    • Compliance status and reason analysis of patients with intermittent pneumatic compression device after lumbar surgery

      Objective To investigate the compliance status of intermittent pneumatic compression device (IPCD) in patients after lumbar surgery, and to analyze the reasons and influencing factors affecting compliance. Methods The continuous enrollment method was used to select patients who underwent posterior decompression for lumbar degenerative diseases in the orthopedic department of Peking Union Medical College Hospital between December 2022 and June 2023. The general information of patients and their compliance with IPCD were collected, and the reasons that affected compliance were analyzed. ResultsA total of 46 patients were included. The overall proportion of patients with good compliance was relatively low, and their compliance was poor. On the first and second day after surgery, the daytime compliance was relatively good (the proportion range of excellent compliance was 39%-52%); the compliance at night was significantly lower than that during the daytime (the proportion range of excellent compliance was 21%-26%); after 3 days of surgery, the patients’ compliance significantly decreased. A total of 460 time periods were observed and 195 reasons for not using IPCD were collected. The main reasons were physical discomfort caused by the device, inconvenience during bedside activities, and little significance to the patient. The compliance of female patients was better than that of males (P<0.05). Patients with education level of primary school and below had the highest compliance, while patients with high school and above had the lowest compliance (P<0.05). There was no statistically significant difference in compliance among patients of different age groups (P>0.05). Conclusions The overall compliance of patients with IPCD after lumbar surgery is poor, which is an urgent clinical problem, and the adaptability and cognitive level of patients to the device are the main factors affecting compliance. In clinical nursing work, men and patients with higher education level should be strengthened.

    • Laser treatment for premacular hemorrhage

      Objective To evaluate the effective method for treatment of premacular hemorrhage. Methods In 36 cases (36 eyes) of premacular hemorrhage (2PD),25 cases were treated with medicine, and 11 cases were treated with laser surgery and medicine. Results In 25 patients treated with medicine, the average time of vision recovery was 24 days and the average time of clearance of vitreous hemorrhage was 40 days; in the other 11 patients treated with laser surgery and medicine,10 were treated successfully, and in these 10 patients, the average time of vision recovery were 5 days and the average time of clearance of premacular hemorrhage were 11 days in 10 cases which were operated successfully with laser in 11 cases. Conclusions Laser surgery of inducing preretinal hemorrhage to vitreous body is an effective method for treatment of premacular hemorrhage to relieve the impaired central vision.     • Risk factors and clinical characteristics of late-onset septicemia in neonates

      Objective To explore the risk factors, clinical characteristics and pathogenic bacteria of late-onset septicemia (LOS) in neonates, so as to guide clinical diagnosis and treatment. Methods Collect LOS in neonates admitted to the Department of Pediatrics, Chaohu Hospital of Anhui Medical University between January 2015 and February 2020, and set them as the observation group. The neonates born at the same time and hospitalized without septicemia were selected as the control group. The general situation and risk factors of the two groups of neonates were analyzed, and the clinical manifestations, complications and pathogenic bacteria of LOS in neonates were analyzed. Results 182 neonates were enrolled, 91 in each group. There were significant differences between the two groups in mechanical ventilation, indwelling peripherally inserted central catheter, parenteral nutrition, tracheal intubation, neonatal asphyxia, gestational age<37 weeks, birth weight<2.0 kg (P<0.05). Logistic regression analysis showed that gestational age<37 weeks [odds ratio (OR)=3.010, 95% confidence interval (CI) (1.489, 6.085), P=0.002], parenteral nutrition [OR=3.506, 95%CI (1.681, 7.312), P=0.001] were independent risk factors for LOS. The main clinical manifestations of LOS were abnormal temperature, feeding difficulties, jaundice, apnea, hypersensitive C-reactive protein and procalcitonin increase. The neonates with LOS were prone to necrotizing enterocolitis and purulent meningitis. A total of 74 pathogenic bacteria were cultured from neonates with LOS, including 49 Gram-positive bacterium, 21 Gram-negative bacteria and 4 fungi. The critical and death cases were mainly infected by Gram-negative bacteria. Conclusions A number of factors are related to LOS. Gestational age<37 weeks and parenteral nutrition are independent risk factors for LOS. In order to avoid LOS, attention should be paid to prevention, aseptic concept should be strengthened, and drugs should be used reasonably.

