Abstract: Objective To summarize the clinical diagnostic and therapeutic experiences of infective endocarditis (IE). Methods From Jan. 2000 to Aug. 2006,60 IE patients underwent heart operation in PLA General Hospital. There were 46 male and 14 female patients, with an average age of 34.3 years old. Blood culture was positive in 25 cases (41.7%), Streptococcus was found in 12 cases, Staphylococcus in 6 cases and other bacteria in 7 cases. Ultrasonic cardiography(UCG) revealed vegetations or valve perforation in 42 cases, including 26 aortic valves, 9 mitral valves and 6 double valves. 28 cases had primary cardiac diseases,including 16 cases of congenital heart anomalies,9 cases of rheumatic heart disease and 3 cases of mitral valve prolapse. High dose of sensitive antibiotics were utilized all through the treatment in all IE patients. There were 55 selective surgeries and 5 emergent ones. Infected tissues were debrided radically,intracardiac malformation was corrected in 16 cases, valve replacement was performed in 41 cases, tricuspid plasty in 1 case. Results There were 3 patients of earlydeath. 51 patients(89.5%) were followedup for 5-71 months with norecurrence. Postoperative cardiac function (NYHA): class I was in 38 cases, class II in 13 cases. Conclusion Early diagnosis, optimal surgical timing, combined internal medicine and surgical treatment provided good therapeutic effect of IE.
【摘要】隨著超聲心動圖的普及與發展,無癥狀性類風濕性關節炎心臟瓣膜改變的檢出率逐漸增多。臨床應重視類風濕性關節炎心臟瓣膜損害以及超聲心動圖在這類疾病中的診斷價值,以期改善患者預后。【Abstract】 Objective More and more cardiac valve changes are detected in asymptomatic patients with rheumatoid arthritis by echocardiography. It is essential to pay attention to the clinical features of heart valves damage and the diagnostic value of echocardiography on that in order to improve the prognosis of patients.
Objective To investigate the prognosis and risk factors of cardiac surgical patients with preoperative renal dysfunction,and evaluate the accuracy of estimated glomerular filtration rate (eGFR) as a predictor of adverse outcomes. Methods A total of 2 151 adult patients undergoing cardiac surgery in Renji Hospital,School of Medicine of Shanghai Jiaotong University from January 2005 to December 2009 were included in this study. There were 1 267 male patientsand 884 female patients with their average age of 58.7 (18-99)years. Clinical characteristics of patients with preoperative renal dysfunction,severity of postoperative acute kidney injury (AKI)and patients’ outcomes were analyzed. Multivariate logistic regression was performed to analyze perioperative risk factors of postoperative AKI. Receiver operating characteristic(ROC) curve was used to evaluate the accuracy of eGFR to predict patients undergoing postoperative renal replacement therapy (RRT) and in-hospital death. Results A total of 221 patients (10.27%) had preoperative renal dysfunction,among whom 124 patients (56.11%) developed postoperative AKI. Patients with preoperative renal dysfunction were older,had more comorbidities including hypertension and diabetes mellitus,were more likely to receive postoperative RRT,and had worse outcomes. Patients with decreased preoperative eGFR had significantly higher in-hospital mortality. Patients with preoperative renal dysfunction who developed postoperative AKI had the worst prognosis. Multivariate logistic regression showed that hypertension (OR=4.497,P=0.003),postoperative central venous pressure (CVP) <6 cm H2O (OR=16.410,P=0.000) and postoperative CVP>14 cm H2O (OR= 5.178,P=0.013) were independent predictors of postoperative AKI for patients with preoperative renal dysfunction. The areas under the ROC curves of eGFR to predict in-hospital mortality and postoperative RRT were 0.691 and 0.704 respectively (95% CI 0.630-0.752,P=0.000;95% CI 0.614-0.795,P=0.001). Conclusion Patients with preoperative renal dysfunction are older,have more comorbidities,higher likelihood to develop postoperative AKI and worse prognosis. Hypertension,postoperative CVP<6 cm H2O and postoperative CVP>14 cm H2O are independent predictors of postoperative AKI for patients with preoperative renal dysfunction. We believe eGFR can accurately predict the risk of adverse kidney outcomes and in-hospital death of patients undergoing cardiac surgery.
