目的總結小切口改良螺絲釘內固定在踝關節融合術中的應用效果。方法 2008年7月-2012年1月，收治22例踝關節終末期病變患者，采用小切口改良螺絲釘內固定行踝關節融合術。男12例，女10例；年齡45～67歲，平均55.4歲。創傷性關節炎8例；腓總神經損傷晚期足下垂，行肌腱轉移術效果不佳3例；骨關節炎嚴重退變4例；類風濕性關節炎2例；踝關節細菌性關節炎靜止1年以上2例；距骨壞死2例；Charcot關節病1例。病程36～75個月，平均52個月。結果術后切口均Ⅰ期愈合。22例均獲隨訪，隨訪時間13～18個月，平均15.2個月。X線片示患者均獲骨性融合，20例融合時間10～22周，平均14.3周；2例延遲融合，術后12個月獲骨性融合。術后13個月美國矯形足踝協會（AOFAS）踝關節功能評分為（71.2±7.3）分，較術前（40.9±9.1）分顯著改善（t=-2.974，P=0.000）。結論小切口改良螺絲釘內固定具有創傷小、固定牢固、適應證廣、融合率高、并發癥少等優點，是一種較好的踝關節融合術式。
