    • PROGRESS IN RESEARCH OF CLINICAL APPLICATIONS OF FASCIA LATA ALLOGRAFT

      Objective To find out the recent progress in research of cl inical appl ication of fascia lata allograft. Methods The domestic and international articles were reviewed to summarize the princi pal properties, processing techniques, and various uses of fascia lata allograft. Results Histologically fascia lata is composed of parallel and compact bundles of collagen fibers with few cells and immunologically it is low-antigenic. After varied tissue processing and storage techniques, fascia lata, as the scaffold only with the extracellular matrix, has been used in cl inical practice and achieved good results, such as ophthalmology, urology, and orthopaedics. Conclusion Because of these unique properites in repairing defects and reconstructing functions, fascia lata allograft, as a natural biomaterial, is promising to be used in more aspects withthe development of the biomedical techniques.

    • Significance of the hyperfluorescent areas in the posterior ocular fundus demonstrated by indocyanine green angiography in healthy volunteers

      Purpose To observe the features of the hyperfluorascent areas in the posterior ocular fundus detected by indocyanine green angiography(ICGA) in healthy volunteers, and to study circulatory properties of choroid. Methods Routine ICGA was performed on each of fifty consecutive normal eyes. Results ⑴Hyper fluorescence began at an average time of (30.80plusmn;5.42) seconds. ⑵The patterns of the hyperfluorescence revealed themselves in doubling areas divided symmetrically by the relatively hypoer fluorescence blelt running horizontally across the fovea in 29 eyes(58%), and single area in 21 eyes(42%).⑶The average area of the hyper fluorescence was (57.27plusmn;14.08)mm2.⑷ The sustaining time of the hyper fluorescence was (172.44plusmn;59.70) seconds at average. Conclusion During ICGA, a very patchy filling pattern of hyper fluorescence was visible in posterior fundus in normal eyes, and its filling time and shape presented choroidal blood supply and circulation. These parameters would offer consulted bases for clinical diagnosis of the choroidal diseases. (Chin J Ocul Fundus Dis,1999,15:1-3)

    • Application Status of Evaluation Methodology of Electronic Medical Record: Evaluation of Bibliometric Analysis

      In order to provide a reference and theoretical guidance of the evaluation of electronic medical record (EMR) and establishment of evaluation system in China, we applied a bibliometric analysis to assess the application of methodologies used at home and abroad, as well as to summarize the advantages and disadvantages of them. We systematically searched international medical databases of Ovid-MEDLINE, EBSCOhost, EI, EMBASE, PubMed, IEEE, and China's medical databases of CBM and CNKI between Jan.1997 and Dec.2012. We also reviewed the reference lists of articles for relevant articles. We selected some qualified papers according to the pre-established inclusion and exclusion criteria, and did information extraction and analysis to the papers. Eventually, 1 736 papers were obtained from online database and other 16 articles from manual retrieval. Thirty-five articles met the inclusion and exclusion criteria and were retrieved and assessed. In the evaluation of EMR, US counted for 54.28% in the leading place, and Canada and Japan stood side by side and ranked second with 8.58%, respectively. For the application of evaluation methodology, Information System Success Model, Technology Acceptance Model (TAM), Innovation Diffusion Model and Cost-Benefit Access Model were widely applied with 25%, 20%, 12.5% and 10%, respectively. In this paper, we summarize our study on the application of methodologies of EMR evaluation, which can provide a reference to EMR evaluation in China.

    • Research and application implementation of the Internet of Things scheme for intensive care unit medical equipment

      The intensive care unit (ICU) is a highly equipment-intensive area with a wide variety of medical devices, and the accuracy and timeliness of medical equipment data collection are highly demanded. The integration of the Internet of Things (IoT) into ICU medical devices is of great significance for enhancing the quality of medical care and nursing, as well as for the advancement of digital and intelligent ICUs. This study focuses on the construction of the IOT for ICU medical devices and proposes innovative solutions, including the overall architecture design, devices connection, data collection, data standardization, platform construction and application implementation. The overall architecture was designed according to the perception layer, network layer, platform layer and application layer; three modes of device connection and data acquisition were proposed; data standardization based on Integrating the Healthcare Enterprise-Patient Care Device (IHE-PCD) was proposed. This study was practically verified in the Chinese People’s Liberation Army General Hospital, a total of 122 devices in four ICU wards were connected to the IoT, storing 21.76 billion data items, with a data volume of 12.5 TB, which solved the problem of difficult systematic medical equipment data collection and data integration in ICUs. The remarkable results achieved proved the feasibility and reliability of this study. The research results of this paper provide a solution reference for the construction of hospital ICU IoT, offer more abundant data for medical big data analysis research, which can support the improvement of ICU medical services and promote the development of ICU to digitalization and intelligence.

    • Application of multi-subjects and multi-modes intervention in the enhanced recovery afterpancreaticoduodenectomy

