    • Expression of Stromal Cell-Derived Factor-1 and Its Relation with Prognosis in Human Breast Cancer

      【Abstract】Objective Stromal cell-derived factor-1(SDF-1, CXCL12) is a member of the CXC subfamily of chemokines which, through its cognate receptor (CXCR4), plays an important role in tumor invasion and metastasis. This study analyzed quantitatively the expression of SDF-1 and its relation with clinicopathologic feature and clinical outcome in human breast cancer.Methods Expression of SDF-1 mRNA in 8 breast cancer cell lines, an endothelial cell line HECV and a fibroblast cell MRC5 was studied by using RT-PCR. In addition, the expression of SDF-1 was investigated at both protein (immunohistochemistry) and mRNA(real-time PCR) levels in a group of human normal mammary(n=32) and tumour tissues(n=120). Results SDF-1 expression was identified in MRC5, MDA-MB435s, MDA-MB436, MCF7 cell lines, breast tumour and normal tissues. Significantly higher level of SDF-1 was seen in lymph node positive than in lymph node negative tumours (399.00±210.00 vs 0.89±0.47), P=0.048. The level of SDF-1 expression in patients who developed local recurrence or metastasis, or patients who died of breast cancer was higher than in patients who were disease free as well, (670.00±346.00 vs 0.83±0.35), P=0.01. It was most notable that level of SDF-1 was significantly correlated with over survival (P=0.01) and incidence free survival (P=0.035, by Cox proportion analysis).Conclusion SDF-1 is a factor that is expressed in both stromal cells and some breast cancer cells. Its level are correlated with lymph node involvement, prognosis and survival in patients with breast cancer. SDF-1 may therefore have a potential prognostic value in breast cancer.

    • Application of Contrast Enhanced Ultrasound in Sentinel Lymph Node Biopsy of Breast Cancer

      ObjectiveTo evaluate the diagnostic value of contrast enhanced ultrasound (CEUS) to the sentinel lymph node (SLN) of breast cancer. MethodsSeventy-two operable breast cancer patients with clinically negative axillary lymph node were enrolled.Sulphur hexafluoride microbubbles for injection (SonoVue) was used alone as the tracer agent for the sentinel lymph node biopsy (SLNB), and axillary dissection was performed after the methylene blue location.All SLNs were examined pathologically with HE staining.The SLN diagnosis result of contrast enhanced ultrasound and postoperative pathological examination result were comparative analyzed. ResultsAfter the injection of SonoVue can obtain a clear image of the lymphatic vessels and SLN.The success rate of CEUS imaging was 84.72% (61/72) in this group of 72 patients, and the false negative rate was 12.12% (4/33).The sensitivity and specificity of diagnosis by CEUS was 92.50% (37/40) and 92.59% (50/54), respectively, the diagnostic odds ratio (DOR) was 154.17.By the pathology results as the gold standard, the internal consistency of these two methods was good (Kappa value=0.848, P < 0.01). ConclusionCEUS may be a useful orientation and determination method for SLNs.

    • Long-Term Effects of Partial Axillary Dissection in Modified Radical Mastectomy of Breast Cancer

      Objective To study the effects of partial axillary lymph node dissection (PALD) on prognosis and upper limb function in patients with breast cancer. Methods Ninety-eight breast cancer patients with stage Ⅰ and Ⅱ were randomly divided into two groups and different surgical procedures following modified mastectomy were performed: partial axillary lymph node (level Ⅰ and Ⅱ) dissection (PALD) group (n=48) and total axillary lymph node (levelⅠ, Ⅱ and Ⅲ) dissection (TALD) group (n=50). The longterm positive relapse rate and upper limb function between 2 groups were compared. Results During the follow-up of 5 to 10 years (average 4.5 years), there were 2 cases (4.2%) of local recurrence on chest wall and one case (2.1%) of recurrence in axillary lymph node and one case (2.1%) of recurrence in supraclavicular lymph node in PALD group, and 2 cases (4.0%) of local recurrence on chest wall and no axillary lymph node recurrence and one case (2.0%) of recurrence in supraclavicular lymph node happened in TALD group. There was no statistical difference between PALD group and TALD group (Pgt;0.05). The incidence of upper limb edema and dysfunction was 4.2% (2/48) in PALD group and 16.0%(8/50) in TALD group (P<0.01). There was no significant statistical difference of 5year and 10year survival rate between PALD group and TALD group (89.6% vs. 88.0%, 79.2% vs. 78.0%,Pgt;0.05). Conclusion PALD may reduce upper limb dysfunction after operation in patients with stage Ⅰ and Ⅱ breast cancer, and does not increase prognostic risk.

    • Relationship between Birth Number and Breast Cancer Risk of Chinese Female: A Meta-analysis

      ObjectiveTo systematically review the correlation between the birth number and the risk of breast cancer of Chinese female. MethodsWe electronically searched databases including the CNKI, WanFang Data and VIP databases from inception to September 1st 2015, to collect case-control studies about the correlation between the number of births and the risk of breast cancer among Chinese female. Two reviewers independently screened literature, extracted data, and evaluated the risk of bias of included studies. Then meta-analysis was performed by using Stata software. ResultsA total of 14 case-control studies involving 3 428 patients and 3 906 controls were included. The results of meta-analysis showed that:the females who had term birth had significant lower incidence of breast cancer than those without childbirth history (OR=0.429, 95%CI 0.322 to 0.571). Subgroup analysis based on the number of term birth showed that:Compared with the female without childbirth history, those who had term birth of one time (OR=0.464, 95%CI 0.321 to 0.670), two times (OR=0.394, 95%CI 0.269 to 0.576) and≥3 times (OR=0.340, 95%CI 0.232 to 0.499) had significant lower incidence of breast cancer. ConclusionTerm birth is a protective factor for breast cancer of Chinese female, and more times of term birth will decrease the risk of breast cancer.

