目的探討一種新型的改良小切口小骨窗顯微術式治療Chiari畸形合并脊髓空洞癥的療效。方法總結2010年1月－2012年9月采用的改良后4 cm小切口、3 cm×2 cm小骨窗減壓、小腦扁桃體切除、枕大池擴大重建術治療的共計268例Chiari畸形合并脊髓空洞癥患者的治療及預后情況，并進行回顧性分析。結果 268例患者中早期癥狀明顯改善221例（82.5%），遠期隨訪245例癥狀明顯改善212例（86.5%）、穩定25例（10.2%）、加重8例（3.3%）。結論小切口小骨窗顯微術式在治療Chiari畸形合并脊髓空洞癥中有著顯著的療效。
