Objective To investigate the knowledge level about pulmonary rehabilitation in respiratory physicians in Shanghai. Methods A self-designed questionnaire about pulmonary rehabilitation was sent to respiratory physicians in 18 tertiary-care referral hospitals of Shanghai from June to September 2011. Results A total of 237 valid questionnaires were collected. Accuracy rate of single-answer questions was(62.1±18.3)% , while correct rate of multiple-answer questions ( more than one answer) was ( 35.5±15.6) % . Neither working years nor doctor rank had correlation with accuracy of questionnaires. Conclusion The knowledge about pulmonary rehabilitation in respiratory physicians of Shanghai was poor. We need to strengthen the relevant training and continuing education.
ObjectiveTo investigate the prevalence and associated factors of diabetic retinopathy (DR) among diabetic residents in Longyan of Fujian Province. MethodsA investigative research. From January 2022 to December 2023, a total of 10 061 diabetic patients enrolled in the chronic disease follow-up management system from 112 towns and sub-districts in 7 counties and districts of Longyan of Fujian Province were selected as the target population. A questionnaire survey, routine physical examination, vision test, and non-mydriatic fundus photography were conducted. A total of 762 cases with missing height, weight, blood pressure, fasting plasma glucose (FPG), and diabetes duration, and 507 cases with unclear fundus photography were excluded, resulting in 8 792 cases included in the final statistical analysis. DR diagnosis and classification were based on the 2019 International Clinical Classification of DR. The prevalence of DR was calculated for single-eye or double-eye DR cases as 1 case; the more severe eye was used for DR grading in double-eye DR cases. Statistical analysis was performed by grouping based on the presence or absence of DR and dividing into age groups ≤67 years and >67 years. χ2 test was used to analyze factors associated with prevalence; binary multivariate logistic regression analysis was employed to identify influencing factors of DR. ResultsAmong the 8 792 cases, 888 (10.1%, 888/8 792) were diagnosed with DR (DR group), and 7 904 (89.9%, 7 904/8 792) had no DR (non-DR group). Compared to the non-DR group, the DR group showed significant increases in FPG (Z=?12.448), diabetes duration (Z=?18.936), systolic blood pressure (Z=?4.237), diastolic blood pressure (Z=?2.881), and body mass index (BMI) ≥24 kg/m2 (P<0.001). Significant differences were also found between the two groups in hypertension (χ2=11.450), hyperlipidemia (χ2=5.100), kidney disease (χ2=7.039), family history of diabetes (χ2=5.025), and regular medication use (χ2=66.034) (P<0.05). There were 4 688 cases in the ≤67 years group and 4 104 in the >67 years group. In the ≤67 years group, significant differences in DR prevalence were found for FPG levels (χ2=111.754), diabetes duration (χ2=231.658), BMI (χ2=12.404), systolic blood pressure (χ2=17.912), regular medication use (χ2=40.727), hyperlipidemia (χ2=6.816), and hypertension history (χ2=6.775) (P<0.05). In the >67 years group, significant differences in DR prevalence were found for FPG levels (χ2=59.916), diabetes duration (χ2=128.362), systolic blood pressure (χ2=5.183), regular medication use (χ2=22.097), kidney disease (χ2=6.251), and family history of diabetes duration (χ2=4.967) (P<0.05). No significant differences in DR prevalence were found based on sex, education level, smoking history, alcohol consumption, exercise habits, heart disease history, or other family disease history (P>0.05). Logistics regression analysis results show that patients aged >67 years, FPG [odds ratio (OR)=1.074, 95%confidence interval (CI) 1.046-1.102], diabetes duration (OR=1.088, 95%CI 1.071-1.106), systolic blood pressure (OR=1.007, 95%CI 1.001-1.013), and kidney disease (OR=3.617, 95%CI 1.268-10.320) were identified as risk factors for DR (P<0.05). In patients aged ≤67 years, FPG (OR=1.088, 95%CI 1.067-1.110), diabetes duration (OR=1.108, 95%CI 1.091-1.125), and systolic blood pressure (OR=1.008, 95%CI 1.003-1.013) were identified as independent risk factors for DR (P<0.05), while BMI ≥24 kg/m2 (OR=0.934, 95%CI 0.908-0.965) was a protective factor for DR (P<0.05). Age, regular medication use, hypertension, and hyperlipidemia were identified as potential confounding factors for DR occurrence. ConclusionsThe prevalence of DR among diabetes patients in Longyan of Fujian Province, is 10.1%. FPG, diabete duration, and systolic blood pressure are independent risk factors for DR, while age, regular medication use, hypertension, and hyperlipidemia are potential confounding factors for DR occurrence.
