    • Evaluation of performance on the project of health system recovery using World Bank loan after Wenchuan earthquake

      Objective To evaluate the performance on the project of health system recovery using World Bank loan for ‘5.12’ Wenchuan earthquake after three years of these projects have been finished. Methods From March to December 2016, we formulated evaluation frameworks and indicators, used statistical reports and surveys to collect data of the 60 post-earthquake recovery projects using World Bank loan in Chengdu, Deyang and some other cities. Data of pre-earthquake (2007) and post-earthquake (2015) were compared. The evaluation indicators included institutional size, institutional environment, institutional services and social benefits. A descriptive analysis was performed using SPSS 22.0 software. Results Compared with the year of 2007, in 2015, the building and utilized for business purpose areas of loan benefited hospitals increased 4.49 and 3.58 times, respectively. The budgeted and actual beds count increased 43% and 55%, respectively. All inside structure and processes of hospitals were optimized, and green areas, parking slots and waiting areas increased greatly. Headcount of budgeted and actual increased 15.09% and 70.31%, respectively. Qualifications of healthcare technicians were improved in country level hospitals as well; there were more undergraduates and more senior competent personnel. In maternal and child health care hospitals and township hospitals, more diploma holders and personnel who had passed the middle level professional appraisals were observed. Numbers of hospital treatments, hospitalizations counts, numbers of surgeries performed increased 2.0, 2.1, and 2.0 times, respectively. Accuracy of diagnosis on hospitalized counts increased 3.12%. Utilizations of bed counts increased from 63.87% to 66.53%. The average duration of hospitalizations decreased from 7.36 days to 7.10 days. Numbers of clinical and surgery types increased 928 and 285, respectively. Both customer and staff satisfactory score reached 4.5 points in 2015. Conclusion The completed projects of health system recovery using World Bank loan for ‘5.12’ Wenchuan earthquake is running well, and meet the expectations.

    • Investigation into the Practice of Ensuring a Steady Blood Supply for Medical Rescue during the Wenchuan Earthquake△

      In the rescue that follows a major natural disaster, blood donation is a unique, necessary method to assist the injured. To achieve effective assurance of the blood supply for medical rescue in an orderly fashion, the current procedure must be adjusted by using scientific prediction, analysis, and adaptation. After the process of ensuring the blood supply for medical rescue during the Wenchuan Earthquake, the Ministry of Health of China, with great efforts, continues to investigate and reflect upon the application of the above principals during actual practice. Objectively, these efforts will lead to better results and establish a standard supplying blood during a disaster rescue.

    • Investigation for Rehabilitation Requirements of Children Suffering from Wenchuan Earthquake

      Objective? To investigate the dysfunction information of children suffering from Wenchuan earthquake and analyze their rehabilitation requirements. Methods A survey of rehabil itation requirements of 87 children with earthquake injuries was performed using self-made questionnaires. Results Among the 87 children investigated, 64.4% suffered from restricted joint range of motion, 95.4% had abnormal sensory (pain occupying 89.7%). Conclusion The earl ier the patients received the rehabilitation therapy, the better for them to recover functions, improve activity ability in daily life and promote participation ability.

    • A Cross-Sectional Study on the Mental Status of 780 Survivors after Wenchuan Earthquake

      Objective To evaluate the mental status of survivors after Wenchuan earthquake, so as to provide relevant information for psychological and medical intervention. Methods Demographic data was recorded and the post-traumatic stress disorder (PTSD) Checklist-Civilian Version (PCL-C) was evaluated. The acute stress disorder (ASD) were assessed with the DSM-IV criteria in the survivors from the central earthquake area who are now living in two shelters in Chengdu. Results Among the 780 retrieved questionnaires, 729 baseline registration forms and 450 PCL-C were valid. In the 729 baseline registration forms, male to female ratio was 1:1.31 and the mean age was 36.73±20.82. 9.5% of subjects suffered from physical diseases, 9.7% needed assistance in daily activities, 70.7% were passive and didn’t participate in community activities, 9.1% suffered from severe psychological problems. In the present study, 30.9% of subjects were found to meet the DSM-IV criteria for ASD. 41.3%, 22.2% and 18.4% of subjects had a PCL-C score ≥ 38, ≥ 45 and ≥50, respectively. Consistency check showed there was no significant difference between the screening result of PCL-C score ≥45 and clinical diagnosis (Kappa=0.780, P=0.033). There was significant difference in the age of subjects between ASD and non-ASD groups (Plt;0.05). Conclusion  The earthquake survivors suffered from psychological problems after the disaster in the early stage. Thus, in addition to life rescue, we should pay more attention to psychological intervention.

    • Timely Safety Evaluation of Buildings to Secure the Rescue Work for the Wounded after Wenchuan Earthquake

      After the May 12th Wenchuan earthquake, the Department of Architecture and Operation of West China Hospital took prompt action to examine the damage of the hospital buildings. And then experts were invited to perform a safety evaluation of all the hospital buildings. Meanwhile, a real-time monitoring system was initiated to identify any subsequent damage caused by after-shocks. In timely response to the clinical demand, potential dangers were removed so as to ensure the medical rescue work for the wounded.

    • Administration of Medical Materials for the Daytime-sickroom During Wenchuan Earthquake

      Objective To summarize the experience of preparation and administration of medical materials for the daytime-sickroom during the treatment of patients who were injured during Wenchuan earthquake in order to supply management experience for future emergency situations. Methods Data concerning the preparation and administration of medical materials and medical treatment for the daytime-sickroom were collected systematically. Results The high level of activity and order during the rescue provided quality care to patients injured in the earthquake. All 224 patients were treated effectively from May 12-29.Conclusion The daytime-sickroom acts as a combination sickroom, which allowed flexibility. The daytime-sickroom can manage medical materials and succesfully insure the treatement of patients when emergent events happen.

