嬰兒痙攣（Infantile spasms，IS）是一種獨特的，并有年齡依賴性的嬰兒早期癲癇性腦病。具有發病年齡早、發作形式特殊、進行性認知損害、腦電圖呈高峰失律等特征。其病因復雜、多樣，發病機制尚未明確，治療方面仍存在困難，大多遺留智能缺陷等后遺癥。癲癇的發生與神經組織微環境中增加的強烈而持續的炎癥狀態相關，受損神經元組織中炎癥細胞和分子的激活、分解調節障礙是癲癇發展的關鍵因素，炎癥可能起源于中樞神經系統，或通過血腦屏障的破壞從全身循環獲得。同時癲癇也可能激活促炎通路，導致神經炎癥的發生。本文對近年神經炎癥通路在 IS 發病機制中的作用研究進行綜述，通過總結遺傳學進展揭示了許多參與 IS 發病機制的基因，包括直接或間接參與炎癥的基因，同時得到臨床和 IS 動物模型的研究支持。了解 IS 發生發展過程中炎癥的神經生物學將有助于開發新的生物標志物，以便更好地篩選高危患者，為探索 IS 治療新靶點提供方向。
