目的探討富于細胞性血管纖維瘤（cellular angiofibroma，CAF）的影像學表現、病理特征、臨床診斷、鑒別診斷以及治療和預后。方法報道1例腹股溝區CAF的臨床表現、病理特征及治療情況，并結合國內外相關文獻報道的142例病例資料進行分析。結果本例65歲男性患者，因發現左側腹股溝區腫塊5年且逐漸長大而入院行手術完整切除左側腹股溝區腫物，腫物約7 cm×3 cm×5 cm大，質地較韌，邊界清晰，術后病理學檢查見腫物由大量短梭形細胞和小到中等大的血管組成，邊界清晰，可見纖維性包膜。免疫組織化學示雌激素受體（部分 +）、平滑肌肌動蛋白（血管 +）、CD34（+）及CD117（+），Sox-10、S-100、Dog-1、結蛋白、P16、CDK4、mdm2、上皮膜抗原、孕激素受體、視網膜母細胞腫瘤抑制蛋白及神經營養因子絡氨酸激酶均為（–）。最終診斷為CAF，術后隨訪3個月腫瘤無復發。復習國內外文獻報道的142例CAF患者資料，腫瘤主要位于外陰（74例）和陰囊-腹股溝區（47例），21例位于其他部位，均接受手術治療。CAF通常表現為觸及無痛性腫塊，腫瘤大體呈圓形、卵圓形或分葉狀，鏡下可見瘤體由大量梭形或短梭形細胞及豐富中小血管組成，間質膠原纖維化并伴有炎性細胞浸潤，免疫組織化學特征表現為波形蛋白、CD34及平滑肌肌動蛋白陽性，部分表達雌激素受體和孕激素受體，結蛋白及S-100呈陰性。治療以完整切除腫瘤為主，預后良好。結論 CAF為好發于兩性生殖道遠端的良性間葉性腫瘤，治療以手術完整切除腫瘤為主，預后良好，術后需嚴密隨訪。
