    • Influence of Water Content on the Biotribological Behavior of Bone Tissue

      In the present study, swine thighbone samples with different water contents were prepared through process of dehydration. The influence of water content on the biotribological behavior of the bone tissue was studied with nanoindentation test, reciprocating sliding test and impact wear test. It is shown that the water contents had significant influence on the mechanical properties and biotribological behavior of the swine thighbone samples. In this experiment condition, with decreasing water content, the hardness and elastic modulus of the swine thighbone samples increased, while the tribological properties significantly decreased. The viscoelasticity of bone tissue decreased with decreasing water content, which mainly resulted in different wear mechanisms of the bone tissue samples under applied loading.

    • A clinical study on the relationship of the tail femur distance and the lag screw migration or cutting-out after the third generation of Gamma nail fixation of intertrochanteric fracture

      Objective To confirm the association between tail femur distance (TFD) and lag screw migration or cutting-out in the treatment of intertrochanteric fracture with the third generation of Gamma nail (TGN). Methods The clinical data of 124 cases of intertrochanteric fracture treated with TGN internal fixation and followed up more than 18 months between January 2012 and December 2015 were reviewed and analyzed. There were 52 males and 72 females, with an age of 46-93 years (mean, 78.5 years). According to AO/Association for the Study of Internal Fixation (AO/ASIF) classification, 43 cases were type 31-A1, 69 cases were type 31-A2, and 12 cases were type 31-A3. The time from injury to operation was 1-10 days (mean, 2.9 days). According to the fracture healing of the patients, the patients were divided into the healing group and failure group. The age, gender, height, bone mineral density (BMD), fracture AO/ASIF classification, the time from injury to operation, and the TFD value at 1 day after operation were recorded and compared. The risk factors for the migration or cutting-out of lag screw were analyzed by logistic regression. Results There were 111 cases in healing group, the healing time was 80-110 days (mean, 95.5 days). There were 13 cases in failure group, including 2 cases of lag screw cutting-out and 11 cases of significant migration. Except for the TFD value at 1 day after operation in failure group was significantly higher than that in the healing group(t=5.14, P=0.00), there was no significant difference in gender, age, height, BMD, fracture of AO/ASIF classification, and the time from injury to operation (P>0.05) between 2 groups. logistic regression analysis showed that TFD value was a risk factor for the migration or cutting-out of lag screw (B=1.22, standardized coefficient=0.32, Wald χ2=14.66, P=0.00, OR=3.37). Conclusion The patients with higher TFD value had higher risk of postoperative lag screw migration or cutting-out. This result indicates that the appropriate length of the lag screw is helpful to reduce TFD value and prevent postoperative lag screw migration or cutting-out.

    • 股骨干骨折合并同側股骨頸骨折八例

      為了總結股骨干骨折合并同側股骨頸骨折的經驗教訓,報道8例患者的診治經過,并提出在高能量損傷時應注意的問題。本組均為高能量創傷所致。股骨頸骨折為基底部無移位性;股骨干骨折為粉碎性,7例位于中段,1例位于上段。一次手術同時切開復位內固定治療7例,骨牽引治療1例。平均隨訪3年,手術復位除1例股骨干骨折未愈合,再次手術外,7例均滿意愈合,功能良好。認為對有股骨干骨折的患者,均應提高警惕,注意有無股骨頸合并損傷。一旦確診,應盡早手術治療,手術方式以兩處骨折分別切開復位內固定為宜。

    • PRELIMINARY EXPERIENCES IN MINIMALLY INVASIVE AND MINIINCISION SURGERY TOTAL HIP ARTHROPLASTY FOR LATE OSTEONECROSIS OF THE FEMORAL HEAD