ObjectivTo investigate the incidence of depression and its etiological factors in patients with hypospadias after operation. MethodsFrom January to June 2015, we investigated the incidence of depression symptoms among patients with hypospadias after surgical treatment from January 1990 to December 1994 in Zhangzhou Affiliated Hospital of Fujian Medical University, and we matched them with mentally healthy adults of the same age to 1:1 ratio. Zung Self-Rating Depression Scale and Correlation Factor Questionnaire were used to investigate and analyze the related factors of depression symptoms between the patients with hypospadias and the healthy males. ResultsA total of 80 patients with hypospadias after surgical treatment and 80 healthy males as control were included. There were no significant differences in male secondary sexual development, testis development, serum testosterone levels and postoperative length and girth of the penis in two groups. The incidence rate of depression symptoms was 45.0% (16/80) in the hypospadias patients after operation, extremely significantly higher than 6.3% (5/80) in the control group (χ2=6.632, P=0.01). The result of multiple stepwise regression analysis showed that the main risk factors of depression symptoms were worries about dissatisfaction with penile and scrotal appearance (F=16.210 3, P=0.001), sexual satisfaction (F=4.621 2, P=0.036) and sexual function (F=4.103 2, P=0.043). ConclusionSymptoms of depression often occur in hypospadias patients after operation, and the major etiological factors are dissatisfaction with penile and scrotal appearance, sexual satisfaction and sexual function.
Objective To investigate human resource allocation in primary health care and the essential medical service and publ ic health service status in urban and rural areas in Chengdu, so as to provide basel ine data for the Special Healthcare Program of Comprehensive Reform for Coordinated and Balanced Urban-Rural Development in Chengdu. Methods We carried out a stratified (three circles in Chengdu) sampl ing of 7 township hospitals (rural hospitals) and community health service centers; and then performed secondary research based on a comparative analysis of relevant pol icies of the World Health Organization (WHO) and Chinese governments at all levels. Results According to the WHO and national average standards, the number of staff per 1 000 rural hospitals / centers health personnel of the 7 rural hospitals / centers occupied only 1%-22% of the global average standard. There was a very large gap between the number of staff and the number of personnel required, based on the size of the population that should be served in the administrative areas in 2006 or the number of cl inic patients in 2006. The primary healthcare personnel structure was irrational. For example, the constituent ratio of health technical personnel was 4% to 33% higher than the global average level, and the constituent ratio of (assistant) physicians was also 17% to 45% higher than the global average level. However, the ratio of nurses, laboratory workers, other health professionals, administrative and supporting personnel was generally lower than the global average level. Women dominated among the primary healthcare personnel, and people aged 45 years or below counted for more than 75% (except Bailu and Wangjiang rural hospitals/centers). People with an educational background of two-year college education or secondary education or below took up 70% to 90%; while those with an intermediate title or assistant /primary title accounted for 50% to 100%. The structure rational ity of distribution density, educational background and academic titles of healthcare personnel showed a decreasing trend from the first circle to the third circle in Chengdu city. Conclusion The primary health workers in the second and third circle have been overloaded with low incomes for some time. They are facing enormous challenges in their professional skills, service awareness, as well as difficulties in continuing education and professional title promotion. It is very difficult to provide qual ified "six in one" primary health care and publ ic health services in a long-term and stable manner. It is suggested that we enroll and train more skilled people for primary health care service, and provide continuing education chances for current health care personnel. We should also adopt a mechanism to select qual ified personnel based on their performance, and take measures to solve some of the problems faced by the grass-root health personnel, such as heavy work burden, low income, poor skill and promotion. This will help us to construct a stable and qual ified primary healthcare team.
