    • Long-term outcomes of surgical closure of atrial septal defects and concomitant surgical radiofrequency ablation for atrial fibrillation

      Objective To evaluate long-term outcomes of surgical closure of atrial septal defect (ASD) and combined surgical radiofrequency ablation for atrial fibrillation (AF). Methods A total of consecutive 15 patients with ASD undergoing surgical closure of ASD and combined surgical radiofrequency ablation in our department between March 2003 and April 2015. There were 7 males and 8 females at an average age of 47.1±10.8 years ranging from 16 to 62 years. Retrospective analysis and follow-up were performed to evaluate long-term success rate freedom from AF after surgery. Results All patients recovered and discharged, and no patient suffered death or stroke. The duration of follow-up was from 3 to 136 months for all patients. Success rate freedom from AF at 1, 3, 5 and 10 years was 81.3%, 75.0%, 68.8% and 61.1%, respectively. During follow-up, there was no death or stroke. One patient required permanent pacemaker implantation. Conclusion Concomitant surgical closure of ASD and biatrial radiofrequency ablation is safe and effetive with better long-term outcomes. It is necessary to perform the two procedures together for ASD patients.

    • Effect of 99Tcm-sc with carbon nanoparticles suspension injection on sentinel lymph node mapping in patients with colon cancer

      Objective To evaluate the feasibility of sentinel lymph node (SLN) mapping after 99Tcm sulfur colloid (99Tcm-sc) and carbon nanoparticles injection in patients with colon cancer. Methods Forty patients with colon cancer underwent complete mesocolic excision between August 2015 and July 2016 at Qingdao Central Hospital were considered for prospective inclusion. Before resection, SLN mapping was performed with injection of 99Tcm-sc and carbon nanopar-ticles, then all dissected lymph nodes were detected by pathological examination. Results A total of 660 cases of lymph nodes were found in the 40 patients (average of 16.5 cases per patient). Of them, 88 nodes (average of 2.2 cases per patient) were identified as SLN in 36 of 40 patients, with a successful detection rate of 90.0% (36/40). The diagnostic accuracy, sensitivity, and false-negative rate were 87.5% (35/40), 96.2% (25/26), and 3.8% (1/26) respectively. Conclusion 99Tcm-sc and carbon nanoparticles suspension injection for mapping SLN is a feasiblely diagnostic method for predicting local lymph node metastasis in the patient with colon cancer.

    • Clinical Experience of Large Medium-thick Skin Grafting for Granulation Wound after Facial Burns

      目的 探討面部燒傷后肉芽創面移植自體大張中厚皮的可行性。 方法 回顧性分析2002年1月-2012年4月收治的27例面部燒傷后肉芽創面進行自體大張中厚皮移植手術患者的臨床資料。術中用手術刀刮除肉芽組織至基底層,用取皮鼓或電動取皮刀取患者大腿或者腹部大張中厚皮,移植于受皮區,術后5 d左右打開敷料半暴露或暴露。 結果 27例手術患者中24例大張中厚皮全部成活,3例大部分成活,再次補植皮后愈合。患者創面愈合后外形豐滿,皮膚彈性較好,表情豐富。20例隨訪1~5年,患者面部外觀良好,2例發生小口畸形,2例眼瞼輕中度外翻,分別行整形手術后改善。 結論 面部燒傷后肉芽創面移植大張中厚皮可獲得較滿意的治療效果,可在臨床推廣使用。

    • Minimally Invasive Liver Resection:from Laparoscopic to Robotic

      Objective?To approach feasibility, safety, and the application range of pure laparoscopic resection (PLR), hand-assisted laparoscopic resection (HALR), and robotic liver resection (RLR) in the minimally invasive liver resection (MILR). Methods?The clinical data of 128 patients underwent MILR in the Surgical Department of the Shanghai Ruijin Hospital from September 2004 to January 2012 were analyzed retrospectively. According to the different methods, the patients were divided into PLR group, HALR group, and RLR group. The intraoperative findings and postoperative recovery of patients in three groups were compared.?Results?There were 82 cases in PLR group, 3 cases of which were transferred to open surgery;the mean operating time was (145.4±54.4) minutes (range:40-290 minutes);the mean blood loss was (249.3±255.7) ml (range:30-1 500 ml);abdominal infection was found in 3 cases and biliary fistula in 5 cases after operation, but all recovered after conservative treatment;the mean length of hospital stay was (7.1±3.8) days (range:2-34 days). There were 35 cases in HALR group, 3 cases of which were transferred to open surgery;the mean operating time was (182.7±59.2) minutes (range:60-300 minutes);the mean blood loss was (754.3±785.2) ml (range:50-3 000 ml);abdominal infection was found in 1 case, biliary fistula in 2 cases, and operative incision infection in 2 cases after operation, but all recovered after conservative treatment;the mean length of hospital stay was (15.4±3.7) days (range:12-30 days). There were 11 cases in RLR group, 2 cases of which were transferred to open surgery; the mean operating time was (129.5±33.5) minutes (range:120-200 minutes); the mean blood loss was (424.5±657.5) ml (range:50-5 000 ml); abdominal infection was found in 1 case and biliary fistula in 1 case after operation, but all recovered after conservative treatment; the mean length of hospital stay was (6.4±1.6) days (range:5-9 days). The operating time (P=0.001) and length of hospital stay (P=0.000) of the RLR group were shortest and the blood loss (P=0.000) of the PLR group was least among three groups. Conclusions?Minimally invasive resection is a safe and feasible. Different surgical procedures should be chosen according to different cases. The robotic liver resection provides new development for treatment of liver tumor.