      Objective To explore the effect of minimally invasive and mini-incision surgery (MIS) in total hip arthroplasty (THA) on late osteonecrosis of femoral head (ONFH). Methods From March 2003, Eighteen patients (22 hips) with ONFH underwent MIS in THA. Their ages ranged from 24to 57 years, including 13 males and 5 females. The mean body mass index ranged from 17.1 to 30.1(24.6 on average). The Harris hip score was 46 points before operation. Modified posterior-lateral approach was adopted, and the MIS THA was performed by cementless prosthesis. As a comparison, 18 patients (22 hips) were performed by conventional THA at the same period. The data, including bleeding volume during operation, incision length, operative time, and postoperative function recovery, were compared. Results Follow-ups were done for 6 to 20 months (11 months on average). Dislocation occurred in one patient that underwent conventional THA 2 days after operation. No complication occurred in MIS THA group. The incision lengths ranged from 8.7 to 10.5 cm (9.3 cm on average) in MIS THA group, being statistically different (Plt;0.01). There was no significant difference in Harris scoring of the function between the two groups both before the operation and after the operation (Pgt;0.05). The operative time was almost the same, but the bleeding volume in MIS THA group was less (Plt;0.05). The function recovery was faster in MIS THA group.Conclusion The MIS THA is an alternative to the treatment of late ONFH. The advantages of MIS THA are fewer trauma, less bleeding volume, and faster recovery. The MIS THA should be performed by surgeons with rich experiences in THA and hospitals with necessary instruments. 

    • 從醫源性角度探討股骨鎖定鋼板斷裂的原因及防治措施

      目的 探討股骨鎖定鋼板斷裂的醫源性因素及防治措施。 方法 回顧分析2007 年5 月- 2009 年8 月收治的11 例股骨骨折內固定術后鎖定鋼板斷裂患者臨床資料。其中男7 例,女4 例;年齡22 ~ 65 歲,平均38 歲。原骨折位于股骨干5 例,股骨遠端6 例。切開復位鎖定鋼板內固定術后2 ~ 6 個月鋼板斷裂;斷裂原因:適應證選擇不當,違反鎖定鋼板操作原則,術后功能鍛煉及康復訓練不到位,醫患交流缺失。11 例均再次手術,其中股骨干骨折5 例、股骨遠端骨折1 例行髓內釘固定,余5 例股骨遠端骨折再次行鎖定鋼板固定。 結果 二次術后11 例均獲隨訪,隨訪時間8 ~ 20 個月,平均12 個月。術后4 ~ 8 個月,平均6 個月,所有患者骨折均達骨性愈合。無鋼板再斷裂及其他相關并發癥發生。術后8 個月根據美國特種外科醫院(HSS)評分系統對膝關節功能評定:優7 例,良3 例,可1 例;優良率90.9%。 結論 合理選擇適應證、規范手術操作、正規康復訓練、加強醫患交流是防治股骨骨折內固定術后鎖定鋼板斷裂的有效措施。

    • TRANSPOSITION OF MUSCULAR SKELETAL FLAP PEDICLED WITH STRAIGHT HEAD OF RECTUS FEMORIS FOR TREATMENT OF AVASCULAR NECROSIS OF FEMORAL HEAD

      OBJECTIVE: To investigate the clinical results of transposition of muscular skeletal flap pedicled with straight head of rectus femoris for treatment of avascular necrosis of adult femoral head. METHODS: Eight patients with avascular necrosis of femoral head were adopted in this study. There were 6 males and 2 females, the ages were ranged from 24 to 56 years. According to the criteria of Ficat, there were 5 cases in stage II and 3 cases in stage III. The Smith-Peterson incision was used to expose the capsule of the hip. After complete curettage of the necrotic bone from the femoral head, the muscular skeletal flap pedicled with straight head of rectus femoris was resected and transposited into femoral head. Finally, conventional decompression of head was performed. RESULTS: All the cases were followed up for 1 to 3 years. There were excellent results in 5 cases, good in 2 cases and moderate in 1 case. The rate of excellent and good results were 87.5%. CONCLUSION: Comparing with other pedicled bony flaps, the muscular skeletal flap pedicled with straight head of rectus femoris is characterized by its convenience and efficacy. It is suitable for the treatment of avascular necrosis of femoral head in stage II or III, but the contour of the femoral head should be nearly normal.

