從國內外研究現狀出發, 對癲癇持續狀態(Status epilepticus, SE)的定義、流行病學和病因、發病機制、分類、診斷評估、治療及預后等進行了全面闡述。有關SE定義的歷史變遷, 從理論性定義到實用性定義均給出了明確界定, 其中實用性定義為適應人們對SE病理生理機制的進一步理解和滿足臨床容易操作需要不斷更新。國際抗癲癇聯盟(ILAE)已對強直-陣攣SE、復雜部分發作SE和失神SE實用性定義給出了明確規定。并對SE的流行病學及病因進行了探討, 多種神經系統及全身性病理生理改變可導致SE, 發作自我維持與自我終止的平衡破壞是SE發生的必要條件, 發作導致神經系統興奮性和抑制性系統失衡, 后者反過來又促進發作持續。有關SE分類一直備受關注, 幾經變遷, 最新方案重新按驚厥性和非驚厥性分為兩大類。診斷需依靠腦電圖監測, 尤其非驚厥類型, 典型癲癇發作且持續時間足夠長, 可無需腦電圖監測證據。神經影像及實驗室檢查有助于發現病因。SE為神經科急癥, 盡早控制發作, 并應與詳細的病史采集同時進行, 院前積極給藥明顯增加SE控制率, 咪達唑侖和苯二氮卓類為首選藥物。對難治性SE及早應用麻醉藥物, 可減少并發癥及死亡率, 提高控制率