    • Survey of the Reality of Community Health Service after Wenchuan Earthquake and Postdisaster Emergency Response Capability of Community Hospital in Mianzhu

      摘要:目的: 了解綿竹市社區衛生服務系統震后現狀,同時分析社區醫療震后居民滿意度和社區衛生服務機構震后災害干預能力,以期為社區衛生服務體系地震應急恢復和重建提供參考意見。 方法 :采用隨機抽樣的方法,抽取綿竹市劍南社區衛生服務中心和天河社區衛生服務中心進行訪談,采取方便抽樣的方法,抽取24‰的綿竹城區居民采用面對面訪談的方式用自制問卷進行調查,并用Epidata30 進行數據錄入、SPSS130進行統計分析。 結果 :共發放問卷240份,收回有效問卷229份(有效回收率954%)。當地社區衛生服務系統在地震中受損嚴重。社區衛生服務系統災后工作居民滿意度為454%,社區衛生服務機構對居民進行抗災/防災知識教育的比例為336%,災后是否有持續而足夠的常見病藥品供應及是否有持續而足夠的慢性病藥品供應是影響當地居民對當地社區衛生服務體系災害應急工作的滿意度的影響因素(P 值分別是0033,0001)。 結論 :震后社區衛生服務居民滿意度較低,服務體系地震災害干預能力不足。居民在在災前接受抗災教育的比例較低,加強藥品儲備能提高社區衛生機構災害應急工作的效果。在社區衛生服務體系重建的過程中,應注重社區醫療基礎工作的恢復,基礎設施的重建和健全社區急救體系。Abstract: Objective: To investigate the reality of community health service system after earthquake in Mianzhu, the satisfaction of community residents to the community health service as well as the postdisaster emergency response capability of community hospital in order to provide decisionmaking suggestions on better reconstruction of community health service system. Methods : Jiannan and Tianhe community hospital were randomly selected for visiting and 24‰ of community residents in the city zone of Mianzhu were selected by convenience sampling for a facetoface interview using a questionnaire. Data entry and statistically analysis were completed by Epidata30 and SPSS130 respectively. Results :A total of 240 questionnaires were conducted to facetoface interviews, and 229 questionnaires were returned (response rate 954%).The community health service system was badly injured. Residents’ satisfactory degree of the community health service after earthquake was 454%. The proportions of disaster / disaster prevention education was 336%,medicine supply for familiar diseases and the chronic were the main factors which influenced judgements of residents to the emergency response capabilities of community hospitals(〖WTBX〗P =0033,P=0001,respectively). Conclusion :The community health services after earthquake had not been widely satisfied and the emergency response capability of community hospital was far from enough. The proportions of disaster / disaster prevention education were far from enough. The effectiveness of emergency response work of community hospitals can be enhanced by reinforcing medicine preparation.In the course of the reconstruction, community health service system should pay attention to the resumance of basic community health service,reconstruction of basic establishment and construction of firstaid system.

    • The Experiences of Emergency Treatment in Wenchuan Earthquake

      After 5?12 When Chuan earthquake,The third hospital of Mianyang which is the nearest hospital of severely afflicted area recived 1 804 wounded persons.We aimed to summarize the achievement and limitation in management of medical rescue, medical treatment of the wounded and logistical support. Hospital shouled set up a special medical rescue system to relieve the sufferings of victims of unexpected natural calamities which include staff training, reserve supplies,psychological intervention of the wounded and safeguard of the rescuers.

    • Treatment Analysis of 563 Wounded Patients in the Third People’s Hospital of Chengdu after the Wenchuan Earthquake

      Objective To analyze retrospectively the overall situation of medical rescue in the Third People’s Hospital of Chengdu after the Wenchuan earthquake, so as to provide references for the emergency preparedness for the disaster of earthquake. Methods The analysis was based on the data provided by the Department of Information, the Medical Record Library, the Department of Emergency, the Department of Orthopaedics and other related departments of the Hospital up to 8 a.m. of July 12. The software of Microsoft EXCEL was used for data input, and SPSS 11.5 was used for statistical analysis. Results Up to July 12, 563 cases from the disaster area had been treated in the hospital, of whom 249 were admitted into the outpatient department and 314 into the inpatient department, including 287 men and 276 women, with a median age of 42 years (28, 57) in the outpatient department and a median age of 46 years (33, 65) in the inpatient department. The inpatients’ age was older than the outpatients’s (P=0.003). Most outpatients were sent to the hospital within the first 3 days after the quake (73.50%), while less than half of the inpatients were sent to the hospital during this period of time. The wounded were mainly from Dujiangyan, Aba Prefecture and Pengzhou, which was correlated with the transportation distance, the casualty and the condition of the local hospitals. The wounded were mainly admitted into the Departments of Orthopaedics (69.3%), Neurosurgery (15.6%) and General Surgery (3.7%). The total cost in the Department of Orthopaedics was the highest, followed by the Intensive Care Unit (ICU) and the Department of Neurosurgery. However, the median cost in the ICU was the highest, followed by the Department of Neurosurgery and the Department of Nephrology. Only 9 out of the 563 patients died, including 5 outpatients and 4 inpatients, of whom 8 deaths were due to craniocerebral injury and 1 died of crush syndrome. Conclusion Based on the data, it is of vital importance to develop an emergent plan for the medical rescue after an earthquake disaster, and to strengthen the reserve of medical supplies, personnel training, scientific field triage as well as the construction of information platforms.