    • STUDY ON RELATIONSHIP BETWEEN OSTEOPOROSIS AND mRNA EXPRESSIONS OF VASCULAR ENDOTHELIAL GROWTH FACTOR AND BONE MORPHOGENETIC PROTEIN 2 IN NONTRAUMATIC AVASCULAR NECROSIS OF FEMORAL HEAD

      Objective To explore the significance and the relationshi p between osteoporosis and the mRNA expressions of vascular endothel ial growth factor (VEGF) and bone morphogenetic protein 2 (BMP-2) in nontraumatic avascular necrosis of the femoral head (NONFH), so as to provide a theoretical basis for the pathogenesis and the cl inical treatment of NONFH. Methods Sixty-nine specimens of femoral head were collected from voluntary donators undergoing total hi p arthroplasty, including 37 cases of NONFH (NONFH group) and 32 cases of fresh femoral neck fracture (control group). In NONFH group, there were 26 males and 11 females with an average age of 57.3 years (range, 43-75 years), including 19 cases of steroid-induced avascular necrosis of the femoral head (ANFH), 16 cases of alcohol ic ANFH, and 2 cases of idiopathicANFH; according to Ficat staging system, there were 23 cases at stage III and 14 cases at stage IV. In control group, there were 23 males and 9 females with an average age of 58.6 years (range, 46-79 years). The NO level of serum, the Q value of femur, and the bone mineral density (BMD) of weight-bearing area were measured firstly. The bone tissues were harvested from weightbearing necrosis area and healthy area. The pathological change was observed by HE staining, the percentage of empty bone lacuna and the percentage of trabecular bone area were calculated. The mRNA expressions of VEGF and BMP-2 in femoral head were detected through in situ hybridization technique. Results There were significant differences (P lt; 0.05) in the NO level of serum, the Q value of femur, and the BMD between NONFH group and control group. In NONFH group, the femoral head showed irregular shape, the articular cartilage exfol iated and collapsed. In weight-bearing necrosis area, the bone trabeculae were sparse and non-intact with a great number of empty lacuna; necrotic bone trabeculae were decomposed and absorbed; no obvious bone regeneration and repair were observed. In weight-bearing healthy area, the fat cells in bone marrow showed prol iferation and hypertrophy. In control group, the femoral head had normal appearance, intact articular cartilage, and intact bone trabeculae with a regular arrange, and osteocytes were clearly seen. There were significant differences in the percentage of empty bone lacuna and the percentage of trabecular bone area between NONFH group and control group (P lt; 0.05). The mRNA expressions of VEGF and BMP-2 were positive in 2 groups. The positive area ratio, the absorbance value, and integral absorbancevalue of VEGF mRNA and BMP-2 mRNA in NONFH group were significantly lower than those in control group (P lt; 0.05);the grey scales of VEGF mRNA and BMP-2 mRNA in NONFH group were significantly higher than that in control group (P lt;0.05). Conclusion The pathological stage of osteoporosis may play an important role in the mechanism of the NONFH. The decrease of mRNA expressions of VEGF and BMP-2 in femoral head of NONFH is important reason that affect its bone mass, osteoporosis, rehabil itation, and reconstruction. It may be benefit to the reparative process of the necrosis femoral head to increase the mRNA expressions of VEGF and BMP-2 in the femoral head.

    • Effects of Clinical Nursing Pathway in Femoral Neck Fracture Patients: A Meta-analysis

      ObjectiveTo systematically review the effetcs of clinical nursing pathway (CNP) in patients with femoral neck fracture. MethodsDatabases including PubMed, The Cochrane Library (Issue 9, 2014), CNKI, VIP and WanFang Data were electronically searched to collect randomized controlled trials (RCTs) about CNP in management of patients with femoral neck fracture from inception to September 2014. Two reviewers independently screened literature, extracted data and assessed the risk of bias of included studies. Then, meta-analysis was performed using RevMan 5.1 software. ResultsA total of 24 RCTs involving 1 852 patients were included. The results of meta-analysis showed that: compared with the control group, the time of hospitalization was decreased (MD=-4.47, 95%CI -5.32 to -3.62, P<0.000 01), the patients' nursing satisfaction was increased (OR=5.52, 95%CI 3.75 to 8.14, P<0.000 01), the achievement rate of knowledge was increased (OR=5.97, 95%CI 3.57 to 9.96, P<0.000 01) and the incidence of postoperative complications was decreased (OR=0.34, 95%CI 0.24 to 0.48, P<0.000 01) in the CNP group. ConclusionCNP can reduce the time of hospitalization, improve patients' nursing satisfaction, improve the achievement rate of knowledge, as well as reduce the incidence of postoperative complications. Due to limited quality and quantity of the included studies, the above conclusion need more high quality studies to verify.