Objective To explore the solutions of problems with the ROC analysis for different types data. Method Two kinds of ROC analyses of three cardiac infarction markers, cTNT, CK-MB mass and MYO, were performed with the ROC program developed by Yunnan Provincial Clinical Laboratory Center. Results The distribution of prime data had a large range, which produced a bad analysis result. After logarithmic transformation, the prime data that had smaller range now can be analyzed with full-span method. The results were similar to the ROC analyzed with the overlapped data. Conclusions We should choose different statistical method depend on the distribution of data when we performed ROC analyses.
Objective To analyze the influence of COPD on the structure and function of left ventricular. Methods Sixty-nine COPD patients ( mean age: 69. 0 ±7. 8 yrs) and forty healthy controls ( mean age: 67. 8 ±7. 6 yrs) were enrolled in this study. Both groups underwent Doppler echocardiography.Heart rate ( HR) were recorded. Left ventricular end-diastolic volume ( LVEDV) , left ventricular enddiastolic diameter ( LVEDD) , interventricular septum( IVS) , stroke volume ( SV) , and cardiac output ( CO)were measured. The changes of left ventricular were compared between the COPD patients and the healthy controls, and also between the COPD patients with or without chronic cor pulmonale. Results Compared with the healthy controls, movement range of IVS, LVEDD, LVEDV, and SV reduced significantly ( P lt;0. 05) , and HR raised significantly in the COPD patients ( P lt; 0. 05) . CO had no significant difference between two groups ( P gt;0. 05) . Sub-group analysis indicated that the thickness and movement range of IVSwere greater in the patients with cor pulmonale secondary to COPD than those without cor pulmonale ( P lt;0. 05) . Conclusions In COPD patients, left ventricular chamber size decreases, and left ventricular systolic function is impaired. Left ventricular function is impaired more severe in cor pulmonale secondary to COPD than COPD without cor pulmonale.
ObjectiveTo investigate the clinical manifestations of patients with cardiac myxoma and the factors affecting the occurrence of embolic events. Methods A retrospective study of 38 patients with cardiac myxoma diagnosed and surgically removed from January 2010 to December 2017 was performed. There were 11 males and 27 females at age of 32-75 (50.00±16.12) years. The patients were divided into a non-embolized group and an embolized group. The clinical manifestations of the patients were summarized and the factors leading to embolism were analyzed. RseultsOf the 26 patients in the non-embolized group, 22 patients (84.62%) had dyspnea, 14 patients (53.85%) had palpitations, 4 patients (15.38%) had angina pectoris, and 1 patient (3.85%) had heart failure. Of the 12 patients in the embolized group, 4 patients (33.33%) had dyspnea, 3 patients (25%) had palpitations, and 1 patient (8.33%) had angina pectoris. The mean diameter of the non-embolized group was 5.71±1.63 cm, and the maximum diameter of the tumor in the embolized group was 4.52±1.88 cm. There was no significant difference between the maximum diameter of the tumor in the embolized group and the maximum diameter of the non-embolized group (P>0.05). Atrial fibrillation occurred in 2 patients in the non-embolized group before operation. Atrial fibrillation occurred in 5 patients in the embolized group. Atrial fibrillation was more likely to occur in the embolized group (P<0.05). Conclusion Atrial fibrillation in the patients with cardiac myxoma is closely related to embolic events. The size of myxoma is not related to the occurrence of embolic events.
Objective To investigate the hospital outcomes and therapeutic strategy for multiple organ dysfunction syndrome (MODS) in children after cardiac surgery. Methods Seventy-seven consecutive pediatric patients (57 male/20 female, age 3.47±3.67 years, weight 13.08±7.52 kg) with MODS after cardiac surgery were enrolled in the study from 1999.7 to 2005.10. Corrective and palliative operation were performed in sixty-six patients and eleven patients, respectively. We evaluated the clinical score for all study patients according to the extent of organ injury. Results The overall mortality rate was 28. 6%(22/77). (1) Cardiovascular, renal, hepatic, hematologic, neurologic and respiratory dysfunction was present in 100% (77/77), 97.4% (75/77), 84.4% (65/77), 48.1%(37/77), 45. 5%(35/77) and 44. 2%(34/77) of the patients, respectively. Cardiac injury appeared much earlier than other organs (P〈0. 05). (2) Mortality rate with two, three, four, five and six dysfunctional organ systems was 0%, 12.5 %, 31.8 %, 42. 9 % and 87.5 %, respectively (r=0.487, P〈0. 001 in trend). Furthermore, there was a positive correlation between the clinic score and mortality rate (r=0.603, P〈0. 001). (3) Compared with survivors, non-survivors had longer cardiopulmonary bypass time, clamping time, higher incidence of accidental events and cardiopulmonary resuscitation during and after surgery (P〈0. 05). Conclusion Mortality associated with MODS was highly correlated with the number of organ failing and clinical score. Cardiac dysfunction was the primary disease in MODS after cardiac surgery. Therefore, therapeutic strategy for MODS should be focused on management of primary disease, as well as providing consecutive evaluation and improvement for organ function.
