目的分析感染性心內膜炎的誤診原因。方法回顧性分析我科 2008 年 1 月至 2018 年 1 月確診的 84 例感染性心內膜炎（infective endocarditis，IE）患者的臨床資料，其中男 62 例（73.8%）、女 22 例（26.2%），中位年齡 39.5（11～77）歲。當患者出現符合 IE 的疑診條件時，根據醫師是否疑診為 IE，將患者分為未誤診組（22 例）和誤診組（62 例）。比較兩組患者的臨床資料。結果全組患者體溫>38℃ 占比 97.6%（82/84），心臟雜音 82.1%（69/84），脾腫大 27.4%（23/84），心力衰竭 27.4%（23/84），栓塞 46.4%（39/84）。首診誤診率為 73.8%，上呼吸道感染、感染原因待查、下呼吸道感染是較常見的誤診病種。多因 IE 易患因素（P=0.001）、血管征象（P=0.001）、心臟雜音（P=0.034）、栓塞（P=0.004）等臨床表現未引起醫師重視而出現誤診，且被忽視的次要標準越多，越容易出現誤診（P=0.005）。結論對于不明原因反復發熱、心臟雜音、合并血管栓塞等征象和各種 IE 易患因素的患者，應警惕 IE 的可能，及時采用 IE 診斷基石。臨床醫師應提高對 IE 的認識，注意加強多專業協作，從而減少 IE 的誤診。