    • A COMBINATION OF ARTERIOVENOUS IMPULSE SYSTEM AND LOW-MOLECULAR-WEIGHT HEPARINS CALCIUM FOR PROPHYLAXIS OF DEEP VENOUS THROMBOSIS FOLLOWING TOTAL KNEE ARTHROPLASTY

      Objective To analyze the effect of arteriovenous impulse system (AVIS) combined with lowmolecular-weight heparins calcium (LMWHC) for prophylaxis of deep vein thrombosis (DVT) following total knee arthroplasty (TKA). Methods From March 2006 to March 2008, 76 cases of osteoarthritis patients (76 knees) accepted TKA, including 25 males and 51 females with an average age of 66.6 years (range, 58-79 years). The affected knees were left side in 41 cases and right side in 35 cases. They were randomly divided into experimental group and control group before surgery. Then LMWHC and rehabil itation training were routinely given in two groups before and after surgery. However, only experimental group was treated with AVIS continually during the first four days and then two times a day for 30 minutes one time during 5-7 days. At 7 daysd after operation, color Doppler ultrasound was used to detect the occurrence condition of DVT. Results Five cases (13.16%) had thrombosis of calf and recovered after treated with urokinase and salvia in the experimental group. Eleven cases had thrombosis of calf and 3 cases had thrombosis of whole low extremities (36.84%), and improved after treated with urokinase and salvia in the control group. There was significant difference in DVT incidencerate between two groups (P lt; 0.05). No pulmonary embol ism or death was found in both groups. Conclusion AVIScan effectively accelerate the venous blood return velocity, a combination of AVIS and LMWHC has a better effect in theprevention of DVT following TKA.

    • Medical computer-aided detection method based on deep learning

      This paper performs a comprehensive study on the computer-aided detection for the medical diagnosis with deep learning. Based on the region convolution neural network and the prior knowledge of target, this algorithm uses the region proposal network, the region of interest pooling strategy, introduces the multi-task loss function: classification loss, bounding box localization loss and object rotation loss, and optimizes it by end-to-end. For medical image it locates the target automatically, and provides the localization result for the next stage task of segmentation. For the detection of left ventricular in echocardiography, proposed additional landmarks such as mitral annulus, endocardial pad and apical position, were used to estimate the left ventricular posture effectively. In order to verify the robustness and effectiveness of the algorithm, the experimental data of ultrasonic and nuclear magnetic resonance images are selected. Experimental results show that the algorithm is fast, accurate and effective.

    • Expression of hepatocyte growth factor receptor in human epiretinal membranes and RPE cells

      Objective To investigate the expression of hepatocyte growth factor receptor (HGFR) in epiretinal membranes (ERM) of eyes with proliferative vitreoretinopathy (PVR) and cultured retinal pigent epithelium (RPE) cells. Methods Fifteen human epiretinal membranes were obtained from eyes undergone vitrectomy for rhegmatogenous retinal detachment complicated with PVR and observed by immunohistochemical examination to study the expression of HGFR. Using the immunohistochemical technique to evaluate the expression of HGFR in cultured RPE cells. Results In 6 membranes of PVR-grade C, HGFR were expressed in 5/6, and 7 cases were detected in 9 membranes of PVR-grade D.RPE cells express readily detectable levels of HGFR. Conclusion The findings indicate that HGF might be involved in the formation of epiretinal membranes in PVR. (Chin J Ocul Fundus Dis, 2002, 18: 221-223)

