OBJECTIVE: To compare the effect of several types of rib rings with intercostal muscles for the replacement of trachea in thorax. METHODS: The surface layer of the third rib of dogs were ripped off and curved into triangular, quadrilateral and polygonal form. These three types of rib rings with intercostal muscles were used to replace a segment of trachea in thorax. RESULTS: The stability of triangular rib ring was very well, but stricture of ring were often happened because of its smaller internal diameter. These stability of quadrilateral rib ring was the worst. The polygonal rib ring presented the biggest diameter and good stability compared to the other two kinds of rings. If silicone tube was supplemented in the polygonal rib ring, the quality of artificial trachea was excellent. CONCLUSION: The rib rings with intercostal muscles are successfully used for replacing the defect of trachea in canine thorax. The polygonal rib rings have the best quality in the three types of rib ring for tracheal replacement.
Objective To investigate the effectiveness of groin flap combined with medial plantar artery perforator flap (MPAP) for degree Ⅲ-Ⅳ defects of multiple fingers. Methods Between January 2018 and June 2019, 12 patients with degree Ⅲ-Ⅳ defects of multiple fingers caused by crushing were admitted. There were 9 males and 3 females with a median age of 29 years (range, 16-42 years). The mean interval between the injury and admission was 3 hours (range, 1-9 hours). The injured fingers of 7 cases were index and middle fingers, 4 cases were middle and ring fingers, and 1 case was index, middle, and ring fingers. All fingers were taken thorough debridement and covered by the vacuum sealing drainage device during the emergency operation. The mean interval between the debridement and flap repairing was 18 hours (range, 12-36 hours). During the first-stage operation, the iliac bone graft was used to reconstruct bone frame, and the proximal interphalangeal (PIP) joint from the foot was transferred as the digital PIP joint, then the thin groin flap and MPAP were tailored to cover the dorsal and palmar defects, respectively. The size of the groin flap was 7.0 cm×4.5 cm-14.0 cm×9.0 cm, and the size of the MPAP was 8.0 cm×4.5 cm-14.0 cm×6.5 cm. The abdominal donor site was directly sutured, and the foot was repaired with full-thickness skin grafting. The flaps were separated into the finger shape at the second-stage. Results All the flaps survived, and the wounds healed by first intention; the incisions in the donor site healed by first intention, and the skin grafts survived completely. All patients were followed up 12-18 months (mean, 16 months). At last follow-up, the injured finger was similar to the contralateral one in terms of texture, appearance, and color. The mean two-point discrimination was 8 mm (range, 6-10 mm), and the sensate level recovered to the S3-S4. According to the Michigan Hand Outcomes Questionnaire (MHQ), the reconstructed hand function was excellent in 8 cases and good in 4 cases. There was no complication in the donor sites. Conclusion The degree Ⅲ-Ⅳ defects of multiple fingers were repaired by the groin flap and MPAP, and the reconstructed fingers can perform good texture and motion with being sensate, with less sacrifice on the foot.
ObjectiveTo review the advances in methods for reconstructing nipple projection based on tissue graft support. MethodsThe literature related to nipple projection reconstruction based on tissue graft support was reviewed and summarized in terms of the advantages and disadvantages of various tissue grafts and the improved nipple projection results. Results Loss of nipple projection is a common cause of decreased patient’s satisfaction. Reconstructing nipple projection based on tissue graft support is a more common clinical method and can be done with autologous and allogeneic tissues. Autologous tissue grafts include dermis, adipose tissue with dermis, adipose tissue, ear cartilage, rib cartilage, and contralateral nipple tissue. Autologous tissue grafts are easy to obtain and have no immune rejection, but may lead to donor area damage and prolong the surgical time for tissue collection. Allogeneic tissue grafts include acellular dermal matrix, lyophilized rib cartilage, and extracellular matrix collagen, and decellularized nipple tissue. Allogeneic tissue grafts do not cause additional donor area damage, are highly malleable, and can be designed to be utilized according to the recipient area, but the high cost often limits the development of this technique. Conclusion There is no gold standard regarding tissue graft-assisted nipple projection reconstruction techniques, and there are advantages and disadvantages to both autologous and allogeneic tissue grafts. Surgeons should choose the appropriate graft based on the actual condition of the patient.