Objective To investigate the use of evidence-based medicine (EBM) and barriers to applying EBM in clinical practice in medical postgraduates. Methods Three hundred and sixty-five first and second-year medical postgraduates selecting the EBM courses were included. All the students were surveyed by questionnaires. Result The response rate to the survey was 100%. About half of the students read 1~5 professional articles per month before postgraduate study. Fifty-six percent of the students used professional literature and research findings when making clinical decisions. Fifty-seven point three percent used electronic databases to search for practice-relevant literature before postgraduate study. Sixty-one point four percent found it difficult to obtain relevant clinical guidelines and 68.5% never applied relevant clinical guidelines in clinical decision-making. The barriers to using EBM in clinical practice included difficulties in searching and accessing full text of potentially relevant articles, lack of EBM knowledge, influences from clinical experience and authorities, uncertainties about the quality of evidence and time-consuming. Conclusion EBM clinical practice and EBM medical education has transformed from dissemination and popularization stage to skill acquisition stage.
Objective To evaluate the status of asthma control in asthmatic outpatients.Methods We performed an investigation by a questionnaire in a face-to-face setting from Feb 2006 to May 2006 in asthmatic outpatients of China-Japan Friendship Hospital.Results A total of 101 asthmatic patients were investigated with a mean age of 47±14.8 years and course of disease of 9.1±12.8 years.80.2% of the asthmatic patients had various social insurance.40.6% of the respondents had visited emergency department because of asthma exacerbation.The percentage of adults with lost workdays caused by asthma was 61.7% (29/47),and which of children with lost schooldays was 75% (3/4).37.6% of asthmatic patients were completely controlled.Approximately three fourth of respondents (75.2%) was either well or completely controlled.72.3% of respondents had undergone a lung-function test during the past year.The one third of respondents (36.6%) owned oneself peak flowmeter.Only 12.9% reported kept regular use of peak flowmeter.87.1% of patients use inhaled corticosteroids (ICS) regularly.Conclusion With the implementation of patient education program and asthma guideline,the asthma control level has been further improved.
Objective To get the information about the cognition and attitude towards organ transplantation and donation in inpatients. Methods The inpatients were investigated by using a self-designed questionnaire,including general data,cognition and attitude towards organ transplantation and donation. The influence factors of organ transplantation and donation were analyzed by SPSS 18.0. Results A total of 1 000 questionnaires were delivered,973 returned,the effective questionnaire was 906,and the effective rate was 93.1%. In 906 valid questionnaires,788 (87.0%) patients knew organ transplantation;189 (20.9%) patients had transplanted relatives;511 (56.4%) patients were willing to accept the organ transplant surgery,226 (24.9%) patients did not want,169 (18.7%) patients did not know;490 (54.1%) patients agreed to donate organ,173 (19.1%) patients disagreed,243 (26.8%) patients did not know. The age,education,occupation,knowing organ transplants,and with organ transplanted relatives or not had significant influences to the willingness towards organ transplantation and donation (P<0.01). The gender had no effect on organ transplantation (P=0.727) and donation (P=0.935) concerned issues,but the age,education,knowing organ transplantation,and with organ transplanted relatives or not had significant influences on it (P<0.01). Most concerned factors about organ transplantation were as follows:with an available organ for transplantation 〔28.7% (260/906)〕,postoperative quality of life 〔23.0% (208/906)〕,risk of surgery 〔21.5%(195/906)〕,surgery costs 〔19.5% (177/906)〕,and postoperative rejection 〔7.3% (66/906)〕. Most concerned factors about organ donation were as follows:the reasonable usage of donated organs 〔57.4% (520/906)〕,attitude of relatives 〔23.8% (216/906)〕,and donation compensation 〔17.6% (159/906)〕,and others 〔1.2% (11/906)〕. According to the way of organ donation,403 (44.4%) patients were willing to accept donation after cardiac death (DCD),257 (28.4%) patients accept donation after brain death,246 (27.2%) patients accept living organ donation. The gender,education,and occupation had significant influences on the way of organ donation (P<0.05),but age,knowing organ transplantation,and with organ transplanted relatives or not had no effects on it (P>0.05). Conclusions The lack of propaganda and the high costs of transplantation surgery are the main factors restricting the acceptance rate of organ transplantation. Over time,with continuous improvement of universal education and strengthening the publicity of organ transplantation,the acceptance rate will show a constant increase in the trend. By now,the most accepted way of organ donation is DCD in inpatients. The establishment of a reasonable organ allocation and compensation system is a corner stone to promote organ donation.