    • A comparative study of hepatocyte growth factor levels in the vitreous of patients with diabetic retinopathy

      Objective To determine the concentration of hepatocyte growth factor (HGF) in vitreous of diabetic retinopathy (DR) and investigate the result and influence of HGF in neovascularization in proliferative diabetic retinopathy (PDR). Methods The high sensitive sandwich enzyme-linked immunosorbent assay (ELISA) was used to measure the level of HGF in vitreous of normal group for 10 eyes, simple DR group for 7 eyes, PDR without iridal neovascularization group for 24 eyes, PDR with neovascularization of iris group in 9 eyes, other retinopathy group associated with neovascularization for 8 eyes. Results The mean value of HGF level in vitreous of the former groups was: (3.3 ±1.9) μg/L in normal group; (4.8±2.5) μg/L in simple DR group; (13.0 ±5.2) μg/L in PDR without iridal neovascularization group; (18.6±7.2) μg/L in PDR with neovascularization of iris group;(12.1±8.9) μg/L in other retinopathy associated with neovascularization group. Stastistics showed that HGF level in PDR group and other retinal diseases associated with neovascularization were significantly higher than that in normal group (P<0.01), PDR with neovascularization of iris group showed a higher mean vitreous HGF concentration than those in PDR without iridal neovascularization and simple DR group (P<0.05 or P<0.01). Conclusion Our results indicate that vitreous HGF may play an important role in retinal neovascularization in PDR and other retinal diseases. (Chin J Ocul Fundus Dis, 2002, 18: 131-133)

    • Forensic Pathological Analysis of Death Cases after Cardiac Valve Replacement

      【摘要】 目的 分析心臟瓣膜置換術后死亡案例的法醫病理學特點,探討有關瓣膜置換術后死亡案例的法醫病理學鑒定思路及原則。 方法 從手術方式、術前心臟狀況及術中所見、法醫病理檢驗所見、死亡時間、死亡原因等方面,對2008年1月-2010年2月4例心臟瓣膜置換術后死亡案例進行綜合研究分析。 結果 4例均患有風濕性心臟病,且術前心功能較差;3例二尖瓣瓣膜置換術,3例主動脈瓣瓣膜置換術,2例三尖瓣成形術;二尖瓣瓣膜置換術1例平行房間溝的左房縱行切口,1例右心房-房間隔切口,1例右心房-房間隔-左房聯合切口;主動脈瓣瓣膜置換術均行升主動脈根部斜形切口或S狀切口;死亡時間為1例術中,1例術后30 min,1例術后2 d,1例術后8 d;死亡原因為1例心肌炎,1例傳導系統出血,2例失血性休克;1例術前有潛在感染灶。 結論 對有關心臟瓣膜置換術后死亡案例的法醫病理學鑒定時,應在了解患者瓣膜置換術前心臟情況以及手術方式、手術路徑和縫合方法的基礎上,結合法醫病理組織學檢查進行綜合分析。【Abstract】 Objective To analyze the forensic pathological characteristics of those who died after cardiac valve replacement and explore the mentality and principle of documentary evidence of medicolegal expertise. Methods Four death cases after cardiac valve replacement between January 2008 and February 2010 were comprehensively analyzed from various aspects of surgery style, preoperational heart condition, peri-operational observations, results of forensic pathological tests, time of death, and causes of death. Results All cases were rheumatic heart disease and the preoperational heart function was poor. Of the four cases, three had mitral valve replacement (MVR), three had aortic valve replacement (AVR) and two had tricuspid valvuloplasty. Of the three MVR cases, cutting on the left atrium paralleling the interatrial groove was performed in one case, cutting through right atrium and interauricular septum was performed in one case, and cutting left atrium and right atrium through interauricular septum was performed in another case. For all the three AVR cases, cutting on the root of aorta with the shape of diagonal or ’S’ was carried out. One patient died during operation, one died thirty minutes after operation, one died two days after operation and one died eight days after operation. Among them, one died of myocarditis, one died of hemorrhage in the conducting system and two died of hemorrhagic shock. Besides, one patient had potential bacterial infection before surgery. Conclusion For patients who died after cardiac valve replacement, we should acquaintance ourselves with the preoperational heart condition, surgery style, surgical approach and the methods of stitch to make a comprehensive analysis with forensic pathology examination.