ObjectiveTo compare the therapeutic effect of dual-chamber pacing (DDD) and ventricular single-chamber pacing (VVI) on arrhythmia via systematic evaluation. MethodsWith the method of Cochrane system evaluation, we searched Medline, Embase, CNKI, PubMed and Wanfang database (the searching time was up to June 30, 2016) for randomized controlled trials comparing DDD with VVI treatingcardiac arrhythmias. Meta analysis was performed using RevMan5.3 software. ResultsWe collected 12 randomized controlled trials of DDD and VVI pacing treating cardiac arrhythmia including 1 704 patients, but the quality of the studies were not good. The results of Meta analysis showed that:compared with VVI pacing mode, DDD pacing mode reduced the risk of atrial fibrillation[RR=0.36, 95%CI (0.22, 0.59), P < 0.000 1]; besides, it reduced the left atrial diameter[SMD=-0.43, 95%CI (-0.68, -0.17), P=0.001], the left ventricular end diastolic dimension[SMD=-0.33, 95%CI (-0.61, -0.05), P=0.02] and increased the left ventricular ejection fraction[SMD=1.03, 95%CI (0.49, 1.57), P=0.000 2]. ConclusionsComparing DDD with VVI on the treatment of cardiac arrhythmia in patients with cardiac arrhythmia, DDD pacing can reduce the incidence of atrial fibrillation and thrombosis, enhance heart function and improve blood supply. But because of the low quality of the included studies, the curative effect cannot be confirmed, and more randomized controlled trials with high quality needs to be carried out in the future.
Objective To systematically review risk prediction models of in-hospital cardiac arrest in patients with cardiovascular disease, and to provide references for related clinical practice and scientific research for medical professionals in China. Methods Databases including CBM, CNKI, WanFang Data, PubMed, ScienceDirect, Web of Science, The Cochrane Library, Wiley Online Journals and Scopus were searched to collect studies on risk prediction models for in-hospital cardiac arrest in patients with cardiovascular disease from January 2010 to July 2022. Two researchers independently screened the literature, extracted data, and evaluated the risk of bias of the included studies. Results A total of 5 studies (4 of which were retrospective studies) were included. Study populations encompassed mainly patients with acute coronary syndrome. Two models were modeled using decision trees. The area under the receiver operating characteristic curve or C statistic of the five models ranged from 0.720 to 0.896, and only one model was verified externally and for time. The most common risk factors and immediate onset factors of in-hospital cardiac arrest in patients with cardiovascular disease included in the prediction model were age, diabetes, Killip class, and cardiac troponin. There were many problems in analysis fields, such as insufficient sample size (n=4), improper handling of variables (n=4), no methodology for dealing with missing data (n=3), and incomplete evaluation of model performance (n=5). Conclusion The prediction efficiency of risk prediction models for in-hospital cardiac arrest in patients with cardiovascular disease was good; however, the model quality could be improved. Additionally, the methodology needs to be improved in terms of data sources, selection and measurement of predictors, handling of missing data, and model evaluations. External validation of existing models is required to better guide clinical practice.
Cryptogenic stroke (CS) accounts for 25% of ischemic stroke. The etiology of undetermined stroke is unclear leading to untargeted secondary prevention, high recurrence rate, so the clinical burden of cryptogenic stroke is substantial. Cardiac magnetic resonance (CMR) imaging can identify more occult cardiac embolism that cannot be identified by standard cardiac assessment based on its excellent spatial resolution and contrast, three-dimensional imaging capacity and ability to depict soft tissues, to accelerate the initiation of optimal secondary prevention and improve the prognosis of patients. This review summarizes the application of CMR in the field of CS in recent years. Based on the latest evidence of diagnosis and management strategies, this paper proposes a cardiac diagnostic examination plan for CS patients, thereby improving the secondary prevention strategy of CS patients and improving their quality of life.
