目的探討慢性阻塞性肺疾病（COPD）合并抑郁患者與呼吸衰竭、肺性腦病發生的關系。方法隨機選取2009年10月－2011年10月住院期間COPD患者共92例，行抑郁自評量表及漢密爾頓抑郁量表測評，記錄住院期間血氣分析結果。結果漢密爾頓抑郁量表研究計算入選92例患者中合并抑郁患者53例，抑郁發病率57.6%，其中37例（69.8%）為中重度抑郁。COPD抑郁患者合并呼吸衰竭者33例，合并肺性腦病表現24例。呼吸衰竭、肺性腦病的發生率與COPD患者抑郁狀態有關，合并抑郁狀態的COPD患者發生呼吸衰竭的可能性是單純COPD患者的2.5倍，發生肺性腦病的可能性是單純COPD患者的1.7倍。結論 COPD合并抑郁患者較單純COPD患者易發生呼吸衰竭及肺性腦病，肺性腦病產生的神經精神異常進一步加重抑郁情緒。
