目的探究擺位系統誤差對乳腺癌患者實施調強放射治療的劑量學影響。方法 2012年10月對一典型乳腺癌改良根治術后患者分別設計常規三維適形切線野（CRT）、多野靜態調強（s-IMRT）、旋轉調強（VMAT）3種不同治療技術的計劃，在計劃中將治療中心點向患者左、右、腹、背、頭、腳方向分別平移3、6 mm模擬2種不同程度的系統誤差，共計39個計劃與原計劃進行比較，觀察不同治療技術間、不同平移距離，不同方向上的靶區及主要危及器官患側肺和心臟的劑量變化。結果對于3 mm擺位系統誤差，靶區D95%于向背側s-IMRT降低4.0%，VMAT降低3.5%，向右（患側）s-IMRT降低3.0%，VMAT降低2.8%，其余均方向降低且＜1.6%。對于主要危及器官，系統誤差對s-IMRT技術的患側肺接受20 Gy劑量的體積影響大，對VMAT技術的患側肺接受10 Gy劑量的體積影響大。除此外，對主要危及器官患側肺及其他劑量參數影響均小（3 mm＜3%，6 mm＜6%）。系統誤差對3D-CRT技術的影響小（3 mm＜3%，6 mm＜6%）。6 mm的系統誤差對靶區及危及器官的影響與3 mm趨勢一致，僅變化程度更大，兩種調強技術的靶區D95%降低＞5%。結論對乳腺癌改良根治術后的呈弧段狀、位于胸廓表面的放射治療靶區，s-IMRT和VMAT方式的調強治療技術對治療擺位系統誤差的影響程度相當，但較CRT技術更敏感，其中以向患者背側、健側方向偏移影響最大。改良根治術后乳腺癌調強治療的實施需要有較三維治療更高的位置精度保證。
