Since 1979, nine children had been treated by free fibular graft in our hospital. Five of them were males, and 4 were females, with a mean age of eight years. The average length of the skeletal defects was 7 cm. The average length of the fibula that was removed for reconstruction was 12.3 cm. The blood circulation of the donor bone was re-established by direct anastomosis of one peroneal artery and vein of the transfer with the appropriate artery and vein of the host region. Nine patients had been followed from 1 year to 9 years,avereged 5.5 years. S even grafts which were substituted in the tibia defects took the same shape and caliber as that of the normal tibia with canalization of the marrow cavity . From the roentgenographic examination of the side where the fibular graft was removed, there was definite caudal migration of the upper fibular in 5 cases,averged 5.2mm,while in 7 cases there was cephalic migration of the distal fibular segments,averaged 4.3mm. The breadth of ankle mortise where increased in 7 cases,The tibio-talus angle was averaged 7 degrees,in 3 cases. In 3 of the 9 cases during follow-up,alaterl wedging of the distal tibia epiphysis with lateral tilting of the talus invarably associated with instability of the ankle was evident. We believed that the elevation of the lateral malleolus and latral wedging of the distal tibia epiphysis were responsible for instability of the ankle.
Objective To investigate the spatial and temporal regulation effect of VEGF on human fetal retinal vascularization and angiogenesis. Methods The posterior segmental retinas from 54 human fetuses of the 9th week to the 40th week were studied by immunohistodhemistry standing for the expressions of VEGF and PCNA. Results 1. The distribution of VEGF espression was spiking and the peaks were during the 9th-13th and around the 26th week. 2. PCNA immunoreactivity was localized in spindle cells and vascular endothelial cells. The expression level was fluctuated during the developmental process. The peaks were during the 9th-13th and around the 21st week. In these periods, the spindle cells kept proliferating and differentiating, and remodelled subsequently to form the inner side retinal vessels. From the 26th or 34th week, the PCNA immununoreactivity is fully expressed in the vascular endothelial cells of the inner and outer margin of inner nuclear layer(INL) and kept to full terms. 3. Significant positive correlation were shown between the content of VEGF in the retina and that of PCNA in spindle cells and vascular endothelial cells(r=0.736,p<0.01). Conclusion VEGF was positively involved in modulating human fetal retinal vascularization and angiogenesis. (Chin J Ocul Fundus Dis,1999,15:12-15)
Objective To explore the application of multi-disciplinary team (MDT) clinics in fetal developmental abnormalities. Methods A retrospective analysis method was used to collect case data of fetal developmental abnormalities and completion of prenatal MDT clinics diagnosis and treatment in pregnant women who were registered in the Department of Obstetrics of West China Second Hospital, Sichuan University between December 2021 and November 2022. The situation of pregnant women and fetuses was summarized and analyzed. Results There were 19362 registered pregnant women, of which 1125 (5.8%) had abnormal fetal development. Among them, 272 (24.2%) received prenatal MDT clinics diagnosis and treatment. Fetal developmental abnormalities were mainly characterized by structural abnormalities (208 cases, 76.5%), with the top three being central nervous system abnormalities, circulatory system abnormalities, and multiple malformations. There were 202 pregnant women who continue to conceive, and 70 cases had undergone induced labor, with an induced labor rate of 25.7%. The top three main causes of induced abortion were multiple malformations, central nervous system abnormalities, and circulatory system abnormalities. After prenatal MDT clinics diagnosis and treatment, the diagnosis of fetal developmental abnormalities in 46 pregnant women were corrected. Conclusion Prenatal MDT clinics are helpful for the early comprehensive evaluation of fetal developmental abnormalities and the determination of diagnosis and treatment plans.
Objective To investigate effects of neural retina on development of the structure of outer blood retinal barrier in embryogenesis. Methods The retinal neural epithelium (RNE) and pigment epithelium (RPE) layers of 150, 120 and 90 embryonic chicken eyes incubated for 7,10, and 14 days were peeled off. RNE was used to prepare the culture medium with different conditions (7drcSF3, 10drcSF3, 14drcSF3). RPE cells of 7- and 14-incubated chicken embryos were cultured on laminin-coated transwell filter. The SF3, 7drcSF3, 10drcSF3 , 14drcSF3 medium were used respectively in the apical chamber and SF2 was used in basolateral chamber. After the formation of monolayer, the transepithelial electrical resistance of the RPE was detected. After the fixation of RPE cells, the condition of the tight junction among the cells was observed by immunohis tochemistry and transmission electron microscopy. Results For the RPE cells of 7-and 14-day incubated embryonic eyes, the difference of TER in various medium of SF3/SF2, 7drcSF3/SF2, 10drcSF3/SF2, 14drcSF3/SF2 was statistically significant (P<0.01). The polarity of RPE cells was induced and the netlike tight junctional strands was urged in the retina-conditioned medium. Conclusion The neural retina may actively promote the formation of the structure of outer blood retinal barrier. (Chin J Ocul Fundus Dis,2004,20:237-240)
Surgery is an essential method of comprehensive treatment for lung cancer, but it also impairs patients’ cardiopulmonary function. A subset of patients who undergo surgery may suffer from postoperative complications, and even death. Preoperative pulmonary rehabilitation is a part of enhanced recovery after surgery, and can improve patients' cardiopulmonary function, reduce postoperative complication rate and shorten hospital stay. It has been already demonstrated a great value in lung cancer surgery. In this review, we summarized the three important components of preoperative pulmonary rehabilitation, including smoking cessation, chest physical therapy, and preoperative exercise training. Moreover, this review outlined the development of pulmonary rehabilitation for lung malignancies, aiming to promote its application and standardization.