OBJECTIVE: To investigate the effect of breast reconstruction with latissimus dorsi musculocutaneous flap. METHODS: Since 1994, 60 cases were performed breast reconstruction with latissimus dorsi musculocutaneous flap with fat tissue nourished by thoracodorsal artery according to the shape and volume of the normal breast on the other side. All of cases were followed up for 3 months to 5 years. RESULTS: Among the 60 cases, excellent effect was obtained in 41 cases (68.3%), good effect in 16 cases (26.7%), unsatisfactory in 3 cases (5.0%). CONCLUSION: Modified latissimus dorsi musculocutaneous flap to reconstruct breast overcome the shortcoming of volume deficiency of traditional latissimus dorsi in breast reconstruction, and it is a safe and easy-manipulated surgical operation.
Objective To evaluate a new reconstruction method for grades Ⅴ and Ⅵ defect with flap by skin soft tissue expansion technique. Methods From May 1998to September 2003, 8 cases of serious thumb defect were treated, including 6 males and 2 females, aging 18-27 years. The defect was caused by crush injury ofmachine in 6 cases and hot crush injury in 2 cases( 5 cases of grade Ⅴ and 3 cases of grade Ⅵ). The expander was placed under the tenor skin and softtissue.And then normal saline was infused to expand the skin and soft tissue graduallytill it was available for thumb reconstruction. Iliac autograft was fixed to residual thumb stump and covered with flap produce by expanded skin and soft-tissue.Postoperative rehabilitation was carried out. Results Allreconstructed thumbs were alive. After3-24 months follow-up, all reconstructed thumbs were with good sensation, appearance and durable. Twopoint discrimination was less than 5 mm. The functions of opposition, extend, abduction and endoduction were better in grade Ⅴ thumb defect than in grade Ⅵ thumb defect. Bone union was achieved within 3 to 4 months. Conclusion It is a convenient-to-operate and reliablemethod for thumb reconstruction. It is an alternative new reconstruction methodfor grades Ⅴ and Ⅵ thumb defect.
The deltopectoral flaps were applied in 5 cases for whole nasal reconstruction. The follow-up period has lasted from 1 to 3 years and the results are satisfactory. This flap for whole nasal reconstuction provides abundant repair material. The operative procedure is simple, easy and the donor area is hide. The color and quality of the deltopectoral flap are good postoperation.
Objective To evaluate a new alternative method for thereconstruction of vagina with deep inferior epigastric perforator(DIEP) flap. Methods From January 2004 to May 2005, DIEP flaps were used for vaginal reconstruction in 5 patients(19 to 40 years), including 4 cases of congenital vaginal agenesis and 1 case of vaginal tumor. Before operation, the perforators were detected by theDoppler and the flaps based on the perforators ranged from 10 cm×9 cm to 12 cm×11 cm. DIEP flaps were elevated and then transferred to reconstruct the vagina. Results Deep inferior epigastric perforator flaps were used in 5 patients. Only 1 patient developed haematoma in the posterior aspect of thereconstructed vagina, but the flap was viable. The wounds healed secondarily after conservative therapy. All the flaps survived completely. No complication occurred at donor site of abdominal wall. Conclusion Despite technical difficulties in elevatingthe deep inferior epigastric perforator flap, the flap is a good choice for vaginal reconstruction.
Fifty-four cases of tongue with carcinoma were re-constructed by forearm skin flap(in 17 cases),forchead flap(in 15 cases) ,pectoral skin flap (in 14 cases ), stenocleido-mastoid flap (in 1 case), tongue flap(in 6 cases) and buccalmucoidal flap (in 1 case) , Fifty-two case were sucecssfulwith 2 failure. Partial function was recovered after opera-tion. The author denoted that a better result were obtained byforehead flap,forearm skin flap,and pectoral skin flap in re-construction of the tongue.