    • Research progress of imaging in postoperative efficacy evaluating of hepatocellular carcinoma after transcatheter arterial chemoembolization

      ObjectiveTo summarize research progress of imaging methods in postoperative efficacy evaluating of hepatocellular carcinoma (HCC) after transcatheter arterial chemoembolization (TACE).MethodsThe related literatures of imaging methods in postoperative efficacy evaluating of HCC after the TACE were searched in the PubMed, Web of Science, CNKI, and Wanfang Data databases, etc. The clinical efficacy, advantages and problems of different imaging were analyzed and summarized.ResultsThe contrast-enhanced ultrasound (CEUS) had the higher value in diagnosing of residual or recurrent lesion, but the observation of the multiple or larger lesion was limited on the CEUS. The contrast-enhanced computed tomogaphy (CECT) had the lower diagnostic value for the residual or recurrent lesions as compared with the CEUS due to the artifact of iodine oil deposition. However, the CT perfusion and gemstone spectral imaging parameters could help to predict postoperative efficacy, tumor residue, and tumor recurrence after the TACE. The combination of dynamic contrast enhanced magnetic resonance imaging (DCE-MRI), functional and perfusion imaging had the better diagnostic value, and the liver function could be predicted by the DCE-MRI. The PET/CT had the excellent value in diagnosing of the residual or recurrent lesion and could be used in predicting of the patient’s survival after the TACE. The DSA was the reference standard in diagnosing of the residual or recurrent lesion after the TACE for the HCC patients, however, it had the disadvantages of invasiveness and side effect and the high operational requirements should be emphasized.ConclusionsImaging examinations including ultrasound, CT, MRI, PET/CT, and DSA are widely applied to evaluate curative effect and residual or recurrent lesion of HCC after TACE. However, principles and imaging methods of mentioned above diagnostic methods are inconsistent, as well as results of research are quite different, therefore, clinician should choose the best imaging method according to patient’s condition in diagnosis and treatment.

    • Comparison of hospitalization expenses for identical diseases between traditional Chinese medicine and Western medicine hospitals based on interrupted time-series analysis

      ObjectiveIn light of the comprehensively implemented reform of medical insurance payments, this study analyzed the impact of the payment intervention and COVID-19 pandemic on hospitalization expenses for identical diseases between traditional Chinese medicine (TCM) and Western medicine hospitals, to provide evidence to promote high-quality coordinated development of hospitals and insurance while reducing patient load. MethodsFrom January 2014 to December 2020, we gathered data including 9 900 individual medical records of woman-related malignant tumors (WMT) from all 23 public hospitals in a district of Shanghai. We developed an interrupted time-series analysis model based on the above two interventions, to compare the inpatient average per-time expenses between different hospitals and different groups. ResultsThe average per-time expenses of WMT in Western hospitals changed from rising to declining after the policy intervention, and increased again during the pandemic. In TCM hospitals, the expenses continued to increase and fluctuated after the pandemic. ConclusionThe policy intervention has achieved a good effect on controlling the cost of Western hospitals, rather than the significant increase in TCM hospitals. Meanwhile, the COVID-19 pandemic has had a significant impact on hospitalization expenses. It’s urgent to develop a payment model that fits the development and characteristics of TCM, to control the unreasonable growth of expenses. Moreover, the financial compensation methods and supervision mechanism of public hospitals should be improved to effectively resist the threat of public health emergencies for the development of hospitals and the legitimate rights of patients.
