目的探討全腹腔鏡下肝切除的可行性、安全性及手術技巧。方法回顧性分析2010年7月至2013年3月期間我院行全腹腔鏡下肝切除20例病例（腹腔鏡組），其中肝海綿狀血管瘤10例，原發性肝細胞癌6例，肝內膽管結石4例。與同期25例開腹肝切除手術（開腹組）進行比較。結果 2組患者一般臨床資料比較差異無統計學意義（P＞0.05），具有可比性。腹腔鏡組術后有1例出現膽汁漏（＜100 mL/d），經通暢引流7 d后停止，未發現腹腔積液后拔除引流管，順利恢復；術后1～2 d逐步恢復飲食，5～7 d出院，原發性肝細胞癌患者病理檢查切緣未發現殘余病灶。腹腔鏡組術后切口疼痛持續時間明顯短于開腹組〔（1.67±0.52）d比（3.39±0.63）d，t=2.266，P=0.035〕。無一例切口出現感染及脂肪液化。腹腔鏡組和開腹組的手術時間比較差異無統計學意義〔（110.29±25.11）min比（100.95±24.32）min，t=0.731，P=0.415〕，腹腔鏡組的術中出血量明顯少于開腹組〔（345.11±45.21）mL比（506.72±58.32）mL，t=25.10，P＜0.001〕，住院時間也明顯短于開腹組〔（3.57±1.52）d比（6.39±3.63）d，w=15.00，P=0.005〕。結論全腹腔鏡下肝切除（特別是肝臟邊緣及左外葉病灶）是安全、可行的，較開腹手術其具有微創、住院時間短、術中出血量少、手術并發癥少等優點。
