    • Expressions and Significance of miR-196b mRNA and HoxB8 mRNA in Human Colorectal Cancer

      Objective To research the expressions of miR-196b and HoxB8 mRNA in colorectal cancer and theircorrelation with clinicopathologic features,and to explore the relationship between miR-196b and HoxB8 in vivo. Methods Expressions of RNA (including miR-196b and HoxB8 mRNA) and HoxB8 protein were detected respectively by using quantitative real-time reverse transcriptase PCR and Western blot in 30 cases of colorectal cancer and corresponding normalmucous membrane tissues. Results In colorectal cancer tissues,expressions of miR-196b and HoxB8 mRNA were higher than those of the corresponding normal mucous membrane tissues (P<0.05). Expression of miR196b mRNA was assoc-iated with lymph node metastasis,neoplasm stages (Ⅰ+ⅡandⅢ+Ⅳ),and distant metastasis (P<0.05),on the otherhand,no significant differences were observed regarding tumor site,size,gross type,depth of invasion,tissue differentiation,age,and sex (P>0.05). Expression of HoxB8 mRNA was no significant differences concerning lymph node metastasis,tumor stages (Ⅰ+Ⅱ,Ⅲ+Ⅳ),distant metastasis,tumor site,size,gross type,depth of invasion,tissue differentiation,age,and sex (P>0.05). The expression of miR-196b mRNA was negatively correlated with HoxB8 mRNA expression (r=-0.458,P<0.05),and HoxB8 protein expression with no obvious correlation (r=-0.236,P>0.05) in colorectal cancer tissues. Conclusions The expressions of miR-196b and HoxB8 mRNA in colorectal cancer tissues are higher,the high expression of miR-196b mRNA is related to the tumorigenesis and progression of colorectal cancer as well as correlated with prognosis in colorectal cancer. The miR-196b inhibits the expression of HoxB8 mRNA by binding to the3′-UTR of target HoxB8 mRNA.

    • An improved algorithm for electrohysterogram envelope extraction

      Extraction uterine contraction signal from abdominal uterine electromyogram (EMG) signal is considered as the most promising method to replace the traditional tocodynamometer (TOCO) for detecting uterine contractions activity. The traditional root mean square (RMS) algorithm has only some limited values in canceling the impulsive noise. In our study, an improved algorithm for uterine EMG envelope extraction was proposed to overcome the problem. Firstly, in our experiment, zero-crossing detection method was used to separate the burst of uterine electrical activity from the raw uterine EMG signal. After processing the separated signals by employing two filtering windows which have different width, we used the traditional RMS algorithm to extract uterus EMG envelope. To assess the performance of the algorithm, the improved algorithm was compared with two existing intensity of uterine electromyogram (IEMG) extraction algorithms. The results showed that the improved algorithm was better than the traditional ones in eliminating impulsive noise present in the uterine EMG signal. The measurement sensitivity and positive predictive value (PPV) of the improved algorithm were 0.952 and 0.922, respectively, which were not only significantly higher than the corresponding values (0.859 and 0.847) of the first comparison algorithm, but also higher than the values (0.928 and 0.877) of the second comparison algorithm. Thus the new method is reliable and effective.

    • Research progress of different methods to improve the taste and adverse reactions of intestinal cleansing agents

      Colonoscopy examination is the gold standard for diagnosis and treatment of colon diseases and screening for early cancer. Good quality of bowel preparation, which is also called intestinal cleanliness, is the prerequisite for full visualization of colonic mucosal lesions and their details. In clinic, more intestinal cleansing agents, such as polyethylene glycol and magneium suifate, have bad taste and high dosage, which can not be tolerated by some patients and affect the effect of bowel preparation. The use of oral adjuvant, add adjuvant to intestinal cleansing agents, new low-volume or very low-volume polyethylene glycol-ascorbic acid preparation methods, can reduce the incidence of adverse reactions, such as nausea and vomiting in patients, improve their tolerance, satisfaction and so on. This article reviews the methods of improving the taste and reducing the adverse reactions of intestinal cleansing agents at home and abroad to offer references for clinical practice.

    • Establishment and application of control range of monthly nosocomial infection incidences in different departments

      Objective To establish the control range of monthly nosocomial infection incidences in different departments and put them into practice, to provide a scientific and effective method for nosocomial infection control. Methods The surveillance data about nosocomial infection cases in Nanchong Central Hospital from January 2016 to December 2018 were used to set the warning limits and control limits in different departments based on the theory of medical reference range. From January 2019, the clinical departments would be alerted if their nosocomial infection incidences were beyond the warning limits, and investigated and intervened if the incidences were beyond the control limits. Results The control range of monthly nosocomial infection incidences in different departments had been made. For identifying risk events, the sensitivity was 83.3%, the specificity was 96.2%, the positive predictive value was 29.4%, the negative predictive value was 99.7%, the coincidence rate was 96.0%, and the consistency was medium (kappa=0.419, P<0.001). The effective rate of the initial alert intervention was 83.3%, and the effective rate of the field intervention was 100.0%. Conclusion The establishment and application of the control range of monthly nosocomial infection incidences in different departments can identify potential risk events and realize precise nosocomial infection control.

