    • Perioperative Nursing of Intranasal Endoscopic Surgery for Traumatic Optic Neuropathy

      目的:探討經鼻內窺鏡下手術治療管內段視神經損傷的圍手術期護理。方法:對收治的11例視神經損傷住院患者的資料及圍手術期護理過程進行分析總結。結果:行視神經減壓術的11例患者中1例失訪,7例有效,其中4例視力有較明顯的提高。結論:經鼻內鏡視神經減壓術損傷小,并發癥少,手術時間短,療效滿意,其中圍手術期的護理是提高手術療效的一個重要環節。

    • The Analysis of the Application of Perioperative Antibiotics

      【摘要】 目的 了解在圍手術期術前30 min應用抗生素的情況。 方法 根據2004年衛生部、國家中醫藥管理局、總后勤部發布的《抗菌藥物臨床應用指導原則》中圍手術期抗生素的使用原則,對四川大學華西醫院2010年4-6月500臺手術圍手術期抗生素的使用情況進行分析。 結果 抗生素在麻醉前輸:0臺;抗生素末在術前30 min輸2臺,占0.4%;抗生素末即用即配:0臺;抗生素與麻藥及其他禁忌藥混合輸:0臺;手術3 h后末及時追加抗生素:0臺;特殊患者使用抗生素的注意事項不清楚2臺,占0.4%。 結論 該院99.6%的手術實行在手術室術前30 min輸入抗生素,確保抗生素達到有效濃度,有效控制感染,保證手術的成功,保障患者安全。【Abstract】 Objective To investigate the application of perioperative antibiotics half an hour before operation in West China Hospital of Sichuan University.  Methods According to Clinical Guidance of Antibiotics published by Ministry of Health, State Administration of Traditional Chinese Medicine and General Logistics Department in 2004, we investigated the application of perioperative antibiotics in 500 operations between April to June 2010 in our hospital. Results There was no operation with infusion of antibiotics before anesthesia, 2 operations without infusion of antibiotics half an hour before operation (0.4%), no operation without immediate infusion after preparation, no operation with mixed infusion of antibiotics and anesthesia and other contraindicated drugs, no operation without infusion of antibiotics 3 hours after operation, and 2 operations in which cautious items about the children, pregnancy and old patients were unclear (0.4%). Conclusion About 99.6% operations in our hospital have the infusion of antibiotics 30 minutes before the operation, which is the guarantee of antibiotics with effective concentration, inhibition of infection, success of the operation and safety of the patients.

    • Intelligent fault diagnosis expert system for multi-parameter monitor based on fault tree

      Aiming at the dilemma of expensive and difficult maintenance, lack of technical data and insufficient maintenance force for modern medical equipment, an intelligent fault diagnosis expert system of multi-parameter monitor based on fault tree was proposed in this study. Firstly, the fault tree of multi-parameter monitor was established and analyzed qualitatively and quantitatively, then based on the analysis results of fault tree, the expert system knowledge base and inference engine were constructed and the overall framework of the system was determined, finally the intelligent fault diagnosis expert system for multi-parameter monitor was developed by using the page hypertext preprocessor (PHP) language, with an accuracy rate of 80% in fault diagnosis. The results showed that technology fusion on the basis of fault tree and expert system can effectively realize intelligent fault diagnosis of multi-parameter monitors and provide troubleshooting suggestions, which can not only provide experience accumulation for fault diagnosis of multi-parameter monitors, but also provide a new idea and technical support for fault diagnosis of medical equipment.

    • Application of bone transport with unilateral external fixator combined with locked plate internal fixation in treatment of infected tibial nonunion

      ObjectiveTo summarize the effectiveness of bone transport with unilateral external fixator combined with locked plate internal fixation in treatment of infected tibial nonunion.MethodsBetween January 2010 and December 2014, 23 patients with infected tibial nonunion were treated with bone transport with unilateral external fixator combined with locked plate internal fixation. There were 19 males and 4 females with an average age of 37.8 years (range, 19-54 years). The mean length of the bone defect was 6.5 cm (range, 5.2-8.1 cm). The number of previous operations ranged from 2 to 4 times, with an average of 2.5 times. The time from injury to this treatment was 7-23 months, with an average of 11.8 months. The time of bone transport, time of the external fixation, fracture healing time, external fixation index, healing index, and complication were recorded; and the Association for the Study and Application of the Methods of Ilizarov (ASAMI) bone healing and function scores were used to evaluate the effectiveness.ResultsAll patients were followed up 3-6 years with an average of 4.8 years. Wounds and bone defects healed in all patients. No complication such as infection recurrence, nonunion, re-fracture, malunion, iatrogenic nerve paralysis, or stiffness of knee and ankle joints occurred. Five patients had needle infections which were treated by local care and no deep infection occurred. The time of bone transport was 65-120 days (mean, 75.6 days); the time of the external fixation was 75-145 days (mean, 97.8 days); the fracture healing time was 4-17 months (mean, 8.7 months); the external fixation index was 18-28 days/cm (mean, 22.4 days/cm); and the healing index was 31-52 days/cm (mean, 40.2 days/cm). At last follow-up, according to ASAMI criteria, the result of bone healing was excellent in 15 cases and good in 8 cases, and the result of function was excellent in 18 cases and good in 5 cases, all with the excellent and good rate of 100%.ConclusionFor infected tibial nonunion, bone transport with unilateral external fixator combined with locked plate internal fixation can reduce the time of external fixation and related complications, with a satisfactory effectiveness.

    • Intelligent fault diagnosis of medical equipment based on long short term memory network

      In order to solve the current problems in medical equipment maintenance, this study proposed an intelligent fault diagnosis method for medical equipment based on long short term memory network(LSTM). Firstly, in the case of no circuit drawings and unknown circuit board signal direction, the symptom phenomenon and port electrical signal of 7 different fault categories were collected, and the feature coding, normalization, fusion and screening were preprocessed. Then, the intelligent fault diagnosis model was built based on LSTM, and the fused and screened multi-modal features were used to carry out the fault diagnosis classification and identification experiment. The results were compared with those using port electrical signal, symptom phenomenon and the fusion of the two types. In addition, the fault diagnosis algorithm was compared with BP neural network (BPNN), recurrent neural network (RNN) and convolution neural network (CNN). The results show that based on the fused and screened multi-modal features, the average classification accuracy of LSTM algorithm model reaches 0.970 9, which is higher than that of using port electrical signal alone, symptom phenomenon alone or the fusion of the two types. It also has higher accuracy than BPNN, RNN and CNN, which provides a relatively feasible new idea for intelligent fault diagnosis of similar equipment.