    • A STUDY ON THE GENOMIC VARIANT IN MATCHED ADENOCINOMA AND NON-TUMOR GASTRIC TISSUE BY ARBITRARILY PRIMER POLYMERASE CHAIN REACTION

      Objective To identify and isolate the variant gene associated with gastric adenocarcinoma and clone the fragment of variant gene.Methods By arbitrarily primer polymerase chain reaction (AP-PCR), DNA samples from 5 matched gastric adenocarcinoma and non-tumor gastric tissues were analysed. Results The produced AP-PCR profiles were different in each matched gastric adenocarcinoma and non-tumor gastric tissue. One differentiated amplified DNA fragments PW2.2 from a matched gastric adenocarcinoma were cloned. The result of Southern blot hybridization with PW2.2 as a probe showing that this fragment was also found in some other gastric adenocarcinoma samples. Conclusion AP-PCR fingerprinting assay can be used to identify and clone the variant genes associated with gastric adenocarcinoma.

    • Analysis of Recurrence and Metastasis Factors after Modified Radical Mastectomy

      目的 探討乳腺癌改良根治術后影響局部復發與遠處轉移的相關因素。方法 對陜西省漢中市鐵路中心醫院略陽醫院2000年3月至2007年3月期間行乳腺癌改良根治術且資料完整的637例原發性乳腺癌患者的臨床資料進行回顧性分析。結果 66例(10.36%)患者術后發生局部復發和遠處轉移,其中局部復發50例(7.85%),遠處轉移16例(2.51%);術后3~4年內發生復發與遠處轉移者47例(71.21%)。術后復發和遠處轉移與患者年齡、腫瘤大小、腋窩淋巴結轉移情況、TNM分期、激素受體(ER/PR)表達情況及術后是否接受正規的輔助治療有關,與患者有無家族史無關。結論 腫瘤大小、腋窩淋巴結轉移情況、TNM分期、ER/PR表達情況及術后是否接受正規的輔助治療是影響乳腺癌改良根治術后復發與遠處轉移的危險因素,重視乳腺癌術后隨訪,規范乳腺癌的手術方式,強化乳腺癌的綜合治療是降低乳腺癌改良根治術后復發與遠處轉移率的主要措施。

    • Expression and clinical significance of H2AFX gene in lung adenocarcinoma

      Objective To analyze the expression of H2A histone family, member X (H2AFX) gene in lung adenocarcinoma and its influence on prognosis. Methods We analyzed the expression level of H2AFX gene in the tumor tissues (497 cases) and normal adjacent tissues (54 cases) of lung adenocarcinoma patients via The Cancer Genome Atlas. The patients were divided into high expression group and low expression group according to the expression level of H2AFX gene in lung adenocarcinoma samples. The relationship between H2AFX and clinicopathological features of patients was analyzed through logistic regression. Kaplan-Meier survival curve and log-rank test were used to study the correlation between H2AFX expression and the prognosis of lung adenocarcinoma patients. Univariate and multiple Cox regression analyses were performed to determine the prognostic significance of H2AFX expression in lung adenocarcinoma patients. The research also covered H2AFX-related pathways of genes in the development of lung adenocarcinoma with gene set enrichment analysis (GSEA). Results The H2AFX expression was higher in lung adenocarcinoma tissues than that in normal adjacent tissues (P<0.001). Besides, it was significantly correlated with age (P<0.001), T staging (P=0.007), and N staging (P=0.010), but had little to do with M staging or gender (P>0.05). Kaplan-Meier survival curve and log-rank test showed that the survival rate of patients with high H2AFX expression was vastly lower than that of patients with low H2AFX expression (P<0.001). Multiple Cox regression analysis demonstrated that H2AFX could be an independent prognostic factor for lung adenocarcinoma [hazard ratio=1.41, 95% confidence interval (1.11, 1.78), P=0.004]. The results of GSEA displayed that H2AFX was involved in cell cycle, homologous recombination, DNA replication, base excision and repair, spliceosome, mismatch repair, p53 signaling pathway, nucleotide excision and repair, RNA degradation, RNA polymerase, and other pathways. Conclusions The expression of H2AFX gene is high in lung adenocarcinoma, and closely connected to the prognosis, occurrence, and evolution of lung adenocarcinoma. This gene can be one of the new molecular markers and therapeutic targets for lung adenocarcinoma.

    • THE SIGNIFICANCE OF BCL-2 PROTEIN EXPRESSION IN HUMAN BREAST CARCINOMA

      To evaluate the significance of bcl-2 protein, estrogen receptor (ER), progesterone receptor (PR) and p53 oncogene in invasive breast cancer of ductal and lobular type. Tumor tissues were examined immunohistochemically in paraffin embedded tissues from 125 patients. Results: The invasive ductal breast carcinomas expressed bcl-2 protein significantly less frequently than the lobular type (P<0.001). And the expression of bcl-2 protein was significantly correlated with ER, PR (P<0.001) and p53 (P<0.001), also correlated with primary tumor size and grade. No statistical evidence was found to indicate the relationship between bcl-2 protein expression and anxillary lymph node metastasis. Conclusions: The expression of bcl-2 protein may be regarded as a biological index and may play important role in evaluating the biological characteristics of breast cancer.