Objective To study the cause of the low rate of breast conservation and reconstruction by investigating the approval degree of breast conserving therapy and breast reconstruction of women with breast diseases, to help the breast surgeons make better communication with the patients and make more pertinent choices of therapeutic methods. Methods The age, occupation, educational background, the attitudes towards breast conserving therapy and breast reconstruction, and the choice of operative method of breast reconstruction of 139 patients with breast cancer and 224 patients with benign breast disease were investigated by questionnaire. Results In breast cancer group, 23.9% (28/117) of patients chose breast conserving therapy and 35.9%(42/117) of patients chose breast reconstruction, while the rates of breast conservation and reconstruction were 53.3% (106/199) and 63.8% (127/199) in benign breast disease group. In both groups, the higher rates of breast conservation and reconstruction were associated with better educational background (in breast cancer group: P=0.029, P=0.296; in benign breast disease group: P=0.081, P=0.019) and lower age (all Plt;0.05). Patients engaged in commerce showed higher rates of breast conservation and reconstruction (in breast cancer group: P=0.013, P=0.042; in benign breast disease group: P=0.032, P=0.044). Age, occupation or educational background was not related with the choice of operative method of breast reconstruction (Pgt;0.05). Conclusions Patients with lower age, better educational background, and better job condition have ber desire of breast conservation and reconstruction. Breast surgeons should enhance communication with those patients about relevant information of breast conservation and reconstruction to make the more pertinent choice of therapeutic methods.
Objective By investigating the employment situation and intentions of medical students, to provide scientific evidence for guiding students’ employment by relevant departments, and to give references for formulating reasonable medical personnel training and distribution policy by health administrations. Methods The medical students of Grade 2005 in Lanzhou University were investigated by using a self-designed questionnaire. Results Among 397 questionnaires distributed, 337 were returned (84.8%). The analyses showed, when hunting a job, 45% of the respondents thought the “opportunities for personal development” was the most important factor, 61% thought the influence of parents’ or family’s desires was “greater” or “extremely greater”, 85% thought the job matching their majors was “important” or “very important”, 57% thought it is “easier” to get a content job by their majors, 51% held that the “fewer opportunities” were the most unfavorable conditions for working in township hospitals, 68% wouldn’t directly choose township hospitals after graduation, and 67% still gave up the choice even if an official position was provided. Conclusion The medical students attach great importance to the opportunities for their personal development, but they lack self-consciousness in choosing the occupation. They are optimistic about the employment but lack passion for primary hospitals. Meanwhile, they aren’t familiar with national policy guidance; their employment fields are narrow.
ObjectiveTo investigate the status-quo of urban and rural residents' depression in the communities of Chengdu city and its influencing factors. MethodsA multi-stage stratified cluster sampling survey was carried out from February to October 2010. Residents aged from 35 to 70 years in 4 urban and rural communities of Chengdu were investigated in the incidence of depression. The health survey questionnaire and PHQ-2 Depression Screening Scale were used for the household investigation. Data was analyzed using SPSS 18.0 software, and the risk factors were also analyzed using multivariate logistic methods. ResultsA total of 2 027 residents were interviewed, including 1 015 urban (616 women and 399 men, with mean age of 58.90 ±9.48 years) and 1 012 rural residents (582 women and 430 were men, with mean age of 54.94 ±9.64 years). The results showed that the overall depression prevalence rate of residents aged from 35 to70 years old in Chengdu was 2.91% (59/2 027); and the prevalence was 4.33% (44/1 015) in urban and 1.48% (15/1 012) in rural. The prevalence was significantly higher in urban areas than that in rural areas (χ2=13.296, P < 0.001). The results of multivariate analysis showed that a significant difference was found in the prevalence of depression among the different marital status groups, and more divorced people suffered from depression. No statistically significant correlation was found between the factors (including occupation, education level and family income status) and depression. ConclusionThe prevalence of depression among residents in urban and rural communities of Chengdu was high, and the divorced population should be paid more attention to in the prevention and treatment of depression.
