was 15%. Tumors of 6 patients were flat and 2 nodular in shape, 4 epithelial cell and 1 spindle cell in type. The follow-up results of 5 patients in this series indicated that the prognosis was related to the size of the tumor, the degree of extraocular extension, and histopathologic type. Conclusion Extraocular extension of malignant uveal melanoma might happen in early case and the occurrence is usually related to the histopathologic type of the tumor. Iconographic examination is very helpful for diagnosis in extraocular extension of this tfumor. (Chin J Ocul Fundus Dis,1999,15:30-32)
目的:討論側后方減壓融合術治療胸腰椎骨折的優缺點及臨床應用。方法:對12例胸腰椎骨折采用側后方減壓植骨融合術,經一個手術入路完成減壓及脊椎的穩定重建。 結果:12例病例隨訪13～36個月,術后無一例神經癥狀加重;6例術后原有神經癥狀明顯改善,2例2個月后神經癥狀開始恢復,3例無改善;12例均獲得骨性融合。 結論:側后方減壓融合術是治療胸腰椎骨折的較好方法。但其技術要求高,視野顯露有限,操作范圍較窄,易出血增加了手術難度。
目的:探討行氣管切開術搶救成功的重型顱腦損傷及高血壓腦出血患者直接除管的安全性和可行性。方法:在507監護儀行SPO2監測和嚴密觀察下,不經過試阻管而直接將氣管套管拔除。結果:本組除1例患者因帶管時間長,切口周圍氣管內炎性肉芽生長而重新插管外,其余患者呼吸平穩,呼吸道通暢,無呼吸急促、嗆咳、紫紺及SPO2降低。結論:此法避免了傳統除管前試阻管的繁鎖和由阻管而引起的多種不良反應,有臨床實用價值。
Objective To study the expression and its significance of bcl-2 associated death (bad) gene in human optic nerves from traumatic atrophic eyeballs. Methods The optic nerves from 8 normal human donor eyes and 31 traumatic atrophic eyes were studied by immunohistochemistry technique. Results Bad protein was positively expressed in the normal optic nerve myelin sheath and residual myelin portions of optic nerve tissues from traumatic atrophic eyes. The expression of bad protein in the residual portions of myelin sheath was stained significantly ber than that in normal optic nerves (P＜0.05)。The pathological durations for ocular atrophy was not co-related with the quantites of expression of bad protein. There was no significant difference between pathogenic causes of ocular atrophies and the quantites of bad expression (P＞0.05). Conclusion Bad might possess the function of promoting the optic nerve atrophy processes in traumatic atrophic eyes. (Chin J Ocul Fundus Dis, 2002, 18: 276-278)
Objective To assess the change of the motion perception of cones in patients with AMD. Methods Motion perceptions of B、G and R cones of 20 eyes with dry type AMD were isolated by blue,green and red random dots stimulus displayed on the yellow,purple and blue-green background respectively,and keeping equiluminance between stimulus and background. Results The detecting displacement threshold and identifying threshold of motion perception from B,G and R cones were all damaged in eyes with dry type AMD compared with the normal group. Conclusion The abnormal changes of motion perception may reveal preclinical damage of visual function damage in dry type AMD. (Chin J Ocul Fundus Dis, 1999, 15: 219-221)