To review the latest development of amniotic fluid-derived stem cell(AFS) and to predict its future appl ication. Methods The recent articles about AFS were extensively reviewed. The origin, culture, identification, and directional differentiation as well as future appl ication of AFS were summarized and discussed. Results In 1993, AFS were thought to be exist possibly, and it was confirmed in recent years. AFS have some surface markers l ike other stem cells, and has multi-differentiated potential and extensive future appl ication in cell therapy and tissue engineering approaches. Conclusion As a new stem cell, AFS remains to be further researched.
PURPOSE:To observe tbe development of structure of the retinal blood vessels in human fetuses. METHODS:The retinas of 134 human fetuses from 12-weeks of fertilization to full term and of 4 adults were collected,and examined with immunohlstoebemical method. RESULTS:The examinations showed:(1)The spindle ceils only existed in the retinas of fetuseslt;27 weeks. Till full term,the vessels in ganglion cell layer(GCL)was still budding. (2)BM of all arterioles,venules and capillaries within the retinas of all fetuses and adults,was immunostained after exposure to antlsera against fibronectin.The laminin(LN)and type Ⅳ collagen(COL-Ⅳ)immunoreactive product appeared outside tbe endothelial cells of blood vessels in GCL at 25 weeks and 29 weeks,and tile b LN and COL-Ⅳ immunostaining around four layers of the retinal vessels in tile shape of sbeath appeared at 29 weeks and 32 weeks respectively. (3)As soon as the cords of the endothelial cells were canaized,the smooth muscle cells and perieytes appeared outside the walls. (4)While four layers of the retinal vasculature was spreading to the ora serrata,the glial limitans was formed outside the endothelial cells by the processes of astrocytes and Muuml;ller cells. CONCLUSIONS:Comparing with the adults,certain de fleieneies of the structure of the retinal vessels exisl in the human fetus,and tile endnthelial cells in the retina of the full lerm fetus arc still in proliferation and migration. Objective To investigate the effect of the traditional Chinese medicine “Tetrandrine”(TET) and its significance on epidermal growth factor(EGF) and its receptor(EGFR) in the lung of nitrofen-induced congenital diaphragmatic hernia(CDH) rat model. Methods Twenty female rats were given maternal administration of a single oral dose (115 mg/rat) of nitrofen to induce CDH at 9.5 days after pregnancy and were dividedinto normal solution group(NS, n=5), dexamethasone group (Dex, n=5),tetrandrine group (TET, n=5) and Dex+TET group(n=5) at 18.5 days; 4 rats were given edible oil as controls. All fetuses were delivered by cesarean section at 21.5 days. Lung histologic evaluations and EGF, EGFR immunohistochemical staining and image analysis were performed. Results CDH was observed in 64 of the 137 rat fetuses (46.7%) in the experimental groups; no CHD was observed in 36 rat fetuses of control group. The lungs of CDH fetuses showed marked hypoplasia in NS group, in contrast to improved mesenchymal differentiationin that of Dex, TET, Dex+TET groups. The expression of EGF was weaker and weaker and that of EGFR was ber and ber as following order: NS, TET, DEX, T+D and control groups; showing significant differences between them (Plt;0.05). Conclusion Prenatal TET administration shows marked improvement in pulmonary hypoplasia through preregulating crest-time of EGF expression and upregulating EGFR expression in the lungs of nitrofen-induced CDH rat model. A combination of TET and Dex would generate evident synergistic effect.
The Second People’s Hospital of Yibin was taken over by West China Hospital of Sichuan University into its close hospital alliance, and simultaneously undertook the task of construction of regional hospital alliance in Yibin city. The article introduces the experiences of the Second People’s Hospital of Yibin in the high-quality development of novel healthcare system. Led by West China Hospital of Sichuan University and based on all-level medical institutions in Yibin, the Second People’s Hospital of Yibin preliminarily established a four-level hierarchical medical system as “province, city, county, village” model, according to the guide of governments within the whole region. It aims to propose a “Yibin model exploration” in hospital alliance construction based on the western regional situation.