      Objective The objective of this study is to evaluate the effect of enhanced recovery after surgery (ERAS) in the perioperative period of pancreatoduodenectomy. Methods This article conducted the forward-looking analysis on the information of 227 patients undergoing the pancreatoduodenectomy in West China Hospital from January 2016 to June 2017, and then compared the differences between the patients subjected to ERAS (ERAS group) and thosesubjected to regular measures (control group) with respect to time of setting in sickbed, time of mobilizing out ofsickbed, time of starting drink water, time of resumption of diet, exhaust time, defecation time, the time of nasogastric tube, postoperative hospitalization duration and expenses, postoperative complications, and postoperative pain scores. Results ① Postoperative indexes: by comparison of the ERAS group and the control group, it was found that the ERAS group had shorter (or lower) time of setting in sickbed, time of mobilizing out of sickbed, time of starting drink water, time of resumption of diet, exhaust time, defecation time, the time of nasogastric tube, postoperative hospitalization duration and expenses (P<0.05). ② Postoperative complications: of all postoperative complications, including pancreatic fistula, postoperative hemorrhage, delayed gastric emptying, biliary fistula, abdominal infection, incision complication, lung infection, and heart complication were without statistically significant differences (P>0.05) between the 2 groups.③ Reoperation and readmission: there was no significant difference on the incidences of reoperation and readmission between the 2 groups (P>0.05). ④ Postoperative pain scores: except 22 : 00 of the 6-day after operation, the pain scores in the ERAS group were all lower than those in the control group at 2 h and 8 h after operation, and the time points of 1–6 days after operation (8 : 00, 16 : 00, and 22 : 00), with statistically differences (P<0.05). Conclusion Without increasing the incidence of complications, ERAS may speed up the rehabilitation of patients undergoing the pancreatoduodenectomy and mitigate the pain of patients.

    • CLINICAL APPLICATION OF PERCUTANEOUS LASER DISC DECOMPRESSION IN THE TREATMENT OF CERVICAL DISC HERNIATION

      Objective To evaluate the clinical effect of percutaneous laser disc decompression (PLDD) in the treatment of cervical disc herniation. Methods From March 2003 to December 2005, 47 patients with cervical disc herniation(96 cervical disc) were treated with PLDD. There were 25 males and 22 females with an average age of 56 years, ranging from 37 to72 years. The lesion were located at the levels of C3,4 in 20 discs, C4,5 in 27 discs, C5,6 in 31 discs, C6,7 in 18 discs. The laser fiber was introduced into the center of the herniated disc space by percutaneous puncture from anterior neck surface under fluoroscopic guidance. Laser reduced the intradisc pressure through the vaporization of disc nucleu. The adopted laser was semiconducted with a wavelength of 810 nm. Each laser output power was15 W with 1 s emission and 2 s interval. The total laser output power was decided depending on the degenerative degree of the disc and the reactive process of heat, ranging from 300 to 1 000 J.Results Of 47 patients,42 were followed up for 3 to 31 months (mean 13 months). The clinical evaluation was classified as excellent in 18 cases (42.9%), good in 14 cases (33.3%), fair in 6 cases (14.3%) and poor in 4 cases (9.5%). The general response rate was 90.5%. The excellent and good rate was 76.2%. No complications occurred. Conclusion PLDD can relieve the symptoms and signs of patients suffering from cervical disc herniation with less complication. The manipulation of PLDD is easy, safe and mini-invasive.

    • Research progress on characteristics and applications of melanin nanoparticles

      Melanin nanoparticles (MNPs) not only retain the inherent characteristics of melanin (metal ion chelation, photothermal conversion property, etc.), but also can exhibit more excellent properties, such as high dispersion stability, good biocompatibility and biodegradability, etc. Furthermore, these performances can be enhanced to target the specific sites and treat diseases by the surface modification or combination with functional substance. In this paper, the characteristics, preparation methods and applications of MNPs were reviewed. It provides a reference for further development of application for MNPs, and theoretical basis for practice in biology, medicine and so on.

    • Application status of patient-reported outcome scale in lung cancer surgery

      Patient-reported outcome (PRO) has been paid increasing attention in lung cancer surgery. It has gradually become an important outcome indicator in clinical research of lung cancer surgery and an important tool for symptom management. Commonly used lung cancer-specific PRO measurement tools include: Lung Cancer Symptom Scale, European Organization for Research and Treatment of Cancer-Core Quality of Life Questionnaire and Lung Cancer module, Functional Assessment of Cancer Therapy-Lung, MD Anderson Symptom Inventory-Lung Cancer module, Postoperative Symptom Scale for Lung Cancer Patients, and Perioperative Symptom Assessment for Lung Surgery. The application of lung cancer-specific scales lacks authoritative implementation norms in the field of lung cancer surgery in terms of scale selection, data collection, and outcome application. This review aimed to analyze the current status of application of PRO scales in lung cancer surgery.

    • Promotion and Application of Appropriate Health Technology in China: A Systematic Review

      ObjectiveTo systematically review the model and mechanism, efficacy and its influencing factors of the promotion and application of appropriate health technology in China. MethodsSuch database as VIP, CNKI, CBM and WanFang Data were electronically searched to collect studies on the promotion and application of appropriate health technology in China published from the establishment dates to January 2013. According to the inclusion and exclusion criteria, two reviewers independently screened literature, extracted and cross-checked data, and assessed quality. Qualitative synthesis method was used to systematically summarize the included studies. ResultsA total of 73 studies were finally included. Targeted populations included primary medical staff and personnel who provided or developed appropriate health technologies, medical institutions and their managers, as well as patients who received service or treatment of appropriate health technology. The promotion of appropriate health technologies mainly contained five fields: Chinese medicine, biomedicine, family planning services, maternal and child health, and community health services. The government-leading model had been widely applied currently, besides, there were other four models: government-guidance, government-driven, market-leading and a third sector-leading models. Factors related to project funding, organization and management, technology holders, the supply and demand parties in technical services and technology itself might affect the application of appropriate health technology. Among all evaluation studies, the number of needs assessment was less, the contents of process evaluation were one-sided and the effect evaluation was mainly cross-sectional studies. ConclusionThis study shows that sustainable development of appropriate health technology promotion model need to fully consider market demand and people's needs, on the basis of using government-leading or introducing a third sector-leading model. Corresponding measures should be taken according to the influencing factors of promotion and application of appropriate health technology. It is necessary to conduct high quality evaluation studies in future and comprehensively evaluate the efficacy of the promotion and application of appropriate health technology.