    • Relationship between AST/ALT Ratio and Thyroid Hormone Level in Patients with Chronic Hepatitis B

      【摘要】 目的 通過檢測臨床確診為慢性乙型肝炎患者血清谷丙轉氨酶(ALT)、谷草轉氨酶(AST)、甲狀腺激素T3、T4及促甲狀腺素(TSH)含量,進一步探討慢性乙型肝炎患者肝損情況與甲狀腺分泌功能的關系。 方法 收集2009年1-4月79例確診為慢性乙型肝炎患者(病例組)血清標本,分別檢測其ALT、AST、甲狀腺激素T3、T4及TSH含量。以同期健康體檢者30例作為對照組。比較兩組的差異及各指標間的相關性。 結果 病例組AST為(87±113) U/L、ALT為(135±241) U/L、AST/ALT為0.97±0.57、TSH為(1.63±1.29) mU/L、T3為(1.61±0.52)ng/mL、T4為(10.7±2.9) μg/dL,對照組分別為(23±5) U/L、 (18±5) U/L、1.31±0.26、(2.13±0.90) mU/L、(1.19±0.16) ng/mL和(8.6±0.9) μg/dL,兩組各指標比較均有統計學差異(Plt;0.01)。所有指標均正常的共有6例(7.6%),有73例(92.4%)存在不同程度的指標異常;在68例AST/ALT比值降低的慢性乙型肝炎患者(93.2%)中,伴有單純T4升高的有11例(16.2%),單純T3升高的有4例(5.9%),T3、T4同時升高的有11例(16.2%),T3、T4同時升高且TSH降低的有2例(2.9%),1例T4升高且TSH降低(1.5%),1例僅TSH升高(1.5%);在4例AST/ALT比值正常的慢性乙型肝炎患者(5.5%)中,有1例T3、T4同時升高且TSH降低,1例T3和T4同時升高,1例單純T4升高,1例單純TSH降低;有1例僅AST/ALT比值升高而其他項正常。 結論 慢性乙型肝炎患者除AST/ALT比值異常外,還同時伴有不同程度的甲狀腺激素指標異常,其原因可能與慢性乙型肝炎應用干擾素治療時甲狀腺功能受損有關。【Abstract】 Objective To explore the relationship between liver damage and the secretion function of thyroid by detecting the serum alanine aminotransferase (ALT), aspartate aminotransferase(AST), T3, T4, and thyroid stimulating hormone (TSH) in patients with chronic hepatitis B. Methods The serum samples of 79 patients with chronic hepatitis B from January to April 2009 were collected, and the ALT, AST, T3, T4, and TSH concentrations were detected. Another 30 healthy subjects were selected as the control. The detected indexes were compared between the two groups. Results In the case group, the concentration of AST was (87±113)U/L, ALT was (135±241)U/L,AST/ALT was 0.97±0.57, TSH was (1.63±1.29) mU/L, T3 was (1.61±0.52) ng/mL, and T4 was (10.7±2.9) μg/dL; while in the control group, the concentrations of the items were (23±5) U/L, (18±5)U/L, 1.31±0.26, (2.13±0.90) mU/L, (1.19±0.16) ng/mL, and (8.6±0.9) μg/d L, respectively. The differences between the two groups were significant (Plt;0.01). Among the 79 case, 6 (7.6%) had a totally normal result, and 73(92.4%) had an abnormal result. There were 68 patients who had a low ratio of AST/ALT, among whom 11 (16.2%) had a simple T4 elevation, 4(5.9%) had a simple T3 elevation,11(16.2%) had an elevation of both T3 and T4, 2 (2.9%) had an elevation of both T3 and T4 and a depression of TSH, 1(1.5%) had an elevation of T4 and a depression of TSH, and 1 (1.5%) had a simple TSH elevation. There were 4 cases who had a normal ratio of AST/ALT, among whom 1 had an elevation of both T3 and T4 and a depression of TSH, 1 had an elevation of both T3 and T4, 1 had a simple T4 elevation, and 1 had a simple TSH depression. There was 1 case who had only an elevation of AST/ALT. Conclusion Most of the patients with chronic hepatitis B have an abnormal result of thyroid hormone together with the abnormal ratio of AST and ALT. The reason mainly lies in the damage of thyroid function by the usage of interferon for the therapy of chronic hepatitis B.

    • An analysis of the implementation path of the internal control system in colleges and universities under the digital intelligence

      ObjectiveTo analyze the problems in the construction of internal control in colleges and universities, and to explore the implementation path suitable for colleges and universities. MethodsThe relevant documents on the internal control construction of universities at home and abroad were retrieved, and the case study of the implementation path of the internal control system construction of C university was analyzed by using the methods of expert interviews, flow charts and walk-through tests. ResultsThe internal control system of colleges and universities takes digital intelligence as the technical support to explore the implementation path of "external standard rule riveting point, internal standard post entry process; data standardization, process progress visualization; speed of operation tracking, data backtracking to find deviation; business data is all smooth, performance evaluation to promote management", optimize the process and improve the system, real-time extraction of operation data for key projects to track, and shorten the business operation time by more than half. ConclusionThe case university carries out empirical research on the internal control system, based on the principle of effective implementation, makes full use of the digital intelligence technology to extract the whole process data of the business system across the system, and realizes the efficient collaboration, early warning and evaluation, and scientific decision-making of the "digital intelligence" platform.
