目的總結5·12汶川大地震期間5例擠壓綜合征（crush syndrome，CS）患者傷后中期出現遲發性肌肉壞死的可能原因、發病機制及臨床治療措施。方法 5·12汶川大地震期間收治5例CS患者，男3例，女2例；年齡15～35歲，平均23.4歲。肢體共17個受壓部位，受壓時間21～72 h，平均36 h。入院時病情危重，均表現為急性腎功能衰竭和肝功能損害，合并多部位、大面積感染創面。傷后中期創面動脈性大出血后出現寒戰、高熱等全身中毒癥狀，隨后出現遲發性肌肉壞死；傷后晚期壞死肌群與周圍正常肌群形成明顯界限并自然脫離，當創面肉芽屏障形成后，心率、體溫逐漸恢復正常。全身治療主要為糾正肝、腎功能衰竭，抗感染、支持、輸血等，局部早期加強引流、結合手術擴創，中期注意有限擴創，晚期進行創面換藥及植皮覆蓋創面。結果治療后5例患者創面均愈合，皮膚干燥，無滲液、紅腫、壞死、裂開及皮下積膿，愈合時間10～21個月，平均15.2個月。5例獲隨訪13～45個月，平均19.6個月。均康復出院；隨訪期間無死亡，可佩戴假肢或扶拐活動。結論地震CS遲發性肌肉壞死可能是多因素共同作用的結果，具體機制有待進一步研究。
